Welcome to <strong>Prov</strong>idence!<br />

Your job offer is contingent on the completion of all post-offer requirements. This includes a<br />

background check, drug screen and health evaluation. The following outlines time frames and<br />

expectations:<br />

Initial call – You have been directed to complete your drug screen within 48 hours of the call, and<br />

to turn in your new hire paperwork to the employment center.<br />

Employment Eligibility Verification (I-9 form) – You will need to present government issued<br />

identification when you come to the Employment Center. Please see the attached “acceptable<br />

documents” list compiled by the Department of Homeland Security (photocopies are not<br />

acceptable). It is a <strong>Prov</strong>idence <strong>Health</strong> & <strong>Services</strong> policy that you may not begin working<br />

without completing this requirement.<br />

Licensure and/or credentials – Requirements vary depending upon your position. You have been<br />

instructed to bring your Oregon license and/or certifications, specific to your position, with you to<br />

the Employment Center. It is a <strong>Prov</strong>idence <strong>Health</strong> & <strong>Services</strong> policy that you may not begin<br />

working without completing this requirement.<br />

Background check - Your background check may also include verification of your employment<br />

history for the past 5 years and of any education required of the position you have accepted. If<br />

you are aware that any of your past employers are no longer in business, HireRight will ask you<br />

to provide documentation such as a W2 or a pay stub to verify your employment history. If you<br />

are concerned or do not feel you will be able to produce this documentation, please call me.<br />

Department of Human <strong>Services</strong> background consent form – This additional process is required<br />

for some <strong>Prov</strong>idence positions. The consent form must be completed in person. This process<br />

takes 8-10 business days to complete. If you have lived outside the state of Oregon for more than<br />

60 consecutive days in the past 5 years, you will be sent for fingerprinting, as required for an FBI<br />

investigation. This additional background check takes 5-6 weeks on average to complete. You<br />

may be granted probationary clearance to start work.<br />

<strong>Health</strong> Evaluation – You will be contacted once the background check and drug screen results<br />

have been received. <strong>Health</strong> screen appointments are available mornings only at the four Portland<br />

area <strong>Prov</strong>idence hospitals. A clearance from the Employee <strong>Health</strong> Department is required prior<br />

to allowing you to attend New Employee Orientation.<br />

Final clearance – will be sent to your hiring manager once all post-offer requirements have been<br />

met.<br />
