The first and only periodical<br />
of <strong>it</strong>s k<strong>in</strong>d <strong>in</strong> Québec, <strong>Lobe</strong><br />
Magaz<strong>in</strong>e recently crossed a<br />
major milestone—w<strong>it</strong>h 1 million<br />
free copies distributed<br />
throughout the prov<strong>in</strong>ce—and<br />
has been published <strong>in</strong> English<br />
for the last year now. These<br />
achievements we owe to you,<br />
loyal readers, for tak<strong>in</strong>g the<br />
time to share your comments,<br />
questions and stories w<strong>it</strong>h us.<br />
We also owe our success to<br />
all of the contributors and<br />
health care professionals who<br />
have k<strong>in</strong>dly provided us w<strong>it</strong>h<br />
free articles on the topic of<br />
hear<strong>in</strong>g health over the years.<br />
What better way to <strong>in</strong>form<br />
and educate the public about<br />
car<strong>in</strong>g for their hear<strong>in</strong>g?<br />
It is <strong>in</strong> this light that we carry<br />
on w<strong>it</strong>h our awareness campaign<br />
today.<br />
S<strong>in</strong>ce 10% of the population<br />
presents w<strong>it</strong>h some form of hear<strong>in</strong>g<br />
loss at some po<strong>in</strong>t <strong>in</strong> time,<br />
you have probably been part of a<br />
discussion, mostly <strong>in</strong> a group sett<strong>in</strong>g,<br />
<strong>in</strong> which a friend, co-workers<br />
or family member frowned, held<br />
out their ear or asked the speaker<br />
to repeat themselves because<br />
they didn’t understand. At that<br />
moment, did <strong>it</strong> ever cross your<br />
m<strong>in</strong>d that this k<strong>in</strong>d of behaviour<br />
might be related to a hear<strong>in</strong>g<br />
problem?<br />
Pay attention: the effects of<br />
deafness can be subtle.<br />
For <strong>in</strong>dividuals w<strong>it</strong>h an<br />
untreated and/or uncorrected<br />
hear<strong>in</strong>g loss, <strong>it</strong> can often be a<br />
struggle to jo<strong>in</strong> <strong>in</strong> social activ<strong>it</strong>ies,<br />
even among family. Some of the<br />
most common problems associated<br />
w<strong>it</strong>h untreated deafness are<br />
social w<strong>it</strong>hdrawal and isolation.<br />
If anyone you know is show<strong>in</strong>g<br />
signs of such problems, suggest<br />
that they have their hear<strong>in</strong>g<br />
checked. A hear<strong>in</strong>g exam will<br />
help assess the state of their hear<strong>in</strong>g<br />
and ma<strong>in</strong>ta<strong>in</strong> their qual<strong>it</strong>y of<br />
life.<br />
By tak<strong>in</strong>g this small step,<br />
you will also help elim<strong>in</strong>ate the<br />
stereotypes surround<strong>in</strong>g hear<strong>in</strong>g<br />

period dur<strong>in</strong>g which the implant fuses w<strong>it</strong>h the bone<br />
(a process known as osseo<strong>in</strong>tegration), a device called a<br />
bone conductor (or sound processor) is attached to the<br />
implant. This processor transfers sound to the cochlea<br />
by convert<strong>in</strong>g signals to vibrations <strong>in</strong> the mastoid<br />
bone.<br />
We have all heard the name of<br />
Jérémy Gabriel uttered at some<br />
po<strong>in</strong>t or another. He is the boy<br />
w<strong>it</strong>h Treacher Coll<strong>in</strong>s syndrome*<br />
who, <strong>in</strong> 2006, performed a song for<br />
Pope Benedict XVI. At 14 years old,<br />
Jérémy is now enrolled <strong>in</strong> École Card<strong>in</strong>al-Roy’s<br />
<strong>in</strong>tegrated arts program, after hav<strong>in</strong>g graduated<br />
from the École oraliste de Québec pour enfants<br />
malentendants ou sourds. Still w<strong>it</strong>h plenty of<br />
ideas brew<strong>in</strong>g <strong>in</strong> his m<strong>in</strong>d, Jérémy now sports<br />
the brand-new BAHA BP100, present<strong>in</strong>g him<br />
w<strong>it</strong>h a fresh set of challenges.<br />
Jérémy’s BAHA<br />
Jérémy wears a bone conduction hear<strong>in</strong>g aid, commonly<br />
referred to as a BAHA implant. Generally<br />
speak<strong>in</strong>g, the BAHA is designed for people w<strong>it</strong>h hear<strong>in</strong>g<br />
loss due to a middle- or <strong>in</strong>ner-ear disorder. This<br />
can <strong>in</strong>clude the absence or abnormal <strong>format</strong>ion of the<br />
outer ear, or a dysfunction of the middle ear (tympanum<br />
and/or ossicles). The BAHA is also su<strong>it</strong>able for<br />
<strong>in</strong>dividuals who cannot wear conventional hear<strong>in</strong>g aids<br />
as a result of an otologic cond<strong>it</strong>ion prevent<strong>in</strong>g their<br />
cont<strong>in</strong>ual use, such as chronic ot<strong>it</strong>is, psoriasis or otorrhea.<br />
On the other hand, to be su<strong>it</strong>able for a BAHA,<br />
candidates must have normal to near-normal <strong>in</strong>ner-ear<br />
function.<br />
The BAHA BP100<br />
Only recently was Jérémy able to obta<strong>in</strong> a fully dig<strong>it</strong>al<br />
conductor, mean<strong>in</strong>g one that is computer programmed:<br />
the BAHA BP100. This model used technological<br />
developments that are new to the world of BAHAs,<br />
allow<strong>in</strong>g for better sound qual<strong>it</strong>y and greater flexibil<strong>it</strong>y<br />
<strong>in</strong> terms of adjustments. Replac<strong>in</strong>g the previous processor,<br />
the new one simply attaches onto the implant<br />
like a snap fastener.<br />
The price of a BAHA<br />
The <strong>in</strong><strong>it</strong>ial cost for a BAHA is about $6,500, not<br />
<strong>in</strong>clud<strong>in</strong>g the surgeon’s and other professionals’ fees.<br />
And even though they are necessary <strong>in</strong> many cases, the<br />
Régie de l’assurance maladie du Québec does not cover<br />
these implants. Patients thus have two options: they<br />
can e<strong>it</strong>her pay for <strong>it</strong> themselves or apply for assistance<br />
through various foundations, such as the Shr<strong>in</strong>ers Hosp<strong>it</strong>als<br />
for Children affiliated w<strong>it</strong>h the Montréal Children’s<br />
Hosp<strong>it</strong>al or the MUHC or Sa<strong>in</strong>te-Just<strong>in</strong>e UHC<br />
foundations. Recently, a pilot project was conducted<br />
at the Hôtel-Dieu de Québec hosp<strong>it</strong>al, <strong>in</strong> which some<br />
15 patients received BAHA implants paid for through<br />
a special budget granted by the M<strong>in</strong>istère de la Santé<br />
et des Services sociaux.<br />

