Winter, 2011-2012
All the complicated details
of the attiring and
the disattiring are completed!
A liquid moon
moves gently among
the long branches.
Thus having prepared their buds
against a sure winter
the wise trees
stand sleeping in the cold.
„Winter Trees‟ by William Carlos Williams (1883-1963)
FamousPoetsAndPoems.com
The above poem uses evocative imagery to allude to the
necessary preparation for the winter season and the
peace and calm that ensue once everything is ready and
in place. While each one of us has to confront different
hardships depending on where we may live, the metaphor
is still a valuable one. I wish all of you a calm and
tranquil winter; may you all experience at least a glimpse
of that elusive yet inviting ―… liquid moon‖ that will keep
a watchful eye and herald serenity.
I hope that the summer months were enjoyable and reju-
venating for everyone. It is remarkable that we are once
again approaching the eve of winter. As will become clear
from the various contributing articles penned by our
members, everyone has been busy contributing to the
Academy in a myriad of ways and done so with unwavering
enthusiasm and steadfastness. The energy is palpable
and the sense of commitment unmistakable. I am
certain that we are all grateful for all the hard work and
dedication put forth by our colleagues. Some of us may
even be inspired to become more involved in areas that
are new to us.
The annual meeting in Philadelphia in May 2012 promises
to be an exciting one indeed. I look forward to what
is certain to be a fascinating event and gathering. As
well, the collegiality and warmth that greet us yearly have
become a staple and an ever-exciting object of anticipation.
We may meet again even earlier, in Washington DC
or New York City.
Until then, keep warm and dry. I don‘t expect that any of
you will find yourselves ―…sleeping while standing in the
cold…‖ The image, however, might be a helpful one to
hold on to as we count our blessings and truly appreciate
our bounty and daily comforts.
When Joseph Merlino called to tell
me in 2007 that the AAPDP Nominating
Committee recommended me for
the Presidency I experienced a surge
of multiple affective states- extreme
joy, pride, apprehension, anxiety,
gratitude, inspiration, honor and humility.
I thought of the parting words
of my analyst upon terminating my
training analysis with Marvin Drellich,
who was the Academy President 20 years ago, encouraging
me ―to become more active‖ in the organization. After
co-chairing Academy meetings in Toronto, San Juan and
New York, I was the AAPDP Secretary from 2000-2003
and Past Secretary from 2003-2004, served as President-Elect-Designate
from 2008-2009, President-Elect
from 2009-2010, and President since 2010.
The road to the Presidency has been a long journey of
humbling compromise, collaboration, generativity, chal-
lenges and accomplishments. I am grateful to have the
best group of officers to work with anyone could wish
for: our able, affable and indefatigable Secretary- Jerry
Perman, our fiscally conservative, experienced and wise
Treasurer- Ann Price, and our enthusiastic, creative and
connected President Elect- Michael Blumenfield. Our
elected Trustees (Mariam Cohen, Scott Schwartz, Joseph
Silvio, Kim Best, Debra Katz, Helen Ullrich, Abby
Altman, Eugenio Rothe, Crittenden Brookes), under the
leadership of their representative Mariam Cohen, have
efficiently and thoughtfully tackled complex projects, in
near perfect alignment, to be able to accomplish the
implementation of transformative initiatives that have
positioned the Academy as a more visible, prominent,
and vibrant institution, reclaiming our place at the forefront
of Psychoanalysis, Psychodynamic Psychiatry, and
General Psychiatry, as an active affiliate organization of
the APA; and as a vehicle to fulfill training in psychodynamic
psychotherapy competency in underserved
areas through our fortified liaison with AADPRT (the
American Association of Directors of Psychiatric Residency
Training) - largely due to the leadership of Sherry
Katz-Bearnot, and Gene Beresin.
Lastly, we have been able to work collaboratively and
improved the quality of our communication with our
magnificent and wise Executive Director Jackie Coleman,
and our extraordinarily responsive, meticulous,
creative and efficient Executive Assistant Jacqui Davis.
Those of us who have had the pleasure to rely on their
support and follow their guidance appreciate the immense
help that they provide in a responsive, consistent
and attentive way. Our appreciation also extends to
other staff that may not be as visible but do a great deal
for the Academy- Marie Westlake and Sara Elsden.
As you will read in this newsletter in individual reports,
our committee chairs and members are hard at work
and deserve our greatest admiration and appreciation. I
will not repeat in this column membership and committee
achievements to date to avoid being duplicative. I
deeply appreciate how our task forces and committee
leaders and members harmoniously orchestrate the
work delegated by our Board.
In this editorial I wish to raise awareness of future possibilities
and challenges, rather than being retrospectively
self-congratulatory and simply list all we have accomplished
together over the past few years. In other words,
allow me to be prospective and perhaps even provocative,
sharing my personal views (not the views of the
Executive Council but strictly my private thoughts and
ideas). Hopefully I will stimulate a thoughtful discussion
offering my views of directions or changes we may consider
as an organization. It will be up to our next leaders
to debate, deliberate and act upon my suggestions if
they feel that these are meritorious. By the time this
feature will appear in print I will be close to the conclusion
of my service as President, a journey I have thoroughly
enjoyed.
I understand that some of the ideas I present herein will
stir controversy, but urge you to ponder about them and
consider their possible relevance and usefulness.
Governance
One of the most valuable lessons I have learned in the
past few years is the importance and wisdom of term
limits mandated by the Constitution and Bylaws. We
must allow others to take charge, and learn to humbly
take a step back and observe, offering advice only when
asked, without interfering with the democratic process.
Having said this, we are one of the few professional
organizations where Past Presidents remain members
of the Board in perpetuity, albeit without a vote. While I
greatly value the wisdom and experience of our elders,
there are times in our Board meetings where we have
as many nonvoting members as voting members present.
This, in my personal opinion, has the potential to
create an imbalance of power, especially if the Trustees
and Officers (voting members) are younger and less
experienced and may be unduly influenced and pressured
by the opinions of their mentors and senior colleagues.
