Guidelines for Application<br />
How to Submit<br />
Application Materials<br />
If you are interested in applying to the<br />
CPFP and meet the eligibility requirements<br />
(refer to Eligibility section), you must submit<br />
your application online through our website.<br />
Applying Online<br />
Personal Statement of Research Goals and<br />
Curriculum Vitae. Please access the CPFP<br />
application on our website and link to the<br />
Application page. You will be asked to create<br />
a personal account that only you can access<br />
through a unique user name and password.<br />
You will then be requested to provide some<br />
general information and to upload your personal<br />
statement of research goals and curriculum<br />
vitae. Information entered online can be saved<br />
as the application is completed and edited up<br />
until you submit the application. The application<br />
must be submitted on or before August 25.<br />
Letters of Reference, Academic Transcripts,<br />
and Other Documentation. Four letters of<br />
reference should be uploaded to the website<br />
by the referees. Academic transcripts and other<br />
documentation materials should be uploaded to<br />
the CPFP website by the applicant. Please see<br />
website for instructions. All application materials<br />
must be submitted on or before August 25.<br />
