and <strong>Fitness</strong> <strong>to</strong> <strong>Practise</strong><br />
Declaration<br />
I confirm that I have received and read a copy <strong>of</strong> <strong>the</strong> Code <strong>of</strong><br />
Conduct.<br />
Its significance has been explained <strong>to</strong> me and I agree <strong>to</strong> abide<br />
by <strong>the</strong> code throughout my period <strong>of</strong> time as a pharmacy<br />
student at <strong>Medway</strong> <strong>School</strong> <strong>of</strong> <strong>Pharmacy</strong>.<br />
I understand that <strong>the</strong> pr<strong>of</strong>ession <strong>of</strong> pharmacy is exempt from <strong>the</strong><br />
Rehabilitation <strong>of</strong> Offenders Act with regard <strong>to</strong> „spent‟ convictions<br />
or criminal investigations.<br />
I am aware that any in<strong>for</strong>mation that I have supplied or that<br />
arises from any fitness <strong>to</strong> practise proceedings involving me is<br />
not subject <strong>to</strong> <strong>the</strong> requirements <strong>of</strong> <strong>the</strong> Data Protection Act and<br />
may be notified <strong>to</strong> <strong>the</strong> pr<strong>of</strong>essional regula<strong>to</strong>r <strong>for</strong> pharmacy.<br />

practise and failure <strong>to</strong> submit your declaration will be followed up.<br />