S<strong>in</strong>ce most foundations will not cover the cost of<br />
replac<strong>in</strong>g the processor, Jérémy decided to set up his<br />
own foundation, the Fondation Jérémy Gabriel, to<br />
<strong>in</strong>crease access to BAHA implants. Any proceeds from<br />
Jérémy’s appearances at various events will be donated<br />

(aud<strong>it</strong>ory bra<strong>in</strong>stem implant)<br />
Nowadays, <strong>it</strong> is possible to hear w<strong>it</strong>hout ears.<br />
People who are completely deaf <strong>in</strong> both<br />
ears and who do not have an aud<strong>it</strong>ory nerve<br />
between the bra<strong>in</strong> and the ear can now benef<strong>it</strong> from<br />
a new aud<strong>it</strong>ory technology.<br />
An aud<strong>it</strong>ory implant can be implanted <strong>in</strong> a part<br />
of the bra<strong>in</strong> that is accessed via the ear and mastoid.<br />
The implant is placed <strong>in</strong> the wall of the lateral recess<br />
of the fourth ventricle <strong>in</strong> the area w<strong>here</strong> the axons<br />
(nerve fibres) and cochlear nucleus (synapses)—which<br />
transport sounds picked up by the ear to the cerebral<br />
cortex—are found. This is the normal oto-neurological<br />
channel that detects sound and speech. Practically<br />
speak<strong>in</strong>g, a microphone is placed externally beh<strong>in</strong>d the<br />
hear<strong>in</strong>g-impaired person’s p<strong>in</strong>na (ear flap). This transm<strong>it</strong>s<br />
sound to a receiver embedded <strong>in</strong> the sk<strong>in</strong>, which<br />
subsequently transforms electrical impulses to small<br />
electrodes placed <strong>in</strong> the bra<strong>in</strong> (cochlear nucleus).<br />
An ENT surgeon (ear and mastoid) and a neurosurgeon<br />
(bra<strong>in</strong>) jo<strong>in</strong>tly perform the implantation of this<br />
type of implant. The first aud<strong>it</strong>ory bra<strong>in</strong>stem implant<br />
was performed <strong>in</strong> 1979 at the House Ear Inst<strong>it</strong>ute,<br />
<strong>in</strong> Los Angeles. After basic studies, the implant was<br />
marketed officially <strong>in</strong> 2000, and the first implantation<br />
<strong>in</strong> Québec was carried out <strong>in</strong> 2005 at Hôp<strong>it</strong>al<br />
de l’Enfant-Jésus du CHAUQ by Dr. Denis Pouliot,<br />
ENT specialist, and Dr. Claude Picard, neurosurgeon.<br />
Located <strong>in</strong> Québec C<strong>it</strong>y, Hôp<strong>it</strong>al de l’Enfant-Jésus is<br />
the only hosp<strong>it</strong>al <strong>in</strong> the prov<strong>in</strong>ce that performs this<br />
surgery.<br />
A mult<strong>it</strong>ude of participat<strong>in</strong>g partners<br />
First of all, CHAUQ is a health care, teach<strong>in</strong>g and<br />
research establishment, as well as an affiliated univers<strong>it</strong>y<br />
centre. For over 20 years, a multidiscipl<strong>in</strong>ary team, primarily<br />
compris<strong>in</strong>g ENT specialists and neurosurgeons,<br />
has developed cutt<strong>in</strong>g-edge expertise <strong>in</strong> the treatment<br />
of basal skull tumours. For <strong>it</strong>s part, Hôtel-Dieu de<br />
Québec du CHUQ carried out Canada’s first cochlear<br />
implant <strong>in</strong> 1984. S<strong>in</strong>ce then, the nationally and <strong>in</strong>ternationally<br />
renowned Québec Cochlear Implant Program<br />
has become an ultra-specialized centre <strong>in</strong> aud<strong>it</strong>ory<br />
implants, thanks to the participation of a number<br />
of partners. Given the expertise developed <strong>in</strong> cochlear<br />
implants at Hôp<strong>it</strong>al-Dieu de Québec and that of basal<br />
skull surgery at Hôp<strong>it</strong>al de l’Enfant-Jésus, access to<br />
state-of-the-art technology, requir<strong>in</strong>g the <strong>in</strong>volvement<br />
of this team, is now possible. Then t<strong>here</strong> is the Inst<strong>it</strong>ut<br />
de réadaptation en déficience physique de Québec<br />
(IRDPQ), a univers<strong>it</strong>y <strong>in</strong>st<strong>it</strong>ute that offers habil<strong>it</strong>ation<br />
and rehabil<strong>it</strong>ation services, as well as social <strong>in</strong>tegration,<br />
counsell<strong>in</strong>g and family support to <strong>in</strong>dividuals <strong>in</strong>volved<br />
<strong>in</strong> the rehabil<strong>it</strong>ation of patients receiv<strong>in</strong>g aud<strong>it</strong>ory<br />
bra<strong>in</strong>stem implants. F<strong>in</strong>ally, s<strong>in</strong>ce <strong>it</strong>s creation <strong>in</strong> 1984,<br />
the Quebec Cochlear Implant Research Foundation<br />
has funded all new and research activ<strong>it</strong>ies <strong>in</strong> the field<br />
of aud<strong>it</strong>ory implants, and has also f<strong>in</strong>anced the cost of<br />
the first two aud<strong>it</strong>ory bra<strong>in</strong>stem implants.<br />
Patients who most often received the aud<strong>it</strong>ory bra<strong>in</strong>stem<br />
implant are those present<strong>in</strong>g w<strong>it</strong>h type 2 neurofibromatosis.<br />
These patients have benign tumours that<br />
destroy the aud<strong>it</strong>ory nerves, mak<strong>in</strong>g <strong>it</strong> impossible to<br />
use a standard hear<strong>in</strong>g aid or a cochlear implant due<br />
to the complete absence of the nerve. Some patients<br />
who have suffered from men<strong>in</strong>g<strong>it</strong>is result<strong>in</strong>g <strong>in</strong> total<br />
deafness, due to the destruction of the cochlear nerve<br />
and new bone <strong>format</strong>ion w<strong>it</strong>h<strong>in</strong> the cochlea, may also<br />
benef<strong>it</strong> from the aud<strong>it</strong>ory bra<strong>in</strong>stem implant.<br />
Follow<strong>in</strong>g surgery, recipients should receive speech<br />
therapy and audiology follow-up to relearn how to<br />
<strong>in</strong>terpret new sounds. Post-surgery studies show that<br />
84% of patients who have received these implants are<br />
satisfied w<strong>it</strong>h the results and would advise others to<br />
undergo the same procedure.<br />
Thus, patients present<strong>in</strong>g profound deafness <strong>in</strong> both<br />
ears w<strong>it</strong>h a destroyed cochlea and/or aud<strong>it</strong>ory nerve<br />
can benef<strong>it</strong> from this new aud<strong>it</strong>ory technology. W<strong>it</strong>hout<br />

