GENERAL INFORMATION<br />
Accommodation<br />
The conference hotels will be The Radcliffe or<br />
Scarman Hotel, just a few minute’s walk from the<br />
Arts Centre on the University of Warwick campus.<br />
There is a special rate of £73.50 Bed & Breakfast<br />
single occupancy. There is also an option of<br />
booking en-suite study bedrooms in the student<br />
accommodation blocks. The rate is £53.50.<br />
Rooms will only be guaranteed at this rate until 5th April<br />
so early booking is advised. Accommodation onsite<br />
is limited and we strongly recommend booking as<br />
soon as possible. There is a wide variety of alternative<br />
accommodation available in nearby Coventry.<br />
Accreditation<br />
Delegates are recommended to record their<br />
attendance in their CME/CPD diary.<br />
Audience<br />
This meeting is suitable for all doctors, nurses and<br />
allied health professionals working in primary and<br />
secondary care who have an interest in contraception<br />
and sexual health. Delegates will be informed of any<br />
additional programme activities as they are confirmed.<br />
Conference dinner<br />
The FSRH 20th Anniversary<br />
will be celebrated at the<br />
conference dinner which will take place at the Radcliffe<br />
Hotel. Tickets are subsidised by the Faculty and are<br />
priced at just £25.00 and must be booked before<br />
28 February - we would recommend early booking<br />
with your delegate place to guarantee a ticket.<br />

Registration fees<br />
Early bird registration costs for Members/Diplomates<br />
at the <strong>2013</strong> <strong>Annual</strong> <strong>Scientific</strong> <strong>Meeting</strong> are £299 for<br />
two days and £199 for one day before the<br />
28 February 2012. After the 28 February the costs<br />
are £329.00 for two days and £229.00 for one day.<br />
Non Members will need to register at the rate of<br />
£339.00 for two days and £229.00 for one day.<br />
The fee includes registration conference materials,<br />
lunch and refreshments for days for which you are<br />
booked. It does NOT include accommodation or travel.<br />
All registration forms must be accompanied by<br />
payment of fee or invoicing details. No registration<br />
will be confirmed until payment has been received.<br />
Terms and conditions<br />
Completion of this registration form constitutes a<br />
legally binding agreement. Confirmation and joining<br />
instructions will be sent to you within 3 weeks of<br />
receiving your booking. We cannot be held<br />
responsible for the non-arrival of registration<br />
information, so if you have not heard from us<br />
3 weeks after registering, it is your responsibility<br />
to call us on 020 8832 7311.<br />
Cancellations<br />
Any cancellations must be received in writing<br />
and should be sent to Profile Productions or<br />
faculty@profileproductions.co.uk. Cancellations<br />
received prior to March 18 will be refunded subject to<br />
a £45 administration charge. Cancellations received<br />
after March 18 cannot be refunded, however<br />
delegate substitutions can be made at any time.<br />

CALL FOR PAPERS<br />
The Education Committee invites abstracts for<br />
consideration as papers (7 minute oral presentation,<br />
followed by 3 minute discussion), and / or posters.<br />
Authors should indicate whether they would prefer<br />
a paper or poster presentation or either of these.<br />
The Education Committee, however, reserves the<br />
right to allocate each accepted abstract to the<br />
most appropriate form of presentation. Authors<br />
may submit more than one abstract but only<br />
one paper and one poster will be accepted for<br />
presentation by the principal author. In exceptional<br />
circumstances the judges may reserve judgement<br />
to approve multiple papers from one author.<br />
The closing date for receipt of abstracts is<br />
4 February <strong>2013</strong>. Principal authors will be notified<br />
by the end of March whether or not their submission<br />
has been accepted for inclusion in the programme<br />
and whether for an oral or poster presentation.<br />
After the Education Committee completes<br />
selection, all presenting authors will be sent full<br />
information regarding their presentation at the<br />
conference, including details of slide preparation,<br />
poster size, layout and style. All accepted abstracts<br />
will be published in the Conference Programme,<br />
a copy of which will be given to every registered<br />
participant on arrival.<br />
During the course of the meeting a small committee<br />
will judge the posters and oral presentation.<br />
Oral prizes are supported by a grant from<br />
Margaret Pyke Trust.<br />
Poster prizes are supported by a grant from FSRH.<br />
Please note the conditions<br />
of Authorship<br />
Authorship – In line with major medical journals,<br />
it is a requirement that abstracts and manuscripts<br />
submitted for review state that “authorship credit<br />
should be based only on substantial contribution<br />
to (a) conception and design, or analysis and<br />
interpretation of data; and to (b) drafting the<br />
article or reviewing it critically for important<br />
intellectual content; and on (c) final approval of<br />
the version to be published. Conditions (a), (b),<br />
and (c) must all be met. Participants solely in the<br />
acquisition of finding or the collection of data<br />
does not justify authorship”. All authors must<br />
assure the Education Committee that they fulfil<br />
the criteria of authorship and that no-one else<br />
who fills the criteria has been excluded.<br />

