Welcome to <strong>Express</strong> <strong>Scripts</strong>, the company <strong>Bluegrass</strong> <strong>Family</strong> <strong>Health</strong> has selected to manage your<br />
prescription-drug benefit. As one of the nation’s leading pharmacy benefit managers, <strong>Express</strong> <strong>Scripts</strong><br />
administers prescription-drug benefits for more than 75 million Americans with the goal of making the<br />
prescription drugs they take safer and more affordable.<br />
Your prescription-drug coverage through <strong>Express</strong> <strong>Scripts</strong> begins on July 1. You will be able to fill<br />
your current prescriptions through our vast network which includes more than 62,000 pharmacies.<br />
Important information for your pharmacist<br />
New ID cards will be sent to you within the next few weeks. If you need a prescription filled before you<br />
receive your ID card, please present this <strong>letter</strong> to the pharmacist.<br />
Please follow the action steps listed below to enter the claim.<br />
Step 1 Enter Bin # 003858<br />
Step 2 Enter Processor Control A4<br />
Step 3 Enter Rx Group #: K9RA<br />
Step 4 Enter 6 digit Sub ID and 2 digit Person No. (same ID<br />
number used prior to 7/1/11)<br />
Step 5 Enter the member’s date of birth<br />
Attention pharmacist: To simplify your prescription processing, please link the cardholder<br />
and all members of their family to <strong>Express</strong> <strong>Scripts</strong>. If you have any questions while<br />
processing the claim, please call the <strong>Express</strong> <strong>Scripts</strong> Pharmacy Help Desk at 800.824.0898.<br />

If you use a mail order pharmacy<br />
If you are currently receiving maintenance medications from the CVS/Caremark mail order pharmacy,<br />
<strong>Bluegrass</strong> <strong>Family</strong> <strong>Health</strong> has arranged for all of your active prescriptions with available refills to be<br />
transferred to the <strong>Express</strong> <strong>Scripts</strong> Pharmacy on July 1. There may be a brief delay in receiving your<br />
maintenance medications while the prescriptions are being transferred and loaded into the system, so<br />
please plan accordingly. Please note, as mandated by law, certain prescriptions will not transfer. These<br />
include prescriptions for controlled-substance drugs or compounded drugs and any prescriptions over one<br />
year old. If this applies to any of your current medications, you will need to get a new prescription from<br />
your doctor.<br />
