are not required to return this form with their proposal. Only those bidders awarded grants<br />
will be required to execute the Business Associate Agreement.)<br />
3.4 Cost Response Requirements (Budget Worksheet and narrative): Bidders shall complete the<br />
Budget Worksheet, attached to the <strong>RFR</strong> as Attachment C. Bidders must also submit narrative (no<br />
more than one page) to clarify any elements of their Budget Worksheet.<br />
SECTION 4.<br />
RESPONSE EVALUATION PROCESS<br />
Responses to this <strong>RFR</strong> shall be evaluated by an Evaluation Committee (also known as a “Procurement<br />
Management Team,” or PMT) appointed by EOHHS.<br />
4.1 Response Review and Evaluation Criteria<br />
A. Initial Review: All responses shall be initially reviewed to determine compliance with the general<br />
response submission instructions in Section 3.1 and inclusion of the mandatory forms described in<br />
Section 3.3. Responses that meet those requirements shall have their Programmatic (Section 3.2) and<br />
Cost (Section 3.4) responses reviewed and evaluated by the Committee against the criteria in<br />
Sections 4.1.B and C, below.<br />
B. Programmatic Response Review: The Committee shall qualitatively rate each Programmatic<br />
Response by evaluating each element for comprehensiveness, appropriateness, feasibility, clarity,<br />
effectiveness, innovation, and responsiveness to the needs of EOHHS. In addition to the<br />
Programmatic response, the Committee may consider any relevant information about the bidder<br />
known to EOHHS.<br />
C. Cost Response Review: Each Cost Response shall be reviewed and rated by the Committee for its<br />
reasonableness, effectiveness, and ability to identify those resources, both grant-related and internal,<br />
to be used in the bidder’s project implementation.<br />
D. SOMWBA: The Committee shall give preference to any bidder’s aggregate Programmatic and<br />
Budget response rating over an equal aggregate rating where the bid is submitted by an organization<br />
that is certified as a Minority Business Enterprise (MBE) by the State Office of Minority and Women<br />
Business Assistance (SOMWBA). See Section 6.2.<br />
E. Noncompliance: EOHHS reserves the right to reject a bidder’s response at any time during the<br />
evaluation process if the bidder:<br />
• Fails to demonstrate to EOHHS’s satisfaction that it meets all <strong>RFR</strong> requirements;<br />
<strong>RFR</strong> for MassHealth and Commonwealth Care Enrollment Outreach Grants<br />