Terms and Conditions:<br />
1. The organiser of the Faculty and Sexual<br />
Reproductive Healthcare’s <strong>Annual</strong><br />
<strong>Scientific</strong> <strong>Meeting</strong> is Profile Productions<br />
Ltd, Northumberland House, Popes Lane,<br />
London W5 4NG.<br />
2. Registrations can only be accepted on receipt<br />
of a completed registration form. Completion<br />
of this registration form constitutes a legally<br />
binding agreement and you must sign<br />
agreeing to the terms and conditions in order<br />
for your booking to be processed.<br />
3. Your place is not guaranteed until you<br />
have received confirmation and joining<br />
instructions from Profile Productions Ltd<br />
4. Joining instructions and confirmation will<br />
be sent to you within 14 working days of<br />
receiving your booking.<br />
5. Profile Productions Ltd. cannot be held<br />
responsible for the non-arrival of registration<br />
information.<br />
6. If you have not heard from us 14 days after<br />
sending your completed form, or 14 days<br />
before the start date of the event, it is your<br />
responsibility to contact us to ensure your<br />
place is booked.<br />
7. All invoices must be paid within 21 days of<br />
the invoice date and MUST be paid prior to<br />
the conference. Failure to do so may result<br />
in your place being cancelled.<br />
8. Any delegates registering within 21 days of<br />
the event MUST include a valid credit card<br />

9. The delegate is responsible for ensuring<br />
payment to Profile Productions Ltd. has been<br />
made. Any delegates arriving at conference<br />
with an outstanding payment will be asked to<br />
provide credit card details as a guarantee.<br />
10.Registration fees include VAT @ the<br />
prevailing rate, complimentary refreshments,<br />
lunch and conference materials.<br />
11.Cancellations will only be accepted in<br />
writing. Cancellations received on or prior<br />
to 18 March <strong>2013</strong> will be refunded subject<br />
to a £45 administration fee. Cancellations<br />
received after 18 March <strong>2013</strong> will not be<br />
refunded however substitutions will be<br />
accepted at any time, but please notify the<br />
organisers in writing of any name changes.<br />
12.Any delegate registered to attend, who for<br />
whatever reason may not be able to attend<br />
the conference, is still liable for payment as<br />
per the cancellation policy in point 11.<br />

The <strong>2013</strong> <strong>Scientific</strong> <strong>Meeting</strong> will take place at the<br />
Warwick Arts Centre, University of Warwick.<br />
A purpose built events centre situated in the pleasant<br />
greenfield site of the University of Warwick the Arts centre<br />
has tiered auditoria and light airy exhibition spaces.<br />
GETTING THERE<br />
Just five miles from Coventry<br />
city centre and voted Britain’s<br />
most accessible city by the<br />
CBI, the Warwick Arts Centre<br />
is sited at the hub of the<br />
motorway network that includes<br />
the M40, M69, M1, M6 and M5.<br />

follow the signs to Warwick Arts<br />
