Consent form for photographic/media use
(adult, child or young person)
This form should be completed for each photograph/media item that you are submitting to
BATOD. It gives consent to use a photograph that you may own and demonstrates the
consent of the people included – or the person responsible for them.
I, the undersigned, consent to photographs and digital images, either owned by me or of the
person(s) named, appearing in a BATOD (British Association of Teachers of the Deaf) publication
or display. I understand that the images will be used for educational purposes.
I acknowledge that the images may also be used in and distributed by media other than a printed
publication such as, but not limited to, CD-ROM or the World Wide Web pertaining to BATOD's
activities.

responsible, can be displayed on BATOD's display boards or used in any other publicity, for
example Magazine articles or on the BATOD website.
Permission is given on condition that BATOD does not give anyone personal details of the people
in the photograph unless I give my specific permission.

If you submit several photos of a child and this permission covers all of the pictures please list each photo
with its file name eg jonathan.jpg or 00123DSC34.jpg It is also helpful to have a small print of the photo(s)
being referred to.
You may wish to suggest a title or a short explanation that could be used.
Remember that the parent may have signed overleaf to say that no personal details will be given so they
should agree to names being used and countersign the entry below.