A more classless process, such as that followed
by other non-profit professional associations,
would be to create term limits for Past Presidents' participation
in Executive Council, rather than allowing participation
in perpetuity. I understand that this has the
potential to radically change the culture of our governance,
but I urge you to consider the pros and cons of
this idea.
Membership Categories
Most professional organizations our size have fewer
membership categories. Stratification of membership
may lead to inactivity, feelings of alienation and inequality.
Currently, our membership categories include: medical
student members, psychiatric associates, psychiatric
members, psychiatric fellows, psychoanalytic fellows,
scientific associates, and honorary members. Most psychiatric
organizations have a less stratified system – an
example would be simply to have MITs (members in
training, including students and residents), scientific
associates, members, and fellows. A challenge that simplification
of membership categories would pose would
be how to determine which membership categories
would have voting privileges. Currently, only roughly
60% of Academy members have voting privileges.
National Outreach and Recruitment
58% of our membership resides in the Northeast of the
United States, with the South, Midwest and West being
disproportionally represented in our roster. We must
make efforts to try to increase recruitment in underrepresented
areas of the country.
International Outreach and Recruitment
Originally born as the Academy of Psychoanalysis, we
changed to the American Academy of Psychoanalysis,
and then to the American Academy of Psychoanalysis
and Dynamic Psychiatry. To my knowledge, there are no
international organizations representing psychodynamic
psychiatry. At present, 8% of the Academy‘s total membership
is composed of international members. With
initiatives that look to both increase our presence in
international forums and recruit international members,
such as those currently being pursued in various task
forces by our President–Elect, Michael Blumenfield, the
percentage of international members may grow, and we
may find ourselves in a position, in this era of globalization
and blurring of symbolic geographical boundaries,
questioning our cultural identity (national vs. international)
as a group.
Publications and Online Presence
Many medical organizations are giving prominence to
and enhancing their Internet presence. This includes
productive use of membership list serves, electronic
newsletters, e-Journal features such as on-line prepublication
of selected articles, free electronic access to
selected articles and special issues, using social media
to post academic events of interest to our constituents,
etc. Although our three publications, the Academy
Newsletter, Academy Forum, and Psychodynamic Psychiatry
can all be accessed on-line, we may want to
think of ways to further increase our Internet visibility
that could ultimately positively influence the impact factor
of our journal, increase communication, citations,
and visibility.
Maintaining ACCME Self-Accreditation
Our CME mandates specify that our academic activities
must improve our members‘ competence and performance.
This is usually best accomplished by using meeting
formats that are more interactive, dynamic and clinically
relevant. In our annual meetings we should continue
to place greater emphasis on clinically relevant
scientific presentations.
The Academy has always been a self-accrediting organization.
I urge our future leaders to maintain this paradigm,
rather than sub-contracting with other organizations
or institutions to be responsible for granting our
CME credits. The advantages of being self-accrediting
are multiple, allowing for greater flexibility and freedom
in determining content of our programs and establishing
flexible submission deadlines. It is an arduous task to
remain self-accrediting, but one definitely worth our
efforts.
Mentorship and Career Development
Most individuals belong to professional organizations
because these add value to their clinical and academic
practices. We must continue to think creatively of ways
to make membership in the AAPDP more valuable. Mentorship
leading to career development should be systematized
in ways that would increase members‘ satisfaction
and membership retention.
Academic Activities
A small core group of approximately 75 individual members
regularly participate as presenters in our thoroughly
enjoyable annual meetings. The small and intimate
setting of our meetings allows for the development
of effective networks, collegiality and friendship.
Other psychiatry subspecialty organizations have figured
out ways to have a larger percentage of their members
both attend and participate as presenters in annual
meetings. Careful thought should be given to try to figure
out ways to increase meeting registration, which has
consistently averaged 150 registrants per meeting over
the last decade.
In addition to investing effort in our annual meetings, I
am a strong proponent of reaching out and having an
academic presence in other venues. A way to easily accomplish
this is to have Academy sponsored workshops,
symposia and lectures at the annual meetings of diverse
psychiatric organizations, national and international,
and in academic university and medical school
settings throughout the world.
Being AAPDP President has been the highlight of my
professional career. I truly hope I have satisfactorily represented
the Academy‘s constituents dutifully and with
fairness. I welcome an open dialogue with all members
and can be easily reached by electronic mail at
By: Michael Blumenfield, M.D.
The world is flat and probably getting
flatter everyday meaning that
we are in a global era where it is
easier than ever to instantly exchange
ideas and communications
as well as travel throughout
the world . Dr. Alfonso has reported
on the symposium sponsored
by the Academy at the
World Psychiatric Association in
Buenos Aires in September. The
panelists were Drs. Rothe, Olarte,
Goethe and Silvio. In October the
International Psychoanalytic Society will meet in San
Francisco with a workshop sponsored by the Academy
and chaired by Dr. Alfonso which also includes Academy
members Drs. Miriam Cohen, Laura Kent and Sheila
Hafter Gray. Also in October The Chinese American Chinese
Psychoanalytic Alliance (CAPA) which is founded
and directed by Academy member Dr. Elise Snyder will
take off for their 3 rd Study Tour in China. I had the pleasure
of being a co-leader on last year‘s tour and gave two
lectures and witnessed the first graduation of Chinese
students who were trained by American psychoanalysts
and psychodynamic therapists via Skype many of whom
are Academy members. In November the 13 th joint
meeting between the Academy and the Organizzazione
di Psicoanalisti Italiani-Federazione e Registro (OPIFER)
will be held in Rome. Former Academy President Dr.
Joan Tolchin with participation and attendance by many
Academy members has spearheaded these meetings. In
addition Dr. Alfonso is planning an Academy study tour
to Thailand where Academy members will be participating.
