5 To ask the Commissioner for<br />

Care and Independence to<br />

clarify if Personal Health<br />

Budgets could be used to<br />

support patients wishing to<br />

access Phase IV CR.<br />

Members would also be<br />

interested to hear if there is<br />

any potential to assist those<br />

patients receiving CR using<br />

telehealth.<br />

6 To ask the Chief <strong>Executive</strong> of<br />

UHNS to review the<br />

arrangements for those<br />

patients on the waiting list at<br />

UHNS for Phase III care to<br />

ensure that they are being<br />

offered appropriate support<br />

but with a view to reducing the<br />

<strong>Response</strong>/<strong>Action</strong><br />

and reported from the Directorates,<br />

through to the risk group, clinical<br />

governance executive group and up to<br />

the Trust Board. Specific risks<br />

associated with CR have not been<br />

formally raised through the clinical<br />

governance framework. However this<br />

is currently being reviewed by the<br />

Directorate.<br />

●The Joint Commissioning Unit will<br />

investigate the possibilities of using a<br />

Personal Health Budget for patients<br />

wishing to access Phase IV CR.<br />

●The Joint Commissioning Unit will<br />

work with Clinical Commissioning<br />

Groups to see if any of the telehealth<br />

solutions currently being investigated<br />

would support patients requiring<br />

cardiac rehabilitation.<br />

●The CR service is currently not<br />

commissioned by the PCTs. However<br />

there is ongoing dialogue with the<br />

cardiology commissioning group to<br />

identify option for the continuation and<br />

growth of the service. Discussions to<br />

date have focused on the model of the<br />

service based on the national<br />
