We are pleased to let you know that all active <strong>member</strong>s and retirees have an IMPORTANT BENEFIT PROVIDED TO YOU AT<br />
NO COST! This supplemental insurance is being provided through <strong>American</strong> <strong>Income</strong> <strong>Life</strong> Insurance Company (AIL). Unifor<br />
Local 26 is proud to jointly sponsor this <strong>benefit</strong> with AIL, a 100% Union label company , whose <strong>member</strong>s are with Unifor 247.<br />
This <strong>benefit</strong> includes the following:<br />
Accidental Death and Dis<strong>member</strong>ment Benefit<br />
$4,000.00 for EACH Member & Retiree<br />
AIL ASKS EVERY MEMBER & RETIREE TO RETURN THE ATTACHED CARD TO HAVE YOUR<br />
CERTIFICATE OF COVERAGE DELIVERED AND TO DESIGNATE YOUR BENEFICIARY.<br />
Return the attached reply card in the postage paid envelope in order to have your certificate delivered to you<br />
by an AIL representative and your proper beneficiary designated. Re<strong>member</strong>, if you do not designate<br />
your beneficiary, the $4,000 would be paid to your estate and may be taxable.<br />
All <strong>member</strong>s who return this card will receive an option to increase their coverage an<br />
additional $10,000 with a cost of $2 for the first year and $5 every subsequent year.<br />
In addition, once you return your reply card you may request at no cost a Family Eye Care discount card. Any <strong>member</strong> of your<br />
immediate family can use this card.<br />
AIL currently has over 140,000 UNIFOR <strong>member</strong>s covered under AD&D policies at no cost. To date, over $3.4 Million in life,<br />
health, and no cost AD&D <strong>benefit</strong>s have been paid to over 3,200 Unifor families (formally CEP and CAW <strong>member</strong>s).<br />

The insurance programs being offered to you today are made available on a voluntary basis through<br />
the cooperation of UNIFOR Local 26 and <strong>American</strong> <strong>Income</strong> <strong>Life</strong> Insurance Company, a 100% union<br />
label company with a long-standing history of serving unions and their <strong>member</strong>s.<br />
AIL’s supplemental <strong>benefit</strong>s are not in competition with any group <strong>benefit</strong>s through your workplace/<br />
employer and can complement any insurance policies you may already have. If you choose to take<br />
advantage of any insurance programs offered through AIL, your <strong>benefit</strong> program is permanent and<br />
portable throughout your lifetime.<br />
