Now let’s look at another data sheet for frequency. This frequency data sheet can be used to keep track of the frequency count for the skill being measured. In order to use this form, the observer needs to fill in the student’s indentifying information, enter the goal/objective/behavioral definition being observed, date, and initial. On this sheet, you will also provide the times for each observation period, and then mark each occurrence of the goal or objective above the dated column. The frequency count is collected over a period of time and once the observational period is over, you will have a visual representation of the data. Let’s look at this sheet filled out. 13
