Bus rental availability and cost must be verified from the bus company prior to <strong>field</strong> <strong>trip</strong>.


THIS FORM MUST BE SUBMITTED AT LEAST 2 WEEKS PRIOR TO TRIP.<br />

All <strong>field</strong> <strong>trip</strong>s must be approved and the signed approval <strong>for</strong>m must be<br />

submitted to the Activities Office 10 days prior to the <strong>field</strong> <strong>trip</strong>.<br />

FIELD TRIP PROCEDURES<br />

Request date and time of Field Trip in Activities Office.<br />

Pick up ~ Request <strong>for</strong> Field Trip/Transportation Form ~ from the Activities Office.<br />

Fill out the <strong>for</strong>m and obtain all signatures starting with top and work down to the<br />

principal.<br />

Return <strong>for</strong>m to the Activities Office at least 10 days be<strong>for</strong>e the <strong>field</strong> <strong>trip</strong>.<br />

Activities Office will review <strong>transportation</strong> needs and obtain estimate of cost.<br />

Finalize <strong>field</strong> <strong>trip</strong> details. i.e. Number of students, confirmation of costs and<br />

<strong>transportation</strong> as well as chaperone list.<br />

Pick up ~ Off Campus Trip Permission Form~ from the Activities Office.<br />

Fill out in<strong>for</strong>mation that is relevant to all attending the <strong>field</strong> <strong>trip</strong>.<br />

Submit permission slip <strong>for</strong>m to duplicating and distribute to students.<br />

Collect completed <strong>for</strong>ms and any money from students.<br />

Submit money to the Business Office <strong>for</strong> the <strong>trip</strong> <strong>transportation</strong> cost.<br />

Submit copies of completed permission slip <strong>for</strong>ms to the Activities Office 2 days<br />

prior to the <strong>field</strong> <strong>trip</strong>.<br />

Provide a list of students attending to the Attendance Office 48 hours prior to<br />

departure.<br />

Bring original copies of permission <strong>for</strong>ms with you on <strong>field</strong> <strong>trip</strong>.<br />

Activities Office will submit payment approval to Business Office <strong>for</strong> <strong>transportation</strong><br />

and/or coordinate use of school buses.<br />

ADDITIONAL RULES FOR TRAVEL<br />

By METRO:<br />

Additional specific written permission from parent/guardian allowing the use of Metro/public<br />

<strong>transportation</strong>.<br />
