Recall # B-0673-12 CODE Lot # 127129 RECALLING FIRM/MANUFACTURER Novartis Vaccine and Diagnostics, Inc., Emeryville, CA, by telephone, e-mail, and letter on December 12, 2011. Firm initiated recall is complete. REASON Hepatitis C Virus Encoded Antigen (For Further Manufacturing Use, formulated with a reduced concentration of Dithiothreitol (DTT) in the formulation buffer, was distributed. Current Drug Shortages Caffeine and Ergotamine Tartrate February 24, 2012 Products Affected - Description Cafergot tablets, Sandoz 100 mg caffeine/ 1 mg ergotamine tartrate, 100 count bottle (NDC 00781-5405-01) Reason for the Shortage • Sandoz states the shortage is due to a change in the raw material plant location. • Sandoz is the only manufacturer of caffeine and ergotamine tablets. • Cypress discontinued their caffeine and ergotamine tablets in February, 2011 and West-Ward discontinued their product in April, 2010. This recall affects West-Ward as well as Claris because they manufacture product for both companies. • West-Ward discontinued their metronidazole injection in May, 2011.
