The Arizona Nutrition Network provides nutrition services to schools where 50% or more of the student<br />

population receive free or reduced meals.<br />

South West Food Service Excellence request nutrition services for their schools at Chinle Unified School<br />

District. South West has appointed Farrah Begay, Nutrition Specialist/Assistant Director to teach the<br />

nutrition education in the classrooms at the following schools: Chinle, Many Farms and Tsaile. The<br />

Memorandum of Agreement outlines what all parties are responsible for. Navajo County will provide<br />

nutrition materials and training to South West Food Service Excellence and SWFS will teach the nutrition<br />

education lessons to their Pre-K through 8th grade students.<br />

Chinle Unified School District is in Apache County and I have confirmed with Apache County Public<br />

Health Service District Robin Aguero, Division Manager - Public Health Education, that they do not<br />

provide nutrition services to this school.<br />

Fiscal Impact: (what will it cost, where funds will come from, is it budgeted, etc.)<br />

There will be no exchange of money between Navajo County or South West Food Service Excellence.<br />

The only money that is going into this project will be that Navajo County Nutrition program will pay for<br />

the nutrition education materials, train the trainer programs, and travel to the training site in Chinle to<br />

work with Farrah Begay.<br />

South West will pay Farrah Begay to teach the nutrition education lessons in the schools.<br />

MEMORANDUM OF AGREEMENT (MOA)<br />

between<br />

Navajo County Public Health Services District, Nutrition Services,<br />

Nutrition Network Local Incentive Program<br />

and<br />

Southwest Food Service Excellence LLC<br />

and<br />

Chinle Unified School District No. 24 of Apache County<br />

This Memorandum of Agreement (“MOA”) is between the Navajo County Public Health Services<br />

District, by and through its Nutrition Services, Nutrition Network Local Incentive Program (“the Health<br />

District”), Southwest Food Service Excellence LLC (“Southwest”), and Chinle Unified School District<br />

No. 24 of Apache County (“the School District”):<br />

I. PURPOSE AND SCOPE<br />

The purpose of this MOA is to clearly identify the roles and responsibilities of each party as they relate to<br />

collaboration to increase fruit and vegetable consumption, physical activity levels and food safety for<br />

families eligible for the Supplemental Nutrition Assistance Program (SNAP) in Apache County.<br />

II.<br />

BACKGROUND<br />

The Nutrition Network Local Incentive Program is administered by the Health District. The purpose of<br />

the Program is to increase fruit and vegetable consumption, increase physical activity levels, provide<br />

federal nutrition food service information to food stamp eligible youth and families in Apache County,<br />

and administer and oversee SNAP.<br />

The School District operates seven schools within its boundaries. Southwest is a private company that<br />

provides nutrition consulting and planning services to the School District and, in cooperation with the<br />

School District, serves approximately 2,956 participants within the communities of Chinle, Many Farms<br />

and Tsaile. The mission of Southwest is to equip preschool to adult participants and families with the<br />

skills, knowledge and attitudes they will need to pursue their life goals responsibly and creatively in a<br />

radically changing society.<br />

III.<br />

HEALTH DISTRICT’S RESPONSIBILITIES UNDER THIS MOA<br />

• Oversee all activities covered by the Scope of Work.<br />

• Prepare and submit all reports or data required by SNAP.<br />

• Participate in one event per contract year to promote the Fruits and Veggies More Matters<br />

message and activities including a Back to School Night or Health District staff-organized<br />

nutrition education event to promote healthful eating and physical activity.


• Provide to the representative designated by Southwest and the School District adequate quantities<br />

of nutrition newsletters and Community Tool Kits that promote the Fruits and Veggies More<br />

Matters, as well as messages that promote drinking fat-free and 1% low- fat milk.<br />

• Conduct one “train the trainer” session for food service personnel per contract year on use of the<br />

Health District's nutrition and physical activity curriculum to be utilized in the School District.<br />

• Provide nutrition and physical activity curriculum to the representative designated by Southwest<br />

and School District. The curriculum will cover pre-kindergarten through eighth grade.<br />

• Provide to the representative designated by Southwest and the School District (and replenish as<br />

necessary) Fun Food Newsletters, recipe cards, nutrition brochures and educational reinforcement<br />

incentives on a monthly basis.<br />

• Provide to the representative designated by Southwest and the School District required pre/post<br />

tests and parent surveys.<br />

• Provide space and help recruit parents for at least one (30-60 minute) nutrition and physical<br />

activity parent education training.<br />

• Provide School Wellness resources and information to the School District and Southwest.<br />

IV.<br />

SOUTHWEST’S AND SCHOOL DISTRICT’S COLLECTIVE RESPONSIBILITIES<br />

UNDER THIS MOA (the allocation of responsibilities to be determined by Southwest and<br />

the School District)<br />

• Provide facilities for one event per contract year such as a Back to School Night or Health<br />

District-organized nutrition education event to promote healthful eating and physical activity.<br />

• Provide food for one Health District-organized nutrition education event per contract year to<br />

promote healthful eating and physical activity.<br />

• Designate a representative to serve as a contact person to receive the nutrition newsletters and<br />

School Wellness resources, distribute the newsletters and utilize the School Wellness resources<br />

during the school year.<br />

• Designate a representative to utilize and teach the Health District’s nutrition and physical activity<br />

curriculum, and utilize the pre/post tests and parent surveys during the school year.<br />

• Submit all required reports or data reasonably required for SNAP (subject to FERPA, HIPAA and<br />

applicable federal and state laws).<br />

• Utilize nutrition and physical activity curriculum for pre-kindergarten through eighth grade and<br />

return to the Health District any materials that are not used.<br />

• In utilizing the School Wellness resources, Southwest will complete the nutrition portion of the<br />

School Wellness policy.<br />

V. IT IS MUTUALLY UNDERSTOOD AND AGREED BY AND BETWEEN THE PARTIES<br />

THAT:<br />

• This MOA is subject to prior review and written approval of the Arizona Department of Health<br />

Services and United States Department of Agriculture. The parties understand and agree that this<br />

MOA is made contingent upon the availability of SNAP funds. Proposals considered by the<br />

Legislature, including those made by the Governor, could potentially reduce or defer the funding<br />

for the current year's programs. In the event that the Legislature takes action to reduce or defer<br />

the funding for SNAP, this MOA will be amended accordingly.<br />

• Any party may terminate this MOA with or without cause by giving 30 calendar days prior<br />

written notice to the other party.<br />

VI.<br />

FUNDING<br />

• Except as payment/reimbursement may be provided by SNAP, each party shall be solely<br />

responsible for all costs and expenses associated with its performance under this MOA. No party<br />

shall have any financial obligation to any other party independent of SNAP.


• Program activities do not supplant existing nutrition education programs, and where operating in<br />

conjunction with existing programs, shall enhance and supplement them. This applies to all<br />

activities and costs under both federal and state budget shares.<br />

VII.<br />

EFFECTIVE DATE AND SIGNATURE<br />

This MOA shall be effective when it has been executed by all parties and approved by the Arizona<br />

Department of Health Services and United States Department of Agriculture. It shall remain in effect<br />

until September 31, 2013.<br />