It is difficult for parents<br />
to accept the<br />
notion that their<br />
child may have a<br />
hear<strong>in</strong>g loss, especially<br />
if he is <strong>in</strong> perfect<br />
health and no one else <strong>in</strong><br />
the family is hear<strong>in</strong>g impaired.<br />
Deafness <strong>in</strong> children often goes<br />
unnoticed from family members<br />
and health care professionals,<br />
until the child beg<strong>in</strong>s to show<br />
signs of delays <strong>in</strong> his or her<br />
language development. Generally<br />
speak<strong>in</strong>g, hear<strong>in</strong>g impairments<br />
are only detected after<br />
the age of two.<br />
Screen<strong>in</strong>g newborns’ hear<strong>in</strong>g<br />
<strong>in</strong> the days immediately follow<strong>in</strong>g<br />
birth can help lower the age at<br />
which they are diagnosed w<strong>it</strong>h a<br />
hear<strong>in</strong>g impairment and speed up<br />
treatment, potentially m<strong>in</strong>imiz<strong>in</strong>g<br />
the negative effects of the cond<strong>it</strong>ion<br />
on their development.<br />
The screen<strong>in</strong>g tests used are<br />
quick and pa<strong>in</strong>less for the baby.<br />
They are performed when the child<br />
is calm or asleep. The most widely<br />
used test is called an otoacoustic<br />
emission test and consists of record<strong>in</strong>g<br />
an echo produced by the ear <strong>in</strong><br />
response to an aud<strong>it</strong>ory stimulus.<br />
A t<strong>in</strong>y earplug is <strong>in</strong>serted <strong>in</strong>to the<br />
child’s ear and the device automatically<br />
records the response <strong>in</strong><br />
the m<strong>in</strong>utes that follow. The other<br />
test used is the aud<strong>it</strong>ory evoked<br />
potential test, which consists of<br />
record<strong>in</strong>g, by means of small electrodes<br />
placed on the baby’s head,<br />
the electrical impulses produced<br />
<strong>in</strong> the bra<strong>in</strong> when a quiet sound is<br />
played. As w<strong>it</strong>h the other test, the<br />
device automatically records the<br />
subsequent response.<br />
If the screen<strong>in</strong>g tests generate<br />
pos<strong>it</strong>ive results or are not performed<br />
properly, <strong>it</strong> does not necessarily<br />
mean the child has a hear<strong>in</strong>g<br />
impairment. At that po<strong>in</strong>t, more<br />
comprehensive hear<strong>in</strong>g tests will<br />
be needed to establish whether the<br />
child hears well. Performed by an<br />
audiologist, these tests are used to<br />
confirm or deny the presence of a<br />
hear<strong>in</strong>g impairment. If the child<br />
is <strong>in</strong> fact diagnosed w<strong>it</strong>h a hear<strong>in</strong>g<br />
impairment, the audiologist will<br />
discuss treatment options w<strong>it</strong>h the<br />
family based on the extent of the<br />
child’s hear<strong>in</strong>g loss, such as the use<br />
of hear<strong>in</strong>g aids.<br />
Québec will soon have <strong>it</strong>s first<br />
universal newborn hear<strong>in</strong>g<br />
screen<strong>in</strong>g program <strong>in</strong> place,<br />
provid<strong>in</strong>g all newborn babies<br />
free hear<strong>in</strong>g screen<strong>in</strong>g <strong>in</strong> their<br />
first days of life.<br />

