Introducing the new
South East Glasgow
Community Health and
Social Care Partnership

Introduction
NHS Greater Glasgow and Glasgow City Council have
joined forces to create a new Community Health and
Social Care Partnership for South East Glasgow.
This leaflet is designed to provide information on this
new Partnership, including details of the Public
Partnership Forum which is being created to give you
a greater say in the development of your local services.
What are Community Health
and Social Care Partnerships
Community Health Partnerships,
or CHPs for short, are the new
organisations which are being
developed across Scotland to
manage a wide range of
community based health
services. In some parts of the
country, including Glasgow,
these new organisations will
also be responsible for many
local social care services and
will therefore be called
Community Health and Social
Care Partnerships (CHSCPs).
Why are these new
Partnerships being created
To develop closer links between
community health, social care
and specialist hospital based
services. CHPs will build on the
work of the Local Health Care
Co-operatives (LHCCs), which
were previously responsible for
managing local health services.
These new Partnerships,
however, will be larger
organisations with wider
responsibilities and greater
influence. They will also play
a lead role in improving the
health of local people by
working closely with local
housing, leisure and education
services and other organisations
responsible for training
and employment.
Who will run this new
Partnership
The Partnership will be run
jointly by NHS Greater Glasgow
and Glasgow City Council and
will be led by a Director who
will report to both partner
organisations.

What services will the
new Partnership be
responsible for
The new Partnership will be
responsible for a wide range
of health and social care
services provided in local
health centres, clinics and
social work offices across
South East Glasgow. These
include services provided by
health visitors, social workers,
district nurses, school nurses,
community physiotherapists,
dieticians and podiatrists (foot care specialists). The Partnership
will also be responsible for local learning disability, mental health
and addiction services in the South East of the city and will work
closely with local GPs, pharmacists, dentists and opticians.
How many CHPs are being developed across Greater Glasgow
Nine CHPs are being developed across Greater Glasgow. Five of
these are within the boundaries of Glasgow City Council,
including one covering the South East of the city.
What area does the new South East Glasgow Community
Health and Social Care Partnership cover
The new South East Glasgow Community Heath and Social Care
Partnership covers the South East of the city including the
following communities:
Carmunnock, Castlemilk, Croftfoot, Cathcart, Newlands,
Shawlands, Kingspark, Battlefield, Toryglen, Crosshill,
Strathbungo, Govanhill, Gorbals, Pollokshields, Dumbreck,
Kingston, Gorbals and Hutchesontown.
The present boundaries for the Partnership may be altered
slightly to reflect future changes to Glasgow Community Planning
Partnership boundaries and Glasgow City Council electoral
ward boundaries.

What difference will this new Partnership make to patients
The South East Glasgow Community Health and Social Care
Partnership will develop closer partnerships with local council and
hospital based health services to break down some of the barriers
that have traditionally existed between community health,
social care and specialist health services. This will create new
opportunities to develop additional health services at a local
level. It will also make it easier for you to access the local services
you require regardless of whether these are provided by your
CHSCP, local hospital or local council.
How will I access local health and social care services
in the future
Local health and social care services will continue to be provided
in local health centres, clinics and social work offices across South
East Glasgow.
How will the new Partnership be developed
A draft ‘Scheme of Establishment’, which sets out the broad
principles for the development of the five new Glasgow CHSCPs,
including details of the services they will manage and how these
will be organised, has been submitted to the Scottish Executive
for final approval.
A local Development Group has also been formed to develop the
new South East Glasgow Community
Health and Social Care Partnership
over the next year, working closely
with local staff, community groups
and others with an interest in the
development of local health and
social care services.

How can I get involved in the development of local health and
social care services
All CHPs are required to develop a Public Partnership Forum (PPF)
by 1 April 2006. This will give service users, carers, local
community groups and voluntary organisations an opportunity
to get involved in the development of local services and provide
feedback on a wide range of local issues. The PPF will build on
existing local patient and public involvement arrangements to
develop closer links with community groups and voluntary
organisations across South East Glasgow. If you would like to