These activities are just the tip of the iceberg as
many Academy members are having professional interactions
with our international colleagues through various
other meetings and organizations, journals, telepsychiatry
teaching, visiting lectures and travel.
We are learning of the practice of psychoanalytic and
psychodynamic therapy throughout the world and how
many psychiatrists share the interests of our organization.
This was the impetus for Dr. Alfonso appointing me
the Chair of a task force to examine international membership
in our organization. After some rounds of email
correspondence with this group I was appointed Chair
of second task force of international members to look at
this issue. We had a few more email rounds with this
group. Many ideas are emerging from these discussions
concerning mentorship, mutual hospitality arrangements,
a speakers list to encourage presentations
at international members and other ideas to facilitate
membership in the Academy. In addition our dues
structure was examined by the Executive Council since
the cost of living and income varies in different countries
throughout the world. We looked specifically at the
dues structure used by the APA which assigns reduced
dues based on a ranking by the World Bank. We decided
at this time to use a two tier system where we
would keep the current reduced international membership
dues structure of $225 for countries of upper and
middle income levels rated by the World Bank while
those rated as having lower middle and lower income
levels will have membership dues of $40 per year. We
know that in many of the latter countries the preferred
method for receiving journals and newsletters is the
electronic mode which will make the reduced dues
structure feasible for the Academy.
While I am very excited about the upcoming Academy
meeting in Philadelphia, it
becomes my responsibility to
look ahead to the San Francisco
meeting in 2013.
I have been having discussions
with Dr. Eugenio Rothe
our Academy Program Chair
and we have appointed Dr.
Mary Ann Cohen as the San
Francisco Program Chair. We
are in the process of putting
together the planning committee
and of course it
should be a great meeting in
a wonderful city.
I am happy to report that there is a generation of young
psychiatrists who are joining the Academy. Drs. Scott
Schwartz and César Alfonso have recruited several
whom I have met and am quite impressed. During a
recent Grand Rounds presentation at New York Medical
College I met with the residents and several seemed
interested in joining the Academy. I will continue the
Academy tradition of appointing Early Career Psychiatrists
to various committees. We also have a thriving
mentorship program which gives our more seasoned
members a chance to interact with new members. I
know that both groups have things that they can teach
each other.
We are very fortunate to have so many Past Presidents
of the Academy who continue to be very active in our
organization today. I will call upon them to assist me
when I assume the presidency and use their experience
and creative juices to continue to develop the Academy.
Dr. Alfonso has been doing an outstanding job in leading
the Academy during his two years as President. He
had kept me closely involved in the workings of our organization
and I know he will be a valuable advisor after
I take over following our Philadelphia meeting. I also
want to welcome each of you to not only attend our
meetings, contribute to our publications and become
J. Christopher Fowler, Ph.D., Deputy Director of Research
at the Menninger Clinic in Topeka, KS will be the
presenter at the 19 th Annual Consortium for Psychoanalytic
Research (CPR) on Sunday, February 5, 2012 from
8:30 AM until 4:00 PM. His topic will be Blind Men, Elephants
and Psychotherapy Effectiveness: How Ignoring
Severity of Impairment and Co-morbidity Skews our
View of Treatment Realities. Jointly-sponsored by the
Academy and the Jungian Analysts of Washington Association,
CPRinc is composed of organizations of psychoanalysts
and psychodynamic clinicians from classical to
Jungian to relational and is the only U.S. organization of
its kind. It offers inexpensive CME credits (CME is pending)
and a good lunch! The February CPR conference is
the de facto winter meeting of the Academy.
The research community has tended to view treatment
resistance as a function of a single psychiatric disorder.
But this does not comport with the experience of clinicians
who work with individuals who do not get well despite
their best efforts, particularly since individuals with
treatment resistant conditions have consistently been
excluded from efficacy studies. This conference will attempt
to bridge gaps in our understanding of the cross-
cutting dimensions of ―severity‖ and ―dysfunction‖ and
test whether these phenomena are clinically relevant
behavioral markers for treatment resistance. Attendees
will anonymously rate two of their cases on scales provided
in advance of the meeting and the data will be
analyzed by psychology students during the morning
presentations. Dr. Fowler will present his relevant clinical
research on treatment resistant psychiatric disorders,
describe a global measure of psychiatric severity
(Global Psychiatric Severity Index; GPSI) and discuss the
benefits of a Practice Research Network (PRN: Morey,
1988; Westen, Dutra and Shedler 2005) for clinicians in
private practice. The afternoon session will include an
interactive discussion of the results of the morning research
exercise that will allow participants to selfassess
their approach to their patients. There will be
two clinician-researcher discussants.
The conference will be held in the Hospital Auditorium
of the George Washington University Hospital from 8:30
AM until 4:00 PM. It is being hosted by the G.W. Department
of Psychiatry and the Behavioral Sciences. Additional
Kentucky will deliver this year‘s Presidential
Lecture. The Keynote Speaker
will be the highly sought-after Salman
Akhtar M.D. of Jefferson Medical College
in Philadelphia. He will present on the
theme of his latest book: Psychodynamics
of Altruism. Plenary speakers will include Henri
Parens M.D., also of Jefferson Medical College. He is a
well renowned psychoanalyst, scholar and author and a
child survivor of the Holocaust. He will present on the
topic of resiliency in the face of adversity. The other plenary
speaker will be our own Clarice Kestenbaum M.D.,
who will discuss the precursors to personality disorders
in children and adolescents. This year, the theme of
psychodynamics of psychopharmacology will be represented
by Drs. Mintz and Kalman, who will present on
the meaning of medications to patients, and on the
medication management of patients, when the patient
is concurrently being seen by a non-medical therapist.