The toys <strong>in</strong>dustry<br />
generates 60 billion<br />
dollars i n a n n u a l<br />
revenues and is a<br />
$1.4-billion bus<strong>in</strong>ess<br />
<strong>in</strong> Canada.<br />
Sixty per cent of all toy purchases<br />
a r e m a d e a t C h r i s t m a s t i m e<br />
alone. Anyone who is <strong>in</strong> contact<br />
w<strong>it</strong>h children knows that batterypowered<br />
sound-produc<strong>in</strong>g toys<br />
are extremely noisy and <strong>in</strong>creas<strong>in</strong>gly<br />
prevalent (63% of Fisher-<br />
Price toys and 54% of L<strong>it</strong>tle Tikes<br />
toys for children under the age<br />
of three make noise).<br />
The noise produced by such<br />
toys not only gets on parents’<br />
nerves, but some even reach<br />
decibel levels high enough to<br />
cause temporary or permanent<br />
hear<strong>in</strong>g damage.<br />
Studies have shown that, <strong>in</strong><br />
add<strong>it</strong>ion to caus<strong>in</strong>g aud<strong>it</strong>ory damage,<br />
noise also <strong>in</strong>creases children’s<br />
stress levels, which can lead to<br />
hormonal imbalances <strong>in</strong> the short<br />
term, and cardiovascular, immune<br />
system and digestive problems <strong>in</strong><br />
the long term.<br />
Current legislation <strong>in</strong> Canada<br />
bans toys and electronic games that<br />
em<strong>it</strong> noise levels exceed<strong>in</strong>g 100 dB<br />
at a distance of 30 cm. However,<br />
the safety standard recognized by<br />
the World Health Organization<br />
(WHO) is 75 dB for a period of<br />
8 hours. As such, listen<strong>in</strong>g to noise<br />
from a toy that em<strong>it</strong>s sound above<br />
100 dB for 2 to 15 m<strong>in</strong>utes can<br />
cause hear<strong>in</strong>g damage. Moreover,<br />
children tend to hold noisy toys<br />
close to their ears, which is closer<br />
than the distance prescribed by law.<br />
In 2004, a study led by Option<br />
consommateurs revealed that while<br />
only 5% of the toys studied did not<br />
comply w<strong>it</strong>h the law, all of them<br />
exceeded safety standards if the<br />
child holds the toy up aga<strong>in</strong>st his<br />
or her ear.<br />
In 2004, a coal<strong>it</strong>ion was formed<br />
to push for changes to the current<br />
legislation and educate the public<br />
about the dangers of noisy toys.<br />
Desp<strong>it</strong>e <strong>it</strong>s best efforts, the same<br />
laws, which date back to the 1970s,<br />
are still <strong>in</strong> force today.<br />

what do I do now?<br />
After a thorough assessment of your<br />
hear<strong>in</strong>g, you recently found out that<br />
you have a hear<strong>in</strong>g loss. Now that<br />
you’ve gone through a range of emotions,<br />
you suddenly f<strong>in</strong>d yourself full of questions.<br />
Know this: you are not alone! More<br />
than 10% of the population has some<br />
form of hear<strong>in</strong>g impairment.<br />
What should I do?<br />
First and foremost, you should make an<br />
appo<strong>in</strong>tment w<strong>it</strong>h an audioprosthetist.<br />
These are the only health professionals that<br />
sell, f<strong>it</strong> and adjust hear<strong>in</strong>g aids. Always on<br />

and degrees of hear<strong>in</strong>g loss, namely sensor<strong>in</strong>eural,<br />
conductive and mixed, rang<strong>in</strong>g<br />
from mild to profound. The lead<strong>in</strong>g<br />
cause of hear<strong>in</strong>g loss is presbycusis (natural<br />
degeneration of the ear). Presbycusis<br />
mostly affects the hair cells of the <strong>in</strong>ner<br />
ear. The hear<strong>in</strong>g aids recommended will<br />
depend on the type and degree of your<br />
hear<strong>in</strong>g loss.<br />
2 Assess<strong>in</strong>g your listen<strong>in</strong>g and<br />
communication needs<br />
This is one of the most crucial steps <strong>in</strong> the process. Here, the<br />
audioprosthetist will ask you a long list of questions to ga<strong>in</strong> a<br />
thorough understand<strong>in</strong>g of the activ<strong>it</strong>ies and moments <strong>in</strong> your<br />
day-to-day life dur<strong>in</strong>g which you have trouble understand<strong>in</strong>g<br />
what people are say<strong>in</strong>g. These questions might <strong>in</strong>clude:<br />

