How do I apply <strong>for</strong> a go4less <strong>card</strong>?<br />
Complete the application <strong>for</strong>m on this leaflet <strong>and</strong> take it with<br />
all the necessary documentation <strong>and</strong> payment to your local<br />
<strong>Active</strong> <strong>Luton</strong> centre (see over). You can also download the<br />
application <strong>for</strong>m from www.activeluton.co.uk<br />
You must apply <strong>for</strong> the <strong>card</strong> in person. If your partner wishes<br />
to apply <strong>for</strong> a <strong>card</strong>, they must be present at the time of<br />
application with appropriate documentation including bills<br />
showing both names at your address.<br />
If you wish to apply <strong>for</strong> a <strong>card</strong> <strong>for</strong> your child, they must be<br />
present at the time of application with their student union<br />
<strong>card</strong>, birth certificate or letter of confirmation from their<br />
school/college to confirm their age.<br />