Other presenters will include Doug Ingram M.D. who will
focus on peer supervision. There will also be a workshop
led by Cesar Alfonso M.D. on transcultural
issues and globalization. Andres
Pumariega M.D., Chairman of Psychiatry
at the very new Cooper Medical School
in Camden, N.J. will lead a panel addressing
psychotherapy issues with minority
and underprivileged children and
families. Also, there will be a continuation
of last year‘s very well-received
workshop on the changing paradigms in
psychotherapy, led by Crittenden Brooks
M.D. and also co-chaired by Clay Whitehead
M.D. This year, Jennifer Downey
M.D. and Richard Freidman M.D. will
introduce a new workshop to teach
Academy members and other attendees
about the requirements necessary to
submit papers to the Academy journal.
Erminia Scarcella M.D. will present a
workshop on the works of Carl Jung,
commemorating the 100 anniversary of
the publication of his classic work titled:
―The Red Book‖. Also, there will be papers
on topics such as psychoanalysis and politics by
Ruth Lijtmaier Ph.D; ―Life and Death in Japanese Art‖ by
Ron Turco M.D.; psychotherapy issues with adopted
patients by Eugenio Rothe M.D. and other exciting presentations
by well-regarded speakers, such as Scott
Schwartz M.D., Joe Silvio M.D., Raul Condemarin M.D.
and many others.
I encourage you to attend this very exciting and scientifically
rich meeting, which promises to deliver the same
depth and high quality presentations by the innovative,
scholarly and forward-thinking clinicians that the Academy
is well-known for. This will all take place against the
backdrop of a culturally rich historical city, where there
will be a myriad of interesting things to discover and
enjoy.
M e m b e r S p o t l i g h t :
A m a u r y D e l g a d o H e r n á n d e z , M . D .
I was born in Cuba where I trained as
an MD. I lived there until 1996 when I
moved to the UK. Upon my arrival in
England and before regaining my registration
to practice medicine, I undertook
a significant number of nonmedical
jobs. It was during this period
that I became acquainted with psychoanalytic
thinking. This began initially by
working in a therapeutic community
with a psychoanalytically informed
framework. During this time, I also began
attending a number of psychoanalytic
events at different institutions,
including the Tavistock Clinic and the
Institute of Psychoanalysis (both in
London); in addition, my involvement
deepened as a result of my own therapy.
This journey lasted a number of
years. Subsequently, after beginning
my training as a psychiatrist, my interest
in psychoanalysis grew further by
undertaking training in brief dynamic
psychotherapy as well as attending a course in Group
Analytic Psychotherapy at the Group Analysis South
West. I also continued to take an active role in performing
a number of individual and group therapies.
My interest has centred mainly on the management of
severe psychiatric disturbances such as personality disorders
and eating disorders as well as on the understanding
of their impact on multidisciplinary teams. During
my career as a resident in psychiatry, I have always
tried to ensure that my use of descriptive psychopathology
also included a strong psychoanalytic perspective.
Of course, achieving this has been rather arduous
in a profession that at times appears dominated by a
body mind split. However it was my encounter with the
writings of Professor Glen O. Gabbard that gave me the
language and the insight to integrate descriptive and
explanatory psychopathology in a way that was accepted
by my more biologically-minded colleagues.
During my search for further understanding of dynamic
psychiatry, I came across the AADPD. It was very exciting
when, after contacting the Academy‘s executive, I
was offered the possibility to benefit from their thinking
and creativity. Following an interview with Dr. C. Alfonso,
it became clear to me that the Academy was not
an elitist institution but rather an open and welcoming
body of psychiatrists interested in psychodynamic theory
and practice. I felt a tremendous sense of belonging
when the AAPDP offered me a Psychiatric Associate
membership, not only to learn from what the Academy
had to offer but also to contribute to it.
In my short period as an Associate of the Academy, I
have joined the Task Force to attract new International
Members led by Dr Blumenfield. At present, we are developing
ways to bring the AADPD to the attention of UK
psychiatrists. I am also planning an AADPD study-tour to
Havana, Cuba for 2015-2016 entitled Psychoanalysis
and Creativity, along with Dr Cesar Alfonso. This is going
to be an extraordinary journey for many reasons.
At present I am a Resident in General Adult Psychiatry,
and a Member of the Royal College of Psychiatrists, undertaking
a 12 month placement in Eating Disorders
(inpatient and community) at the National Health Service.
I am expected to finish my training in July 2013
when I will be able to work as a Consultant Psychiatrist
(Attending equivalent).
I am also involved in medical education where I teach
other psychiatry residents and offer a Special Study Unit
to medical students on Cinema and Psychiatry. Here we
explore the representation of mental illness in Cinema
and also provide a forum to think about the psychoanalytic
processes that take place.
My goals for the future are to finish the group and individual
psychoanalytic trainings, as well as to continue
developing my special interest in Eating Disorders from
a psychoanalytic perspective. I also look forward to developing
stronger links with my American and international
colleagues at the Academy.
As Trustee Representative I brought three items to the
agenda of the Executive Council meeting on October 15,
2011, in New York City.
The implementation of a Grand Rounds Speakers list on
the Education page of the website.
The Executive Council approved the proposed text for
this section of the website. We are asking those AAPDP
members who are interested in being listed as possible
Grand Rounds speakers to submit a brief description.
The description would then be linked to their name on
the list. The description should be no more than 250
words, should include your contact information, and
your academic affiliations, and topics you have presented
on previously or would be able to present on.
You can include a photograph as well. On the website
you can find some names and descriptions that have
already been posted. If you are contacted by a residency
program as a possible speaker, all arrangements,
including reimbursement, will be between you and the
residency director.
You can send your description for posting to me at
mariamcohen@cox.net.
Beginning regular presentations at the APA‘s IPS meetings
in October
The second item that I brought to the Executive Council‘s
attention involved the possibility of the Academy‘s
starting regular presentations at the APA‘s Institute for
Psychiatric Services meetings, which are usually held in
October. Dr. Alfonso had discussed with some of us a
proposal that responsibility for organizing regular presentations
at these meeting be given to the President
Elect (or Designate). This responsibility would parallel
that of the Past President, who for two years after a
term of service as President organizes a panel at each
APA convention. The motion to initiate these regular
presentations was passed unanimously. This responsibility
will start with the new President Designate who will
be elected this spring.
