As part of the Phase IV programme clients will receive ongoing support from the exercise instructor to access other activity opportunities. The outcomes from all of this will be monitored. Completion date 4 To ask the Chief Executive of UHNS for an explanation of what mechanisms are in place at the Trust for taking on board the concerns of Consultant Cardiologists in relation to the availability of CR provision in north Staffordshire East Staffordshire Borough Council (ESBC) Chief Executive (CE), UHNS The Council is undertaking a review of Phase III and Phase IV services in East Staffordshire in conjunction with Burton Hospitals NHS Foundation Trust and local GPs. ●The Consultant Cardiologists report to the Clinical Director (CD) for Cardiology. The CD attends the fortnightly Executive Directors (ED) meeting and has the opportunity to report any issues or concerns brought to his attention. The EDs meeting is chaired by the CE. However the CEO has an open door policy and whilst does not wish to take action outside of the appropriate management arrangements is accessible to any member of staff. ●Issues of concern can also be raised through the Clinical Governance Framework, where risks are identified To begin end of Oct 2011 Sept 2011
Staffordshire Health Scrutiny Committee Cardiac Rehabilitation Scrutiny Review Summary of Responses to Recommendations and Actions Recommendation 5 To ask the Commissioner for Care and Independence to clarify if Personal Health Budgets could be used to support patients wishing to access Phase IV CR. Members would also be interested to hear if there is any potential to assist those patients receiving CR using telehealth. 6 To ask the Chief Executive of UHNS to review the arrangements for those patients on the waiting list at UHNS for Phase III care to ensure that they are being offered appropriate support but with a view to reducing the Organisation/Lead Officer Commissioner for Care and Independence, Staffordshire County Council Chief Executive, UHNS Response/Action and reported from the Directorates, through to the risk group, clinical governance executive group and up to the Trust Board. Specific risks associated with CR have not been formally raised through the clinical governance framework. However this is currently being reviewed by the Directorate. ●The Joint Commissioning Unit will investigate the possibilities of using a Personal Health Budget for patients wishing to access Phase IV CR. ●The Joint Commissioning Unit will work with Clinical Commissioning Groups to see if any of the telehealth solutions currently being investigated would support patients requiring cardiac rehabilitation. ●The CR service is currently not commissioned by the PCTs. However there is ongoing dialogue with the cardiology commissioning group to identify option for the continuation and growth of the service.
