IntroductionThe <strong>Crisis</strong> <strong>Resolution</strong> <strong>Home</strong> <strong>Treatment</strong>Team (CRHT) is a service set up to supportyou if you are experiencing a severemental health need which could lead toan inpatient admission to a psychiatrichospital.The main aim of the team is to help youmanage and resolve your crisis throughassessment and treatment in your homeenvironment as an alternative to hospitaladmission. We also support you beingdischarged from psychiatric hospital,enabling you to continue your recoveryat home.What we offerWe will work closely with you, yourfamily/carers and your social networksto provide intensive support and care,helping you to draw on your ownstrengths and to help you learn differentways of improving and maintaining yourmental wellbeing. Where appropriate, wewill also help you to access other servicesthat could be relevant to your care, as wellas reviewing your current medicationsand helping with social issues.Whilst under our care you will haveaccess to the Team 24 hours a day, sevendays a week for support and advice. Ourinput will be short term, intensive and isflexible to your current difficulties at theearliest time.Why we are hereYou will have been referred to ourservice for an assessment of your mentalhealth as a matter of urgency. After thisassessment we will make a decision asto whether you will be seen by ourselvesmore regularly or whether we will referyou to other services.What we can doWe operate 24 hours a day, seven daysa week. We visit you at home during theday and early evening. Outside thesehours we offer support by telephoneand this service is available to you,family and carers. We offer support andwill monitor things such as medication,improvements or deterioration in yourhealth. What you require in terms ofguidance and treatment will be discussedwith you and your family or thoseinvolved and you will be instrumentalin telling us what will help you in yourrecovery.Who we are and who youwill seeWe are a team of doctors, nurses, socialworkers, support workers and otherprofessional staff. You will see varyingmembers of our team. We will try tomaintain continuity but this may notalways be possible.We take into consideration personalpreferences when arranging home visits,as far as possible.When we will visit youIf you are taken on by the team you maybe seen every day or even twice a day,if required, for a short period of time.It may be, of course, that you do notrequire us to see you quite so regularlybut the service is intended to provideshort-term, intensive support andmonitoring.We review our input with you on aregular basis.Where we will see youOur aim will be to see you at home.However, if it is more convenient foryou to be seen elsewhere we canaccommodate this request.Some of the people we see may needadmission to hospital but we will try tomaintain your care at home for as longas possible. It is recognised that peoplerecover more quickly if treated at homein familiar surroundings, with friends andfamily close by.