Choos<strong>in</strong>g batteries<br />
The cost of batteries also depends on their qual<strong>it</strong>y.<br />
Cheap batteries are available <strong>in</strong> some big-box stores<br />
and even on the Internet; however, cheaper often<br />
equates to poorer performance. <strong>To</strong> be sure your hear<strong>in</strong>g<br />
aids work properly, superior-qual<strong>it</strong>y batteries are<br />
a must.<br />
By skimp<strong>in</strong>g on someth<strong>in</strong>g as <strong>in</strong>expensive as batteries,<br />
you could significantly compromise the performance<br />
of your hear<strong>in</strong>g aids. S<strong>in</strong>ce hear<strong>in</strong>g aids operate<br />
us<strong>in</strong>g a dig<strong>it</strong>al circu<strong>it</strong> that analyzes the wearer’s sound<br />
environment on a cont<strong>in</strong>uous basis, they require a constant<br />
and stable power source.<br />
Know<strong>in</strong>g how to spot a high-qual<strong>it</strong>y battery<br />
As their name suggests, z<strong>in</strong>c-air batteries conta<strong>in</strong><br />
z<strong>in</strong>c, which is sealed <strong>in</strong>side the battery w<strong>it</strong>h a<br />
sticker. When the sticker is removed for the first<br />
time, the z<strong>in</strong>c comes <strong>in</strong>to contact w<strong>it</strong>h the air,<br />
t<strong>here</strong>by activat<strong>in</strong>g the battery.<br />
The battery should have one flat side and one<br />
rounded side (not two rounded sides, as w<strong>it</strong>h<br />
some brands, s<strong>in</strong>ce this can cause the battery<br />
contacts <strong>in</strong>side the hear<strong>in</strong>g aid to bend, and<br />
eventually weaken the contact).<br />
T<strong>here</strong> should be no glue residue left on the flat<br />
side (this is often an <strong>in</strong>dicator that the battery is<br />
not good or has been exposed to humid<strong>it</strong>y).<br />
Any battery should have small holes on the flat<br />
side (the side to which the sticker was affixed),<br />
s<strong>in</strong>ce this is what allows air to enter and activate<br />
the z<strong>in</strong>c.<br />

In today’s urban, <strong>in</strong>dustrialized society,<br />
noise is pervasive <strong>in</strong> both the<br />
natural landscape and the workplace.<br />
Repet<strong>it</strong>ive noise exposure<br />
causes irreversible damage to the<br />
aud<strong>it</strong>ory organ (cochlea), result<strong>in</strong>g<br />
<strong>in</strong> permanent, sensor<strong>in</strong>eural hear<strong>in</strong>g loss.<br />
The CSST (Commission de la santé et de la<br />
sécur<strong>it</strong>é du travail) has set up a workers’ compensation<br />
program specifically for people who<br />
work <strong>in</strong> noisy environments. Under this program,<br />
workers can even get reimbursed for the costs of<br />
some hear<strong>in</strong>g aids.<br />
The CSST representative re<strong>view</strong><strong>in</strong>g your file will first<br />
establish whether your hear<strong>in</strong>g loss is a direct result of<br />
occupational noise exposure. If so, the ENT specialist<br />
will wr<strong>it</strong>e up a medical report for the CSST. This<br />
report <strong>in</strong>cludes calculations for f<strong>in</strong>ancial compensation<br />
(if eligible), and recommendations for b<strong>in</strong>aural hear<strong>in</strong>g<br />
aids (barr<strong>in</strong>g any medical contra<strong>in</strong>dications aga<strong>in</strong>st<br />
hear<strong>in</strong>g aids).<br />
Follow<strong>in</strong>g the f<strong>in</strong>al approval of your file, the CSST<br />
representative will send you a letter <strong>in</strong>v<strong>it</strong><strong>in</strong>g you to consult<br />
an audioprosthetist to choose the hear<strong>in</strong>g aids best<br />
su<strong>it</strong>ed to your listen<strong>in</strong>g and communication needs.<br />
Some hear<strong>in</strong>g aid models are covered by the CSST:<br />
ITC (half-shell), BTE (beh<strong>in</strong>d the ear w<strong>it</strong>h earmold),<br />
Open F<strong>it</strong> (beh<strong>in</strong>d the ear w<strong>it</strong>h tube and dome) or<br />
RITE-RIC (beh<strong>in</strong>d the ear w<strong>it</strong>h receiver <strong>in</strong> the ear).<br />
S<strong>in</strong>ce their release on the market, RITE-RIC models<br />
have become the lead<strong>in</strong>g choice for persons w<strong>it</strong>h<br />
noise-<strong>in</strong>duced hear<strong>in</strong>g loss. These k<strong>in</strong>ds of hear<strong>in</strong>g aids<br />
comb<strong>in</strong>e style, comfort and performance. The CSST<br />
covers the cost of one or two hear<strong>in</strong>g aids, based on the<br />
recommendations made by the ENT specialist <strong>in</strong> the<br />
report rendered to the CSST.<br />
Hear<strong>in</strong>g aids can be replaced every five years. You<br />
should nevertheless contact your <strong>in</strong>surance company to<br />
<strong>in</strong>sure your hear<strong>in</strong>g aids aga<strong>in</strong>st major loss or damage<br />
through your home <strong>in</strong>surance. When you receive your<br />
hear<strong>in</strong>g aids, a specialized educator and your audioprosthetist<br />
will gladly expla<strong>in</strong> how to have your hear<strong>in</strong>g<br />
aids <strong>in</strong>sured.<br />
In add<strong>it</strong>ion to cover<strong>in</strong>g your hear<strong>in</strong>g test (audiogram)<br />
and hear<strong>in</strong>g aids, the CSST program also<br />
covers the cost of batteries, ma<strong>in</strong>tenance products,<br />
listen<strong>in</strong>g systems for the television (subject to certa<strong>in</strong><br />
restrictions) and many other considerations.<br />
Do not hes<strong>it</strong>ate to consult an audiologist or audioprosthetist<br />

