Welcome to the Children’s Cardiomyopathy Foundation’s Annual Golf Classic! I hope you have
a wonderful day golfing for a great cause.
This marks the eighth year CCF has held this golf event, and I am pleased to see the number
of participants and sponsors double over the years. Your unwavering support has made a
tremendous difference in helping to increase awareness of pediatric cardiomyopathy.
2010 has been a successful year with many highlights that I would like to share with you. A few
months ago, CCF’s Second International Conference on Cardiomyopathy in Children, co-sponsored
by the National Heart, Lung and Blood Institute, was held in Bethesda, Md. 60 of the top
clinicians and researchers from the U.S., United Kingdom, Canada and Australia convened to
identify critical research areas, share best practices, and review new research findings. Since CCF’s
first conference, nearly $1 million has been committed to research on pediatric cardiomyopathy.
Our cardiomyopathy community has grown to 1,400 CCF registered members and 3,670
facebook supporters from around the world. We continue to reach out to more families in
need and have distributed 3,300 pieces of patient education materials to 60 hospitals in
the U.S. and Canada. Our latest patient resource, Ensuring a Good Learning Environment:
A Cardiomyopathy Resource Kit for School Personnel and Parents, is currently in production
and will be ready for distribution to school nurses and parents before the beginning of the
school year. We also continue to build alliances with the top medical centers across the
U.S., which has helped to expand our patient support groups to 11.
Your involvement in CCF’s Golf Classic funds these projects and many other activities that help
families living with this disease. I would like to extend a heartfelt thanks to all of you for helping
us to carry out our mission. My hope for the future is that together we will find cures that will
give children with cardiomyopathy a brighter future.

GOLF FORMAT: Each member of the group tees off and the team selects the best
shot from all that were hit. Players then pick up their ball and play another shot from
the selected best ball location. Players continue playing off the best shot until they finish
the hole. The team receives a score for the hole instead of each player. The maximum
score per hole is a double bogey. Players should pick up after two strokes over par.
Please turn your scorecard into the pro shop after play so that your team's
score can be calculated.

• Closest to the Pin (2 nd hole of course 2, 7 th hole of course 3)
• Longest Drive (9 th hole of course 2, 9 th hole of course 3)
• 1 st , 2 nd , 3 rd , 4 th place teams (foursomes with the lowest gross score)

values. Before leaving, please check to see if you are the winning bidder and claim
your items.

About the
CHILDREN’S CARDIOMYOPATHY FOUNDATION
The Children’s Cardiomyopathy Foundation (CCF)
is a national non-profit organization dedicated to
finding causes and cures for pediatric cardiomyopathy.
Cardiomyopathy is the leading cause for sudden cardiac
deaths and heart transplants in children under age 18.
CCF started in 2002 with one family’s determination
to call attention to this poorly understood heart disease
and to take action on the lack of medical progress and
public awareness. Since then, CCF has grown into a
global community of families, physicians and scientists
focused on improving diagnosis, treatment and quality
of life for children affected by cardiomyopathy.
CCF is involved in all aspects of the disease from
research and education to patient support, awareness
and advocacy. The Foundation has made great
strides in bringing together experts from the medical
and scientific fields to advance knowledge on
pediatric cardiomyopathy and in developing support
services for those affected by this chronic disease.
Nearly $1 million has been committed to research and
treatment initiatives in the U.S. and Canada, and CCF
has led the way in establishing a biologic specimen
repository and planning the first international scientific
conference. In the process, CCF has worked with many
esteemed organizations, including the National Heart,
Lung and Blood Institute, American Heart Association
and the National Organization for Rare Diseases.
CCF continues to be an invaluable lifeline to affected
families providing information, resources and guidance.
CCF’s educational materials are now distributed to
more than 70 hospitals in North America.
The Foundation’s hope for the future is that more
lives will be saved, and every affected child will have
a chance to live a full and active life.
