What service is provided
There are many potential causes of memory
loss and many people in our community
experience memory difficulties.
Our aim is to identify the cause of your
memory problem through an assessment that
is tailor made to suit your needs.
We will also aim to provide you with possible
treatments and offer you and your family
ongoing support and advice.
Over the course of a two hour clinic
appointment we will provide a comprehensive
assessment, treatment pathway and care plan.
Your assessment will be treated with the
strictest confidence and your privacy and
personhood maintained.
To access the necessary resources your
assessment will normally take place at Tiverton
Hospital. However, it may be possible to
arrange this at home in certain circumstances.
We will aim to be as flexible as the service
allows ensuring your individual needs are met.
What will happen
If you would like an assessment of your
memory ask your GP to refer you to the
Tiverton Memory Clinic.
Your GP will conduct a range of blood tests
and undertake a physical examination. The
memory clinic consultant may arrange for you
to have a scan of your brain, before coming to
see us.
A detailed medical and personal history will be
taken and all your concerns and observations
noted.
You and your relative/friend will then meet
with the consultant to discuss your diagnosis
and possible treatment options.
Following your assessment in the memory clinic
we will develop a plan with you to tackle any
memory problems we find. We have a range
of support to offer, including medications for
memory, one-to-one support and access to our
groups to help manage memory problems.
The memory clinic team
The team is made up of specialist professionals
who have experience working with memory
related ill health. They are able to offer a
range of assessments, treatment, support and
advice depending on your identified needs.
The team consists of:
• Community Psychiatric Nurses
• Community Support Workers
• A Consultant Psychiatrist
• A Psychologist and Assistant Psychologist
• Occupational Therapists.
You may be seen by some or all members of
the team throughout your assessment.
We use your records to help plan and guide
your care. They also help to ensure:
• doctors and nurses and other health
professionals working with you have
accurate and up-to-date information
• full information is available if you see
another doctor, or are referred to a
specialist or another part of the NHS
• there is a good basis for assessing the type
and quality of care you have received
• we can investigate your concerns, if you
have a question or need to complain.