On October 27, 2011, Cesar Alfonso, Sheila Hafter Gray,
Laura Kent, and I held a workshop at the APA‘s IPS in
San Francisco, on using a psychodynamic formulation in
brief psychiatric encounters with patients. This workshop
was very well attended; the 40 copies of our handout
that Dr. Alfonso had prepared were all taken away,
and there was considerable discussion after our brief
presentations. At the start of the workshop, Dr. Alfonso
described the Academy and our relationship to the APA;
several people expressed an interest in joining. I hope
we can look forward to more such successful presentations.
Possible video conferencing for EC meetings
As Trustees and officers of the Academy, many of us
bear a considerable financial burden to attend Executive
Council meetings. I discussed this issue in e-mails
with the Trustees before the EC meeting. Trustees were
not favorably inclined toward any sort of stipend for attending
EC (or other meetings) for officers or Trustees.
However, there was discussion of whether any sort of
video conferencing could be arranged for those EC
meetings that are not held along with AAPDP meetings.
No one knows much about how feasible – logistically
and financially – such distance meeting would be.
Therefore, at the October EC meeting I asked if our incoming
President, Michael Blumenfeld, would consider
appointing a task force to look into the possibility of
using some sort of video conferencing to alleviate the
financial burden of traveling to EC meetings for Trustees
and Officers who do not live near the AAPDP offices.
Such a Task Force should, of course, consist of AAPDP
members who have some expertise in this sort of technology.
Since this request did not require a motion, it
has been taken under advisement, and I will check in
with Dr. Blumenfeld at the May EC meeting in Philadelphia.
Have you paid your 2011-2012 AAPDP membership dues
Academy of Psychoanalysis
and Dynamic
Psychiatry, a reception
was held in the home of
Dr. Scott Schwartz honoring
Dr. Miltiades L.
Zaphiropoulos, one the
Founding Members and
a Past President of the Academy. Dr. Schwartz graciously
opened his home for this reception at no cost to
the Academy. Current President Dr. César Alfonso expressed
his admiration for Dr. Zaphiropoulos and also
read a letter from another Founding Member and Past
President, Dr. Marianne H. Eckardt, who was also honored
by the Academy at the recent Hawaii meeting. Dr.
Schwartz then presented Dr. Zaphiropoulos with a certificate
from the Academy which he had handcrafted.
Past President Silvia Olarte read a poem which she
composed and captured how the honoree has always
taught by example. Dr. Ann Turkel, another Past President,
reflected that her biggest regret was never being
supervised by Dr. Zaphiropoulos and Past President
Ingram noted how he was impressed when he first encountered
the Founding Member and he used the word
meretricious of which Dr. Ingram didn't know the meaning.
Dr. Michael Blumenfield President-Elect told how
he was a member of Dr. Zaphiropoulos‘ local Westchester
(NY) District Branch of the APA where Dr. Zaphiropoulos
is also a Founding Member, Past President
and held in great esteem. Dr. Zaphiropoulos graciously
accepted the certificate and all the tributes which were
paid to him. He also stated how he was looking forward
to many more years contributing to the Academy. The
warm feeling, congenial talk and good food continued
after the ceremony. (For a 6 minute video clip of the
Paul Chodoff was a silver-haired, silver-tongued iconoclast
but a true gentleman of the finest caliber. I attended
a panel about "How Women Are treated in Psychotherapy"
in Toronto many years ago. Gloria Steinhem
was at her loveliest and angriest. During the panel Paul
Chodoff discussed the case of a woman he had seen in
3x week psychotherapy for 3-4 years . At the close of
the panel Gloria Steinhem commented that, "If more
women had therapists like Paul Chodoff, psychiatry
would have a better reputation among effective and
knowledgeable women."
Paul Chodoff, Lawrence Kubie and Ralph Romeo
Dr. Alfred M. Freedman, leader in psychiatry, lifelong
human rights activist, advocate for social justice, psychoanalyst,
and academician who led the American Psychiatric
Association in 1973 when, overturning a century-old
policy, it declared that homosexuality was not a
mental illness, died on April 17, 2011 at the age of 94.
His death followed hospitalization and surgery for a hip
fracture.
In 1972, Dr. Freedman was approached by a group of
young psychiatric activists, members of the Committee
of Concerned Psychiatrists, and
asked to run as a petition candidate
for the presidency of the
American Psychiatric Association.
He won the presidency and during
his tenure, supported a resolution
to delete homosexuality from the
list of mental illness diagnoses. On
December 15, 1973, the APA's
board of trustees voted 13—0 in
favor of the resolution, which stated
that "by itself, homosexuality does
not meet the criteria for being a
psychiatric disorder" and that "We
will no longer insist on a label of
sickness for individuals who insist
that they are well and demonstrate
no generalized impairment in social
effectiveness." LGBT rights organizations
have hailed this decision as
one of the greatest advances for
gay equality in the United States.
Dr. Freedman himself believed that
passing this resolution was the most important accomplishment
of his one-year tenure as president. A second
resolution called for an end to discrimination based on
sexual orientation and the repeal of laws against consensual
sex. During his 1973 term as President of the
APA, Dr. Freedman also spoke out against the Soviet
Union‘s use of psychiatric abuse of prisoners of war.
Dr. Freedman‘s human rights activism has made a major
impact on children, adolescents, and adults and
spanned his entire professional lifetime for 70 of his 94
years. He focused on children of poverty, underserved
populations, addicted persons, gay men and women,
oppressed or incarcerated individuals, persons on death
row, persons who are severely and chronically mentally
ill, Soviet dissidents and persons who are victims of
international torture. While many of us are distressed
about human rights violations, it is only the rare individual,
the one in a million, who remains active over the
course of more than 70 years and attains reachable
goals through a systematic approach toward eliminating
or ameliorating violations of human rights. Dr. Freedman
was such a person.
