<strong>Introducing</strong> <strong>the</strong> <strong>new</strong><br />
North <strong>Glasgow</strong><br />
Community Health <strong>and</strong><br />
Social Care Partnership
Introduction<br />
<strong>NHS</strong> <strong>Greater</strong> <strong>Glasgow</strong> <strong>and</strong> <strong>Glasgow</strong> City Council have<br />
joined forces to create a <strong>new</strong> Community Health <strong>and</strong><br />
Social Care Partnership for North <strong>Glasgow</strong>. This leaflet<br />
is designed to provide information on this <strong>new</strong><br />
Partnership, including details of <strong>the</strong> Public Partnership<br />
Forum which is being created to give you a greater<br />
say in <strong>the</strong> development of your local services.<br />
What are Community Health however, will be larger<br />
<strong>and</strong> Social Care Partnerships<br />
Community Health Partnerships,<br />
or CHPs for short, are <strong>the</strong> <strong>new</strong><br />
organisations which are being<br />
developed across Scotl<strong>and</strong> to<br />
manage a wide range of<br />
community based health<br />
services. In some parts of <strong>the</strong><br />
country, including <strong>Glasgow</strong>,<br />
<strong>the</strong>se <strong>new</strong> organisations will<br />
also be responsible for many<br />
local social care services <strong>and</strong><br />
will <strong>the</strong>refore be called<br />
Community Health <strong>and</strong> Social<br />
Care Partnerships (CHSCPs).<br />
Why are <strong>the</strong>se <strong>new</strong><br />
Partnerships being created<br />
To develop closer links between<br />
community health, social care<br />
<strong>and</strong> specialist hospital based<br />
services. CHPs will build on <strong>the</strong><br />
work of <strong>the</strong> Local Health Care<br />
Co-operatives (LHCCs), which<br />
were previously responsible for<br />
managing local health services.<br />
These <strong>new</strong> Partnerships,<br />
organisations with wider<br />
responsibilities <strong>and</strong> greater<br />
influence. They will also play<br />
a lead role in improving <strong>the</strong><br />
health of local people by<br />
working closely with local<br />
housing, leisure <strong>and</strong> education<br />
services <strong>and</strong> o<strong>the</strong>r organisations<br />
responsible for training<br />
<strong>and</strong> employment.<br />
Who will run this <strong>new</strong><br />
Partnership<br />
The Partnership will be run<br />
jointly by <strong>NHS</strong> <strong>Greater</strong> <strong>Glasgow</strong><br />
<strong>and</strong> <strong>Glasgow</strong> City Council <strong>and</strong><br />
will be led by a Director who<br />
will report to both partner<br />
organisations.
What services will <strong>the</strong><br />
<strong>new</strong> Partnership be<br />
responsible for<br />
The <strong>new</strong> Partnership will be<br />
responsible for a wide range<br />
of health <strong>and</strong> social care<br />
services provided in local<br />
health centres, clinics <strong>and</strong><br />
social work offices across<br />
North <strong>Glasgow</strong>. These include<br />
services provided by health<br />
visitors, social workers, district<br />
nurses, school nurses,<br />
community physio<strong>the</strong>rapists,<br />
dieticians <strong>and</strong> podiatrists (foot care specialists). The Partnership<br />
will also be responsible for local learning disability, mental health<br />
<strong>and</strong> addiction services in <strong>the</strong> North of <strong>the</strong> city <strong>and</strong> will work<br />
closely with local GPs, pharmacists, dentists <strong>and</strong> opticians.<br />
How many CHPs are being developed across <strong>Greater</strong> <strong>Glasgow</strong><br />
Nine CHPs are being developed across <strong>Greater</strong> <strong>Glasgow</strong>. Five of<br />
<strong>the</strong>se are within <strong>the</strong> boundaries of <strong>Glasgow</strong> City Council,<br />
including one covering <strong>the</strong> North of <strong>the</strong> city.<br />
What area does <strong>the</strong> <strong>new</strong> North <strong>Glasgow</strong> Community Health<br />
<strong>and</strong> Social Care Partnership cover<br />
The <strong>new</strong> North <strong>Glasgow</strong> Community Heath <strong>and</strong> Social Care<br />
Partnership covers <strong>the</strong> North of <strong>the</strong> City including <strong>the</strong> <strong>the</strong><br />
following communities:<br />
Balornock, Barmulloch, Blackhill, Garngad, Hamiltonhill, High<br />
Possil, Port Dundas, Maryhill, Ruchill, Springburn, Summerston<br />
<strong>and</strong> Woodside.<br />
The present boundaries for <strong>the</strong> Partnership may be altered<br />
slightly to reflect future changes to <strong>Glasgow</strong> Community Planning<br />
Partnership boundaries <strong>and</strong> <strong>Glasgow</strong> City Council electoral<br />
ward boundaries.
What difference will this <strong>new</strong> Partnership make to patients<br />
The North <strong>Glasgow</strong> Community Health <strong>and</strong> Social Care<br />
Partnership will develop closer partnerships with local council<br />
<strong>and</strong> hospital based health services to break down some of <strong>the</strong><br />
barriers that have traditionally existed between community<br />
health, social care <strong>and</strong> specialist health services. This will create<br />
<strong>new</strong> opportunities to develop additional health services at a local<br />
level. It will also make it easier for you to access <strong>the</strong> local services<br />
you require regardless of whe<strong>the</strong>r <strong>the</strong>se are provided by your<br />
CHSCP, local hospital or local council.<br />
How will I access local health <strong>and</strong> social care services<br />
in <strong>the</strong> future<br />
Local health <strong>and</strong> social care services will continue to be provided<br />
in local health centres, clinics <strong>and</strong> social work offices across<br />
North <strong>Glasgow</strong>.<br />
How will <strong>the</strong> <strong>new</strong> Partnership be developed<br />
A draft ‘Scheme of Establishment’, which sets out <strong>the</strong> broad<br />
principles for <strong>the</strong> development of <strong>the</strong> five <strong>new</strong> <strong>Glasgow</strong> CHSCPs,<br />
including details of <strong>the</strong> services <strong>the</strong>y will manage <strong>and</strong> how <strong>the</strong>se<br />
will be organised, has been submitted to <strong>the</strong> Scottish Executive<br />
for final approval.<br />
A local Development Group has been formed to develop <strong>the</strong> <strong>new</strong><br />
North <strong>Glasgow</strong> Community Health <strong>and</strong> Social Care Partnership<br />
over <strong>the</strong> next year, working closely with local staff, community<br />
groups <strong>and</strong> o<strong>the</strong>rs with an interest in <strong>the</strong> development of local<br />
health <strong>and</strong> social care services.
How can I get involved in <strong>the</strong> development of local health <strong>and</strong><br />
social care services<br />
All CHPs are required to develop a Public Partnership Forum (PPF)<br />
by 1 April 2006. This will give service users, carers, local<br />
community groups <strong>and</strong> voluntary organisations an opportunity<br />
to get involved in <strong>the</strong> development of local services <strong>and</strong> provide<br />
feedback on a wide range of local issues. The PPF will build on<br />
existing local patient <strong>and</strong> public involvement arrangements to<br />
develop closer links with community groups <strong>and</strong> voluntary<br />
organisations across North <strong>Glasgow</strong>.<br />
