Welcome to the Children’s Cardiomyopathy Foundation’s Annual <strong>Golf</strong> Classic! I hope you have<br />
a wonderful day golfing for a great cause.<br />
This marks the eighth year CCF has held this golf event, and I am pleased to see the number<br />
of participants and sponsors double over the years. Your unwavering support has made a<br />
tremendous difference in helping to increase awareness of pediatric cardiomyopathy.<br />
<strong>2010</strong> has been a successful year with many highlights that I would like to share with you. A few<br />
months ago, CCF’s Second International Conference on Cardiomyopathy in Children, co-sponsored<br />
by the National Heart, Lung and Blood Institute, was held in Bethesda, Md. 60 of the top<br />
clinicians and researchers from the U.S., United Kingdom, Canada and Australia convened to<br />
identify critical research areas, share best practices, and review new research findings. Since CCF’s<br />
first conference, nearly $1 million has been committed to research on pediatric cardiomyopathy.<br />
Our cardiomyopathy community has grown to 1,400 CCF registered members and 3,670<br />
facebook supporters from around the world. We continue to reach out to more families in<br />
need and have distributed 3,300 pieces of patient education materials to 60 hospitals in<br />
the U.S. and Canada. Our latest patient resource, Ensuring a Good Learning Environment:<br />
A Cardiomyopathy Resource Kit for School Personnel and Parents, is currently in production<br />
and will be ready for distribution to school nurses and parents before the beginning of the<br />
school year. We also continue to build alliances with the top medical centers across the<br />
U.S., which has helped to expand our patient support groups to 11.<br />
Your involvement in CCF’s <strong>Golf</strong> Classic funds these projects and many other activities that help<br />
families living with this disease. I would like to extend a heartfelt thanks to all of you for helping<br />
us to carry out our mission. My hope for the future is that together we will find cures that will<br />
give children with cardiomyopathy a brighter future.<br />

GOLF FORMAT: Each member of the group tees off and the team selects the best<br />
shot from all that were hit. Players then pick up their ball and play another shot from<br />
the selected best ball location. Players continue playing off the best shot until they finish<br />
the hole. The team receives a score for the hole instead of each player. The maximum<br />
score per hole is a double bogey. Players should pick up after two strokes over par.<br />
Please turn your scorecard into the pro shop after play so that your team's<br />
score can be calculated.<br />

About the<br />
CHILDREN’S CARDIOMYOPATHY FOUNDATION<br />
The Children’s Cardiomyopathy Foundation (CCF)<br />
is a national non-profit organization dedicated to<br />
finding causes and cures for pediatric cardiomyopathy.<br />
Cardiomyopathy is the leading cause for sudden cardiac<br />
deaths and heart transplants in children under age 18.<br />
CCF started in 2002 with one family’s determination<br />
to call attention to this poorly understood heart disease<br />
and to take action on the lack of medical progress and<br />
public awareness. Since then, CCF has grown into a<br />
global community of families, physicians and scientists<br />
focused on improving diagnosis, treatment and quality<br />
of life for children affected by cardiomyopathy.<br />
CCF is involved in all aspects of the disease from<br />
research and education to patient support, awareness<br />
and advocacy. The Foundation has made great<br />
strides in bringing together experts from the medical<br />
and scientific fields to advance knowledge on<br />
pediatric cardiomyopathy and in developing support<br />
services for those affected by this chronic disease.<br />
Nearly $1 million has been committed to research and<br />
treatment initiatives in the U.S. and Canada, and CCF<br />
has led the way in establishing a biologic specimen<br />
repository and planning the first international scientific<br />
conference. In the process, CCF has worked with many<br />
esteemed organizations, including the National Heart,<br />
Lung and Blood Institute, American Heart Association<br />
and the National Organization for Rare Diseases.<br />
CCF continues to be an invaluable lifeline to affected<br />
families providing information, resources and guidance.<br />
CCF’s educational materials are now distributed to<br />
more than 70 hospitals in North America.<br />
The Foundation’s hope for the future is that more<br />
lives will be saved, and every affected child will have<br />
a chance to live a full and active life.<br />