As a child psychiatrist, Dr. Freedman‘s human rights
activism made a major impact on the care and teaching
of underserved children and contributed to the development
of Headstart. Dr. Freedman had also worked to
protect the rights of mentally ill persons including addicted
individuals. His human rights activism has been
effective and focused and continued on a national and
international level until his death. A
graduate of Cornell University and
the University of Minnesota Medical
School, Dr. Freedman joined the
New York Medical College faculty in
1960 and became the first full-time
chair of the psychiatry department.
Dr. Freedman realized that the residency
training program would benefit
from a more comprehensive approach
to psychiatry and with Dr.
Harold Kaplan was founding editor
and editor of the first three editions
of the Comprehensive Textbook of
Psychiatry. First published in 1967
at a cost of $24.75, this text is now
in its ninth edition and edited by
Drs. Benjamin J. Sadock, Virginia A.
Sadock, and Pedro Ruiz. Generations
of psychiatrists have studied
this text to learn psychiatry and to
pass their boards.
Dr. Joseph T. English, current Professor and Chair of the
Department of Psychiatry at New York Medical College
states: ―It is with deep sadness that the New York Medical
College, Department of Psychiatry and Behavioral
Sciences mourns the loss of Alfred M. Freedman, M.D.,
Professor and Chair 1960-1988, Professor and Chairman
Emeritus, 1988-2011. Dr. Freedman was a distinguished
and recognized leader in the psychiatric community
who brought great honor and esteem to our profession.
He served as President of the American Psychopathological
Association, 1970; the American College
of Neuropsychopharmacology in 1971; and as
President of the American Psychiatric Association, 1973
-1974. A member of the Ethics Committee of the APA,
he was a leader in the APA‘s work on ethical issues,
especially the abuse of psychiatry in the former Soviet
Union. He was the recipient of the APA‘s 2008 Human
Rights Award recognizing his outstanding contribution to
human rights and mental health throughout the world.‖
Concerned with the high levels of alcoholism, drug
abuse, and crime in the medical school‘s impoverished
East Harlem neighborhood, he established drug treatment
programs for adults and adolescents in the community
and created a social and community psychiatry
division at the school to serve neighborhood residents.
After his retirement as Chair of the New York Medical
College Department of Psychiatry, he became a vocal
opponent of the death penalty. With fellow psychiatrist
Dr. Abraham Halpern, he encouraged the American
Medical Association to enforce a provision in its code of
ethics prohibiting doctors from participating in executions.
In recent years, Dr. Freedman has focused on
ethical and human rights violations of death row inmates.
He has publicized the inhumane attitudes and
practices involved with declaring death row prisoners
competent to be executed, the inhumanity of the methods
of execution, as well as the tragic use of psychiatrists
as ―hangman‘s assistants.‖ Dr. Freedman noted
that when antipsychotic medications are used by psychiatrists
to treat a delusional and psychotic prisoners
prior to an evaluation for competency to stand trial or
be executed, that the prisoner‘s right to treatment in a
mental health facility and to be found not guilty by reason
of insanity is clearly violated. The psychiatrists who
do this are in his words hangman‘s assistants because
they are preparing mentally ill prisoners, who should be
pardoned and found not guilty by reason of insanity and
cared for in a psychiatric facility, to stay on death row,
stand trial, and be executed.
Dr. Freedman has been vociferous in his concern for the
violation of human rights and the importance of the prevention
of physicians from participating in torture.
His most recent article on this topic was published in
2009 when he was 92 years of age. This article, Medical
Ethics and Torture: Revising the Declaration of Tokyo,
called for changes in the code of ethics to make it
readable for persons with only 12 years of education
and also to emphasize prevention of physician participation
in torture of any kind. This important paper was coauthored
with Dr. Steven H. Miles and published in The
Lancet (2009; 373: 344-348).
I will miss Dr. Freedman and his wife Mrs. Marcia Freedman,
who died after a long illness on September 9,
2011. It was Dr. Freedman‘s wish to remain alive for at
least another three years to be able to take care of her.
They celebrated their 68 th wedding anniversary together
in their home only one month before his death and five
months before hers. My initial acquaintance with Dr.
Freedman began when I bought his textbook in 1967 in
its very first edition. I worshipped him from afar, and did
not get to meet him until he was close to his retirement
from New York Medical College. Since that
time I was fortunate to get to know Dr. Freedman
and his family. He was a remarkable psychiatrist,
husband, brother, father, and grandfather. Dr. and
Mrs. Freedman have left an important legacy of
love. Their son, Dr. Paul Freedman, is Professor of
Medieval History at Yale University. Their son, Dan
Freedman, is the National Editor of Hearst Newspapers‘
Washington Bureau. Mr. Dan Freedman‘s son,
Jacob, may have summarized it best in the eulogy
that he gave for his grandfather: ―In the course of
reflecting on my grandfather‘s life ahead of this occasion
- as a leader in psychiatry and an advocate
for social justice, as a devoted husband, father, and
grandfather - I naturally struggled to find the right
story or anecdote that would capture the richness
and meaning of our relationship.
For my entire life, my grandfather‘s influence on me
has been formidable. He was a source of wisdom
and humor, a motivating example of what you can
accomplish through persistence and hard work, and
- above all - a loving family man who found great joy
in spending time together.‖
In addition to his legacy to the worlds of psychiatry
and human rights, Dr. Freedman leaves behind a
legacy of love to his sons, Paul and Dan Freedman,
his grandchildren, Andrew Freedman, Jacob Freedman,
Sasha Johnson, his brother, Aaron Freedman,
his family of relatives and friends including his close
friends, Dr. Abraham Halpern, Dr. Max Fink, and
trainees and colleagues.
As both his friend and colleague, I can recall Dr.