Here are the follow<strong>in</strong>g steps if you need to be f<strong>it</strong>ted<br />
w<strong>it</strong>h a hear<strong>in</strong>g aid.<br />
First, you’ll have to make an appo<strong>in</strong>tment w<strong>it</strong>h an<br />
audiologist, who will determ<strong>in</strong>e the extent of your<br />
noise-<strong>in</strong>duced hear<strong>in</strong>g loss. Dur<strong>in</strong>g the consultation,<br />
the audiologist will perform an audiogram and fill<br />
out the workers’ compensation form. Sixteen hours<br />
of reprieve from noise is required <strong>in</strong> order to undergo<br />
an audiogram. Once the exam is done, the audiologist<br />
will refer you to an ENT specialist to beg<strong>in</strong> the process<br />

out, go<strong>in</strong>g to work or tak<strong>in</strong>g part<br />
<strong>in</strong> social out<strong>in</strong>gs, premium hear<strong>in</strong>g<br />
aids can meet your needs more<br />
effectively. Not only do premium<br />
models offer better noise cancellation<br />
capabil<strong>it</strong>y, they also tune <strong>in</strong><br />
more easily to the most important<br />
signals (speech) than their entrylevel<br />
counterparts.<br />
Naturally, the more advanced<br />
the technology, the faster the hear<strong>in</strong>g<br />

Amigo<br />
FM systems<br />
Understand<strong>in</strong>g speech <strong>in</strong> noisy<br />
environments can be difficult for<br />
most children, and proves an even<br />
greater challenge for children w<strong>it</strong>h<br />
a hear<strong>in</strong>g impairment. Nevertheless,<br />
all children must be able to<br />
access speech for language development and<br />
learn<strong>in</strong>g.<br />
Background noise, distance and reverberation are<br />
the three obstacles that significantly reduce children’s<br />
access to speech.<br />
These three obstacles are often present <strong>in</strong> a classroom,<br />
negatively affect<strong>in</strong>g sound qual<strong>it</strong>y and learn<strong>in</strong>g<br />
abil<strong>it</strong>y. Although hear<strong>in</strong>g aids are <strong>in</strong>creas<strong>in</strong>gly effective<br />
at improv<strong>in</strong>g the qual<strong>it</strong>y, audibil<strong>it</strong>y and clar<strong>it</strong>y of the<br />
signal, they cannot elim<strong>in</strong>ate all the factors that affect<br />
speech <strong>in</strong>telligibil<strong>it</strong>y. By send<strong>in</strong>g a voice signal directly<br />
to the children’s hear<strong>in</strong>g aids or headphones, the FM<br />
systems help overcome these three obstacles.<br />
About Amigo Arc<br />
The Amigo Arc is a universal<br />
<strong>in</strong>duction loop receiver that<br />
connects the FM transm<strong>it</strong>ter<br />
(worn by the teacher) to<br />
the student’s hear<strong>in</strong>g aids.<br />
The teacher’s voice is captured by the transm<strong>it</strong>ter’s<br />
microphone and sent directly to the Amigo Arc, subsequently<br />

About Amigo T5/R5<br />
The Amigo T5/R5 is an FM un<strong>it</strong>. S<strong>in</strong>ce<br />
<strong>it</strong> is worn w<strong>it</strong>h headphones, <strong>it</strong> is ideal for<br />
children w<strong>it</strong>h m<strong>in</strong>imal hear<strong>in</strong>g loss who<br />
are unable to wear hear<strong>in</strong>g aids, those<br />
w<strong>it</strong>h a central aud<strong>it</strong>ory process<strong>in</strong>g disorder<br />
(CAPD) or even those w<strong>it</strong>h attention<br />
defic<strong>it</strong> disorder (ADD). The T5 transm<strong>it</strong>ter<br />
is worn by the teacher and sends a signal directly<br />
to the R5 receiver on the headphones worn by the<br />

About Amigo T30/T31<br />
The Amigo T30 is the ideal FM transm<strong>it</strong>ter<br />
for send<strong>in</strong>g the speaker’s signal<br />
directly to the hear<strong>in</strong>g aids. Designed for<br />
classroom use, this device is extremely<br />
robust and reliable. The teacher’s voice<br />
is captured by the T30 microphone and<br />
sent directly to the sabots connected to<br />
the student’s hear<strong>in</strong>g aids. The transm<strong>it</strong>ter’s<br />
built-<strong>in</strong> LED light and small receivers connected<br />
to the hear<strong>in</strong>g aids provide visual confirmation that<br />
the system is work<strong>in</strong>g properly and that the student<br />
is clearly receiv<strong>in</strong>g the signal <strong>in</strong> question. The Amigo<br />
T31 works <strong>in</strong> the same manner as the Amigo T30,<br />
except <strong>it</strong> also features a built-<strong>in</strong> team teach<strong>in</strong>g function,<br />

