Your membership in the BCGEU entitles you to several benefits for yourself and your family members
through American Income Life Insurance Company (AIL in Canada). Every BCGEU member is already
provided the following benefit at no cost through AIL in Canada.

You are under no obligation to complete the enclosed card or the online registration. But you should do
so if you want to designate the beneficiary of the accidental death and dismemberment benefit, want
to receive a certificate of coverage or more information on additional supplemental insurance benefits
offered through AIL, a 100% unionized company.
In case of accidental death or dismemberment, please contact the AIL office at 1-866-472-0825 .

arrange a convenient time to deliver your certificate of coverage and no cost benefits and explain