Freedman‘s concerns that he had not done enough
for the family or for social justice. He wanted those
three more years for his wife and was concerned
because of the continuing human rights violations
throughout the world and complained that he had
not written another paper for The Lancet in the past
two years. There is no question in my mind that his
contributions to his family, community, psychiatry,
and the world have been prodigious. It was a pleasure
and an honor to have known him. Through his
tireless, systematic, erudite, and effective efforts,
Dr. Freedman has illuminated and eradicated many
human rights violations both nationally and internationally.
Dr. Freedman‘s work continues to have a
substantive impact on protecting the rights of persons
throughout the world.
It is my pleasure to report to the AAPDP membership
that the Teichner Visiting Scholar Program is doing well.
For those of you unfamiliar with the program, please
view the Teichner Scholar Program section of the AAPDP
website at www.AAPDP.org for up-to-date information as
well as a history of the Program.
Dr. Richard Brockman, the Teichner Scholar for the
2010/2011 cycle, will make his visit to the Indiana University
campus on April 18-20 2012.
Joanna Chambers, Training Director at Indiana University,
has assembled a panel and submitted the panel for
consideration for the 2012 meeting of AADPRT in March
2012. The topic of the panel is ―Distance Learning initiatives
in Psychodynamic Education,‖ and will feature
Academy and AADPRT members who have been using
Skype and other telecommunication programs for
teaching and supervision.
If our panel is accepted for the meeting, and a quorum
of Committee members and former program and
scholar winners can be present, it is our hope that we
might, once again, have a face-to-face meeting at the
AADPRT meeting. This was an interesting and productive
meeting last year.
AADPRT has revised their administrative procedures in
order to bring all awards and Fellowships into a uniform
timetable. We polled the members of the Committee
and, finding no objections, agreed to the revised timetable:
All fellowships will be announced in September,
and the closing date for applications will be moved from
December 15 to November 1. Committee members are
prepared to deliberate during November, so that the
winner of the award can be announced December 1.
Along with these changes, we have revised and updated
the information about the program on both the AADPRT
and AAPDP websites, and improved the appearance of
the information describing the Teichner program. We
would particularly like to recognize Jacqui Davis for her
efforts on our behalf. Her administrative and aesthetic
support has been flawless.
We continue to recruit new members to the Academy
who wish to join the list of Teichner Scholars Nominees,
and we continue to recruit longstanding members of the
Academy to the list of Teichner Scholar Nominees. We
encourage all members to join this growing list. Visit
www.AAPDP.org for details.
The PR&I Committee consists of Gerald P. Perman, M.D.
and Michael Blumenfield, M.D. (co-chairs); Mariam
Cohen, M.D., Mary Ann Cohen, M.D., Maurice Preter,
M.D., and Joseph R. Silvio, M.D.
The PR&I Committee has worked closely with the President
and Staff mostly via email to further the publicity
and information needs of the Academy. We had a phone
conference on June 19, 2011.
a. Jacquilyn Davis did an excellent job updating the
Media and Information page on our website that
includes links to ―Members in the News,‖
―Interviews and Video Segments,‖ and ―Books by
Academy Members.‖ The ―Members in the News‖
section includes news articles and obituaries about
Academy members from 2009, 2010 and 2011.
Each entry includes a link to the article referred to.
The ―Interviews and Video Segments‖ includes
video clips of Dr. Glucksman, Silver, Blumenfield
and Eckardt. The Book section has 13 pages of
nicely presented books with photos of their covers.
2. Tasks in Progress
a. The ―Grand Rounds Speaker‖ website page is a
work in progress. A number of entries have been
received by members to be included on this page
when it is ready to go.
3. Suggested Future Tasks: Consider appointing Task
Force(s) to work on these proposals.
a. We believe that the Academy should have a social
media networking presence including Linked-In,
Facebook and Sina Weibo. Although this could potentially
be time consuming, perhaps we could get a
short Facebook page that would direct people to
our website. A Youtube presence on the web with a
brief video by the president about the Academy
could also direct viewers to the Academy website.
These should both be accessible from the Media/
Public Information link on our website.
b. We recommend that biographical information be
added to the Teichner webpage and this will be
taken up by Dr. Mariam Cohen with Ms. Jacquilyn
Davis.
c. The issue of Media Spokespersons is complicated
and needs to be discussed in Council. We could
direct the media to the Academy President who
could speak for the Academy or refer the media to a
best person within the Academy for that particular
topic.
d. The APA provides media training for District
Branches. Michael Blumenfield will find out if the
APA will offer this to us at the Philadelphia meeting.
This could generate additional PR for the Academy
and its members.
e. We would like to improve the relevancy of the Academy
to current and potential members: what‘s in it
for them The ―chatter‖ in medicine is in Social Media
– we must find additional and up-to-date ways
for members to connect with one another. Should
we try to find an ECP to join the PR&I Committee
How can we help members more with their private
practices
The Academy is holding its Thirteenth Joint Meeting with
OPIFER (Organizzazione di Psicoanalisti Italiani-
Federazione e Registro) in Rome, November 12-13,
2011. The theme of the meeting is "Psychoanalytic
Training Today: In the Footsteps of Silvano Arieti." This
meeting honors the memory of Dr. Jules Bemporad,
esteemed Past President and Past Journal Editor of the
Academy, who played an instrumental role in helping to
establish these yearly meetings in Italy.
The Keynote Speaker is Dr. John T. Walkup, Vice Chair,
Department of Psychiatry and Director, Division of Child
and Adolescent Psychiatry at New York-Presbtyerian
Hospital/Weill Cornell Medical Center. He will talk on
"Challenges of Teaching Psychodynamics in Modern
Psychiatry Training Programs." Academy Fellow Dr. Erminia
Scarcella will present "Training in Jungian Analytic
Psychology: A Quick Overview." Dr. James Eaton, also an
Academy Fellow, will present "Using a Consultation-
Liaison Service to Begin Teaching Principles of Psychodynamics
and Countertransference."
