with the giz*<br />

Target: <strong>CompWare</strong> <strong>Medical</strong> ® is realizing PPPs<br />

(public private partnership) in India, Nepal<br />

and Malaysia in cooperation with the giz* and<br />

other partners.<br />

The project aims at establishing model<br />

treatment sites for OST which are supposed<br />

to offer an integrated substitution therapy.<br />

Prisons <strong>as</strong> well <strong>as</strong> clinics are going to be<br />

included in the project.<br />

The therapy covers the regular supply of<br />

methdone / buprenorphine to the patients and<br />

above all the treatment of HIV and TB patients.<br />

The project comprises the installation of<br />

MeDoSys ® and the training of the staff.<br />

MeDoSys ® is an automatic IT-b<strong>as</strong>ed<br />

comprehensive system for dosing and<br />

dispensing methadone and documenting the<br />

administration of buprenorphine tablets.<br />

It h<strong>as</strong> been designed to manage the largescale<br />

opioid substitution treatment of drug<br />

users primarily in treatment centers, hospitals,<br />

clinics and prison settings.<br />

Challenge: Apart from implementing the<br />

prototype of an outpatient clinic, there are still<br />

more challenges to face: The medical<br />

personnel h<strong>as</strong> to be trained and made familiar<br />

with the substitution therapy, methadone h<strong>as</strong><br />

to be approved <strong>as</strong> a drug in India, safe transport<br />

h<strong>as</strong> to be defined and legal conditions<br />

have to be created.<br />

The giz's job is to keep in touch with the<br />

authorities and ministries and to create the<br />

b<strong>as</strong>ic conditions required for running an outpatient<br />

clinic on an official b<strong>as</strong>is.<br />

The giz will send a doctor who is to support<br />

the medical part of the project on site from<br />

the beginning to the end.

<strong>CompWare</strong> <strong>Medical</strong> ® will be responsible for<br />

establishing and organising the outpatient<br />

clinic and for training the personnel who, for<br />

<strong>this</strong> purpose, are supposed to stay with<br />

<strong>CompWare</strong><strong>Medical</strong> ® in Germany for several<br />

weeks. Furthermore, the company will provide<br />

the documentation and dosing system. The<br />

necessary long-term support on site will be<br />

ensured by local partner companies.<br />

Second level service will be guaranteed by<br />

of improving the quality and safety in OST medicine.<br />

Server for OST drug prescriptions for the<br />

Federal Ministry of Health and Federal<br />

Opium Agency<br />

Target: The server for OST drug prescriptions<br />

is supposed to facilitate the process of ordering<br />

drug prescriptions by means of digital data<br />

exchange, at the same time guaranteeing a<br />

higher level of safety <strong>as</strong> compared to the<br />

former system. This procedure is to optimise<br />

the safety of the OST drug prescription and<br />

distribution process, while significantly<br />

reducing the administrative work to be done<br />

by the physicians and the Federal Opium<br />

Agency, a department of the Federal Institute<br />

for Drugs and <strong>Medical</strong> Devices.<br />

Challenge: The prescription server is b<strong>as</strong>ed<br />

on an electronic procedure to order, issue<br />

and cancel safety marks which replace<br />

complete drug prescriptions.<br />

In future, even standard prescription forms<br />

will have these safety marks and thus become<br />

drug prescriptions. 100 000 physicians and<br />

31 000 pharmacies should be connected to<br />

<strong>this</strong> system.<br />

Success: Our test system is running smoothly,<br />

which means that the first step of the project<br />

development h<strong>as</strong> been successfully completed.<br />

The next step is the integration of the project<br />

into the new nationwide electronic prescription<br />

system.<br />

Automatic data transfer for the Central<br />

Data Registration System of the German<br />

Federal Opium Agency<br />

Target: The project's target is to establish a<br />

system which allows the electronic encryption<br />

and transfer of data to the central register of<br />

the Federal Opium Agency for the registration<br />

and deregistration of OST patients.<br />

It is supposed to prevent unintended double<br />

dosings.<br />

The former complicted and error-prone manual<br />

encryption of patient data according to the<br />

BtMVV (Narcotic Drugs Prescription Guidelines)<br />

is to get simpler and safer. The shipping<br />

by mail is to become a thing of the p<strong>as</strong>t.<br />

Challenge: For the data transfer via Internet<br />

<strong>CompWare</strong> <strong>Medical</strong> ® had to develop a safe<br />

and digital method of identifying all those<br />

involved in the substitution process.<br />

Any doctor authorized to carry out substitution<br />

therapies had to be registered.<br />

Success: For several years, the automatic<br />

registration and deregistration of the<br />

substitution patients h<strong>as</strong> been carried out<br />

successfully. Unintended double dosings do<br />

no longer occur. Only authorized doctors can<br />

carry out the substitution treatment.<br />

HepatitisC SUPPORT in cooperation with<br />

Shering Plough<br />

Target: Of all the illnesses afflicting opiate<br />

users, Hep C is one of those that is difficult to<br />

treat.<br />

Recent regulations are now recommending<br />

a therapy of <strong>this</strong> illness which often accompanies<br />

drug addiction. In order to <strong>as</strong>sure the<br />

quality of <strong>this</strong> treatment, <strong>CompWare</strong> <strong>Medical</strong> ®<br />

w<strong>as</strong> <strong>as</strong>ked to develop a software solution<br />

designed to support and document the entire<br />

therapy within a study.<br />

Challenge: In order to make the therapy of<br />

drug addicts more effective, the treatment<br />

demands an intense medical attendance, with<br />

the patients seeing the doctor at regular intervals.<br />

The software solution SUPPORT w<strong>as</strong><br />

expected to back up all details of the therapy<br />

(the treatment of substitution patients with<br />

PegIntron ® and Rebetol ® ). The programme<br />

w<strong>as</strong> to support the therapy by structuring the<br />

complex treatment and documenting the<br />

Success: Having successfully completed the<br />

stage of development, SUPPORT started in<br />

practice in October 2005.<br />

chronic hepatitis C have been treated in 60<br />

medical institutions.
