(Homeless Students)<br />

Note to <strong>School</strong> Personnel: This form is to be filled out whenever any school personnel denies, in whole or in<br />

part, any request for an educational service (e.g., enrollment, transportation, tutoring, waiver of school fees,<br />

transfer of school records, school supplies, medical or social services referrals, etc.), whether written or verbal,<br />

made by a homeless student or his or her parent or guardian, or takes any other adverse action regarding a<br />

homeless student. The completed form must be given to the parent or guardian in every such instance. A copy<br />

of the completed form must also be faxed to the Homeless Education Office at 301-925-2534.<br />

Notice of Right to Appeal<br />

You have a right to appeal this denial. If you want to appeal, fill out the Appeal Form that is attached. Then<br />

give the form to the principal at your child’s school.<br />

The principal will reconsider the decision. The principal will make a new decision as soon as possible, but not<br />

more than five (5) school days after the date you turn in the Appeal Form. The principal could grant your<br />

request, or the principal could uphold the denial. The principal must tell you about the decision in writing.<br />

If you do not agree with the principal’s decision, you can appeal it to the Office of Appeals which is located at<br />

14201 <strong>School</strong> Lane, Upper Marlboro, MD 20772. You will need to complete an appeal form, which is attached<br />

to the Notice of Denial. The telephone number for the Office of Appeals is (301) 952-6195. The fax number<br />

is (301) 952-6765.<br />

The Office of Appeals will reconsider the decision again. They will make a new decision as soon as possible<br />

but not more than ten (10) school days after they receive your appeal. The Office of Appeals could grant your<br />

request, or they could uphold the denial. The Office of Appeals must tell you about their decision in writing.


If you do not agree with the decision of the Office of Appeals, you may appeal to the Board of Education which<br />

is located at 14201 <strong>School</strong> Lane, Upper Marlboro, MD 20772. You will need to complete a new appeal form,<br />

which will be attached to the Decision of the Office of Appeals. The telephone number at the Board of<br />

Education is 301-952-6116. The fax number is (301) 952-6114. You will be given an opportunity for a hearing<br />

before the Board of Education (or its appeal panel) to explain why you disagree with the denial of your request.<br />

The Board of Education of <strong>Prince</strong> George’s <strong>County</strong> will reconsider the decision again. They will make a<br />

new decision as soon as possible but not later than thirty (30) school days after they receive your appeal. The<br />

Board of Education could grant your request, or they could uphold the denial. The Board of Education must tell<br />

you about their decision in writing.<br />

If you do not agree with the decision of the <strong>Prince</strong> George’s <strong>County</strong> Board of Education, you can appeal it to<br />

the Maryland State Board of Education. Send a letter (keep a copy for yourself), along with a copy of the <strong>Prince</strong><br />

George’s <strong>County</strong> Board of Education’s decision to:<br />

Transportation<br />

If the appeal concerns placement, your child may continue to attend the school you have selected<br />

during the appeals process. Your child will receive transportation to the school of origin, if that is the<br />

school you have selected. If the appeal concerns your child’s right to receive transportation, he or she<br />

will continue to receive this service during the appeal process.<br />
