What service is provided<br />

There are many potential causes of memory<br />

loss and many people in our community<br />

experience memory difficulties.<br />

Our aim is to identify the cause of your<br />

memory problem through an assessment that<br />

is tailor made to suit your needs.<br />

We will also aim to provide you with possible<br />

treatments and offer you and your family<br />

ongoing support and advice.<br />

Over the course of a two hour clinic<br />

appointment we will provide a comprehensive<br />

assessment, treatment pathway and care plan.<br />

Your assessment will be treated with the<br />

strictest confidence and your privacy and<br />

personhood maintained.<br />

To access the necessary resources your<br />

assessment will normally take place at <strong>Tiverton</strong><br />

Hospital. However, it may be possible to<br />

arrange this at home in certain circumstances.<br />

We will aim to be as flexible as the service<br />

allows ensuring your individual needs are met.<br />

What will happen<br />

If you would like an assessment of your<br />

memory ask your GP to refer you to the<br />

<strong>Tiverton</strong> <strong>Memory</strong> <strong>Clinic</strong>.<br />

Your GP will conduct a range of blood tests<br />

and undertake a physical examination. The<br />

memory clinic consultant may arrange for you<br />

to have a scan of your brain, before coming to<br />

see us.<br />

A detailed medical and personal history will be<br />

taken and all your concerns and observations<br />

noted.<br />

You and your relative/friend will then meet<br />

with the consultant to discuss your diagnosis<br />

and possible treatment options.<br />

Following your assessment in the memory clinic<br />

we will develop a plan with you to tackle any<br />

memory problems we find. We have a range<br />

of support to offer, including medications for<br />

memory, one-to-one support and access to our<br />

groups to help manage memory problems.<br />

The memory clinic team<br />

The team is made up of specialist professionals<br />

who have experience working with memory<br />

related ill health. They are able to offer a<br />

range of assessments, treatment, support and<br />

advice depending on your identified needs.<br />

The team consists of:<br />

• Community Psychiatric Nurses<br />

• Community Support Workers<br />

• A Consultant Psychiatrist<br />

• A Psychologist and Assistant Psychologist<br />

• Occupational Therapists.<br />

You may be seen by some or all members of<br />

the team throughout your assessment.<br />

We use your records to help plan and guide<br />

your care. They also help to ensure:<br />

• doctors and nurses and other health<br />

professionals working with you have<br />

accurate and up-to-date information<br />

• full information is available if you see<br />

another doctor, or are referred to a<br />

specialist or another part of the <strong>NHS</strong><br />

• there is a good basis for assessing the type<br />

and quality of care you have received<br />

• we can investigate your concerns, if you<br />

have a question or need to complain.