About Amigo T10<br />
The Amigo T10 transm<strong>it</strong>ter, like the<br />

offers multiple microphone options, and may be worn<br />
around the neck or placed on a table for everyone<br />

Residential and long-term care centres:<br />
Negotiat<strong>in</strong>g the maze<br />

Better known as<br />
CHSLDs, these centres<br />
are a topic of<br />
discussion <strong>in</strong> the<br />
media and w<strong>it</strong>h our<br />
families, s<strong>in</strong>ce any<br />
elderly person w<strong>it</strong>h decreas<strong>in</strong>g<br />
<strong>in</strong>dependence could end up<br />
t<strong>here</strong> one day. However, a certa<strong>in</strong><br />
amount of confusion reigns<br />
over the exact ident<strong>it</strong>y of these<br />
establishments, which used<br />
to be called nurs<strong>in</strong>g homes or<br />
old-age homes.<br />
T<strong>here</strong> are, <strong>in</strong> fact, three types of<br />
establishments under the CHSLD<br />
umbrella. The first, public establishments,<br />
are owned and operated<br />
by the State. Next, t<strong>here</strong> are private<br />
<strong>in</strong>st<strong>it</strong>utions that belong to private<br />
enterprise and offer the same services<br />
identified by the M<strong>in</strong>istère<br />
de la Santé et des Services sociaux,<br />
w<strong>it</strong>h accommodation costs identical<br />
to those of a public CHSLD.<br />
The difference between the two lies<br />
at the adm<strong>in</strong>istrative level. Private<br />
CHSLDs under agreement are<br />
responsible for the adm<strong>in</strong>istrative<br />
aspects concern<strong>in</strong>g the ownership<br />
and management of the build<strong>in</strong>g,<br />
w<strong>it</strong>hout State <strong>in</strong>volvement.<br />
Lastly, t<strong>here</strong> are private CHSLDs<br />
not under agreement. These centres<br />
must obta<strong>in</strong> a perm<strong>it</strong> from the<br />
M<strong>in</strong>istère de la Santé et des Services<br />
sociaux authoriz<strong>in</strong>g them to adm<strong>it</strong><br />
persons w<strong>it</strong>h loss of autonomy.<br />
They do not receive any fund<strong>in</strong>g<br />
from the M<strong>in</strong>istère. In fact, the<br />
owners are free to establish their<br />
own prices, rules and cr<strong>it</strong>eria.<br />
Accommodation costs differ<br />
widely among private CHSLDs,<br />
w<strong>here</strong> the greater the loss of<br />
autonomy, the greater the bill<br />
payable by the resident is likely<br />
to be. This is not the case w<strong>it</strong>h<br />
public CHSLDs and private<br />
CHSLDs under agreement<br />
(which receive fund<strong>in</strong>g), w<strong>here</strong><br />
accommodation costs are set<br />
every year by the Régie de<br />
l’assurance maladie du Québec<br />
and adjusted accord<strong>in</strong>g to<br />
the resident’s abil<strong>it</strong>y to pay.<br />
As regards to tax cred<strong>it</strong>s for<br />
home-support services for seniors,<br />
persons aged 70 or older liv<strong>in</strong>g <strong>in</strong> a<br />
health establishment (public or private<br />
under agreement) may claim<br />
certa<strong>in</strong> services not offered by the<br />
establishment and which are purchased<br />
à la carte or by contract.<br />
As for private CHSLDs not under<br />
agreement, eligible services are the<br />
same as those offered <strong>in</strong> residences<br />
for seniors. For details on eligible<br />
and excluded services, please contact<br />
Revenu Québec.<br />

Pharmacist’s report<br />
The use of R<strong>it</strong>al<strong>in</strong> ® <strong>in</strong> the<br />
treatment of ADD/ADHD<br />

