Scholarship application process:
REACH Student Dance Group Scholarship Request Form
1. Submit complete REACH registration form
2. Submit complete Scholarship Request Form including student statement
3. REACH staff will call parent/guardian to discuss program guidelines
4. Scholarship decisions will be announced no later than September 7, 2012

Please review the following attendance policy carefully. Failure to adhere to the attendance policy
or electing to discontinue participation in REACH mid-session will jeopardize eligibility for
scholarships for future REACH sessions.
Attendance Policy:
1. Students are allowed ONE EXCUSED absence per 8-week session. For an absence to be
excused, REACH program staff or the teaching artist must be notified IN ADVANCE of the
absence. Please call 206.441.9411 x4229 to report any absences or email
REACH@pnb.org
2. After the first unexcused absence, REACH program staff will follow up with a phone call
home.
3. After the second unexcused absence, the student may be dismissed from the program.
4. Both excused and unexcused absences may jeopardize the student’s full role in the
performance piece. A student may be left out of choreography completed in his/her
absence.
5. Attendance at the final dress rehearsal is mandatory. Failure to attend dress rehearsal will
result in being unable to participate in the final performance.
