OMHA Tournament Package 2011-2012<br />
Letter from Tournament Chairperson<br />
As per the OMHA Tournament Regulations, reconciliation of your tournament is<br />
due within 30 days of the completion of your tournament.<br />
The OMHA requires the following information for reconciliation:<br />
1. Reconciliation Form (this is included with your tournament package). If<br />
a new form is required, please contact the OMHA office and they would<br />
be happy to supply you with a new one.<br />
2. Schedule of all games completed as part of the tournament.<br />
3. Tournament <strong>Program</strong> (listing all participating teams).<br />
4. Unused game sheets.<br />
It is extremely important to reconcile all tournaments as failure to do so, may<br />
result in refusal of sanctioning future tournaments. No refunds will be issued<br />
30 days after the completion of your tournament.<br />