R<strong>it</strong>al<strong>in</strong> ® is one of<br />
t h e b e s t - k n o w n<br />
medications used<br />
to treat cases of<br />
Attention Defic<strong>it</strong><br />
Disorder w<strong>it</strong>h or<br />
w<strong>it</strong>hout Hyperactiv<strong>it</strong>y (ADD/<br />
ADHD) <strong>in</strong> both children and<br />
adults alike. The active <strong>in</strong>gredient<br />
<strong>in</strong> R<strong>it</strong>al<strong>in</strong> ® is called methylphenidate.<br />
Many concerns have<br />
been raised w<strong>it</strong>h respect to<br />
the use of the drug, which has<br />
<strong>in</strong>creased dramatically over the<br />
last 20 years.<br />
Methylphenidate, first synthesized<br />
by the chemist Leandro Panizzon,<br />
was named after his wife,<br />
R<strong>it</strong>a, and marketed as R<strong>it</strong>al<strong>in</strong> ® <strong>in</strong><br />
1954. R<strong>it</strong>al<strong>in</strong> ® was <strong>in</strong><strong>it</strong>ially used to<br />
treat depression and chronic fatigue<br />
before be<strong>in</strong>g used <strong>in</strong> the 1960s to<br />
treat children and adults w<strong>it</strong>h ADD/<br />
ADHD. <strong>To</strong>day, methylphenidate<br />
can be found <strong>in</strong> long-act<strong>in</strong>g capsules<br />
(once daily dosage) under the<br />
name Concerta ® or Biphent<strong>in</strong> ® .<br />
Persons w<strong>it</strong>h ADD/ADHD<br />
have difficulty concentrat<strong>in</strong>g and<br />
complet<strong>in</strong>g activ<strong>it</strong>ies w<strong>it</strong>h vary<strong>in</strong>g<br />
degrees of complex<strong>it</strong>y. They often<br />
f<strong>in</strong>d <strong>it</strong> difficult to rema<strong>in</strong> still and<br />
frequently act on impulse.<br />
Of course, these behaviours may<br />
be present <strong>in</strong> all <strong>in</strong>dividuals; however,<br />
they occur <strong>in</strong> a more abnormally<br />
pronounced manner and<br />
for extended periods of time w<strong>it</strong>h<br />
ADD/ADHD sufferers. These<br />
symptoms also appear <strong>in</strong> all life<br />
s<strong>it</strong>uations (not only at school or at<br />
home, for example).<br />
The cerebral stimulation triggered by R<strong>it</strong>al<strong>in</strong> ® helps transm<strong>it</strong> messages<br />
between the bra<strong>in</strong>’s nerve cells, thus improv<strong>in</strong>g concentration and memory,<br />
while manag<strong>in</strong>g anger and frustration. This drug must be <strong>in</strong>troduced<br />
gradually (w<strong>it</strong>h a slow <strong>in</strong>crease <strong>in</strong> dosage) to dim<strong>in</strong>ish the onset of side<br />
effects, w<strong>it</strong>h loss of appet<strong>it</strong>e, sleep problems and irr<strong>it</strong>abil<strong>it</strong>y be<strong>in</strong>g the most<br />
frequent. The new, long-act<strong>in</strong>g capsules also help control symptoms, while<br />
mak<strong>in</strong>g <strong>it</strong> easier to swallow the medication.<br />
Nevertheless, the treatment of ADD/ADHD must not focus solely on<br />
medication. A psychosocial approach that <strong>in</strong>cludes establish<strong>in</strong>g a<br />
well-organized schedule or add<strong>in</strong>g educational activ<strong>it</strong>ies, as needed,<br />
will facil<strong>it</strong>ate the patient’s adaptation. Also, “Attentix,” a program<br />
developed by a Québec psychologist (www.attentix.ca), is available<br />
to parents and educators. <strong>To</strong> ensure the best treatment, <strong>it</strong> is important<br />
to accurately dist<strong>in</strong>guish between ADD/ADHD and Aud<strong>it</strong>ory<br />
Process<strong>in</strong>g Disorder (APD), which share behavioural similar<strong>it</strong>ies.<br />
Controversy has surrounded the grow<strong>in</strong>g use of R<strong>it</strong>al<strong>in</strong> ® s<strong>in</strong>ce the early<br />
1990s. Some believe that the explosion <strong>in</strong> demand for this drug is due to<br />
the <strong>in</strong>sistence of the patient’s family. This should t<strong>here</strong>fore be justified by a<br />
steady <strong>in</strong>crease <strong>in</strong> ADD/ADHD cases, but <strong>it</strong> appears that the behavioural<br />
problem today is not significantly greater than <strong>it</strong> was <strong>in</strong> the past.<br />
Others believe that the <strong>in</strong>creased use of R<strong>it</strong>al<strong>in</strong> ® is due to a better<br />
understand<strong>in</strong>g of the disorder and thus a better diagnosis. An ADD/<br />
ADHD diagnosis must <strong>in</strong>clude several evaluations to elim<strong>in</strong>ate other<br />
problems, such as depression, anxiety or hear<strong>in</strong>g loss. The observations of<br />
parents and teachers play an important role.<br />
The advantages and disadvantages of treat<strong>in</strong>g ADD/ADHD w<strong>it</strong>h<br />
R<strong>it</strong>al<strong>in</strong> ® are multiple. It is important to lim<strong>it</strong> <strong>it</strong>s use to children who really<br />
need the drug. The treatment of ADD/ADHD helps ensure adequate<br />
social function<strong>in</strong>g, and the use of medication rema<strong>in</strong>s one solution among<br />

We may have heard this uttered time<br />
and time aga<strong>in</strong> as children, but this<br />
adage takes on a whole new mean<strong>in</strong>g<br />
<strong>in</strong> adulthood, as we understand<br />
all t<strong>here</strong> is to lose. Sadly, each<br />
year, many people’s homes are<br />
destroyed, along w<strong>it</strong>h all of their belong<strong>in</strong>gs, <strong>in</strong><br />
house fires.<br />
Not hear<strong>in</strong>g a smoke detector or fire alarm go off,<br />
especially at night, is a genu<strong>in</strong>e concern for people w<strong>it</strong>h<br />
total or partial hear<strong>in</strong>g loss, s<strong>in</strong>ce most sleep w<strong>it</strong>hout<br />
their hear<strong>in</strong>g aids. However, did you know that, <strong>in</strong><br />
some Québec towns 1 , you can have yourself added to<br />
the local fire department’s registry of persons w<strong>it</strong>h disabil<strong>it</strong>ies<br />
2 . Such registries let the firefighters called to a<br />
scene know whether t<strong>here</strong> is someone <strong>in</strong>side who may<br />