This is the first time the Joint Meeting has been held in
Rome. The meeting venue is the Centro di Formazione
"Polo Didattico" situated in the Piazza Oderico da Pordenone,
easily reached by public transportation or taxi
from central Rome.
We have held 12 Joint Meetings with OPIFER so far. As
a result 14 members of OPIFER have joined the Academy.
These Italian members have included such prominent
psychiatrists as Dr. Adolfo Pazzagli, the past Chair
of the Psychiatry Department at the University of Florence,
and Dr. Giovanni Corsini, the leading psychopharmacologist
in Italy. Additionally, because of their participation
in the Joint Meetings, at least 4 American psychiatrists
became Academy members, including David
Lopez, Sandra Park, Craig Katz and Charles Nemeroff.
Dr. Deborah Rose, recently became a Psychiatric Fellow,
in part because she was enthusiastic about our meetings
in Italy.
OPIFER's Meeting Planning Committees contribute an
enormous amount of work in arranging the meetings
abroad, at no cost to the Academy. We do share the
cost of the meeting venue with OPIFER but the Joint
Meetings have always made money for the Academy.
CAPA (China American Psychoanalytic Alliance) is thriving
and, most especially, growing. As Cesar Alfonso said
in his Presidential address…‖altruism requires empathy
coupled with action.‖ CAPA members demonstrate the
most extraordinary level of altruism. As physicians, we
are sworn to teach the ―sons of our teachers.‖ Indeed,
most of us volunteer at hospitals and medical schools.
In return we often receive an academic appointment.
Such work fosters collegial relations and often brings
referrals. The people volunteering with CAPA, engaging
in the tasks of teaching, supervising, treating and working
in CAPA administration often donate 8-20 hours a
week—some more. This is an astonishing outpouring of
generosity. They receive no perks, no formal honors, no
career or practice advancement. There is collegiality
(most of us have made new friends) but it is nevertheless
a virtual experience. What there is--- is an exciting
adventure: an opportunity to do good while having a
fascinating experience and being a part of the wave of
the future.
We now have more than 300 psychoanalyst and psychodynamic
psychotherapist members from the USA, Canada,
Mexico, Argentina, England, France, Italy, Germany,
Israel and Thailand. Our Chinese student members
come from a fairly diverse geographical area, spanning
a number of Chinese cities, ten to be precise. Invitations
for interviews, publishing and speaking about CAPA arrive
almost daily. Usually, invitations to speak come with
plane fare, a hotel room, a dinner and occasionally an
honorarium. Given that CAPA specializes in distance
training and treatment, I imagine a future where there
will be invitations to address a group on Skype. The audience
will bring their own food and the organizer will
call my local take-out Chinese restaurant and have
them deliver a banquet for me. We will then be able to
eat and speak with each other before the presentation.
CAPA now has a four-year psychotherapy training program
(the third year began in the
fall of 2011). There are 160 students;
only about half those applying
are accepted. Good English is
one or the prerequisites for acceptance.
Our students are psychiatrists,
psychologists and counselors.

54 Students in 1-2 session/week psychotherapy
There are more than 100 teachers, 160 supervisors
and 100 people providing treatment. Some CAPA members
do more than one thing. These staggering numbers
remind us of how vast a country China is and also that
almost every one of the 1.3 billion people seems to
want training and treatment.
All classes use ooVoo (a video conferencing software).
The readings for each class are either on PEPWeb or
available as PDFs. The students receive ―report cards‖
at the end
of each trimester as do their teachers. Several CAPA
students have visited America and attended various
professional meetings. Other students are working at
American universities and medical schools doing post
docs and research.
All treatments employ Skype (which
is secure). Several CAPA analysts
have presented case material to
colleagues with all references to
Skype and China removed. Their
colleagues were startled to learn
that these were Skype analyses
with patients in China.
CAPA has twice-yearly meetings,
with each one offering two discussion
groups. Each meeting is fol-
the discussions have included topics such as ‖Ethical
problems of Treating and Supervising in China,‖
―Problems in Supervision in China,‖ and
―Countertransference Issues in China‖ (a recording of
these last two is available).
And last but not least, the third annual CAPA tour of
China will take place this October 2011. Twenty-five
CAPA members and some significant others will visit
four cities in China (Beijing, Xi‘an, Chengdu and Shanghai)
and Tibet. As well as visiting the usual tourist sites,
they will meet with patients, supervisees and their
classes. They will give lectures, supervisions and consultations.
They will be honored guests at CAPA‘s second
graduation where CAPA students will present papers.
I hope you will consider joining CAPA and participating in
this adventure. Please contact: elise.snyder@yale.edu.
Embedded in the
culture of AAPDP is a
rich history of experienced
clinicians supporting
the professional
development
of more junior colleagues.
Many members
can tell story
after story of the
guidance and practical
support that influenced
their growth.
For regions rich in
dynamically-oriented psychiatrists, it can be easy to find
a mentor. However, for some parts of the country, or for
some interest areas, it can be more difficult. Hoping to
extend the benefits of mentorship, AAPDP has created a
structured mentorship program.
Most people seeking mentorship are medical students,
psychiatry residents, fellows or early career psychiatrists,
but the program is open to all members. The
application process is designed to be simple, and is
outlined on the website. http://aapdp.org/index.php/
education/mentorship-program/
The Future Leaders in Dynamic Psychiatry Committee
provided excellent suggestions for what applicants
might want from a mentor. The program was designed
to be very individualized, with frequency of contact and
topics discussed agreed upon by the mentorship pair.
Members of AAPDP have been very generous in volunteering
to serve as mentors.
An application deadline is listed for July in order to encourage
individuals to apply early in the academic year,
but the Education Committee will accept applicants on a
rolling basis.
The Education Committee will gather data on how the
mentorship was used by current pairs in order to develop
the Mentorship Project further next year. Questions
can be directed to Kim Best, MD at
bestk@einstein.edu
