convenience and value, plus the comfort of knowing you<br />

can access legal services for almost all personal legal<br />

matters. And there are no limits on the number of times<br />

you can use the plan…and no dollar limit on your use<br />

of a plan attorney for a wide range of legal matters (see<br />

Covered Services).<br />

The <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong> is offered through <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong>s,<br />

a MetLife company.<br />

How the <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong> works<br />

When you enroll in the <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong>, it’s like having<br />

your own lawyer on retainer! No deductible or copay, no<br />

waiting periods, no claim forms and no limits on usage<br />

– as long as you use a plan attorney (an attorney who is<br />

part of the <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong>).<br />

As a member of the <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong> you’ll have access<br />

to its network of more than 10,000 attorneys. Each one<br />

meets the plan’s stringent selection criteria and, on<br />

average, has 22 years of experience. Whenever you need<br />

legal assistance contact the <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong>:<br />

• Online – go to www.legalplans.com any time, day or<br />

night, or<br />

• By phone – call (800) 821-6400 between 7 a.m. and 6<br />

p.m. Central time, Monday–Friday.<br />

You’ll receive the name, address and phone number of<br />

plan attorneys in your area, as well as a case number.<br />

Contact any one of these attorneys to schedule an<br />

appointment. And remember…as long as you see an<br />

attorney who is a member of the <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong> network,<br />

there’s no deductible or copay, no waiting period, no claim<br />

forms and no limits on usage for covered services.<br />

Important! You have the option of using an attorney<br />

who is not in the plan’s network; in this case, you will<br />

receive reimbursement of your attorney’s fees – up to<br />

certain amounts for covered services according to a<br />

set fee schedule. If you choose to use a non-network<br />

attorney, you must contact <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong>s and request<br />

a claim form and a copy of the current fee reimbursement<br />

schedule. You will be responsible for any difference<br />

between what the non-network attorney charges and the<br />

amount you are reimbursed by the plan.<br />

Covered services<br />

Attorney fees for the following personal legal services<br />

are fully paid by the <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong> when you use a<br />

plan attorney (an attorney who is a member of the <strong>Hyatt</strong><br />

<strong>Legal</strong> <strong>Plan</strong> network). There are no limits on the number<br />

of times you may use the plan, and there are no dollar<br />

limits on your use of plan attorney for the following<br />

<strong>Legal</strong> services that are not covered<br />

The plan does not provide any benefits (including<br />

consultations) related to the following:<br />

• Employment-related matters, including company or<br />

statutory benefits<br />

• Matters involving the employer, MetLife® and affiliates,<br />

and plan attorneys<br />

• Matters in which there is a conflict of interest between<br />

the employee and spouse/domestic partner or<br />

dependents, in which case services are excluded for the<br />

spouse/domestic partner and dependents<br />

• Appeals and class actions<br />

• Farm and business matters, including rental issues when<br />

the participant is the landlord<br />

• Patent, trademark and copyright matters<br />

• Costs and fines<br />

• Frivolous or unethical matters<br />

• Matters for which an attorney-client relationship<br />

exists prior to the participant becoming eligible for<br />

plan benefits.<br />

Cost of coverage<br />

You pay for group legal plan coverage with after-tax<br />

deductions from your paycheck. This cost provides<br />

coverage for yourself, your spouse or domestic partner<br />

and your eligible dependents.<br />

Ready to enroll?<br />

If you:<br />

• Are a newly-hired <strong>Advocate</strong> or <strong>Advocate</strong> BroMenn<br />

associate at any location (or move into a benefits eligible<br />

position) and you enroll in the <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong> during<br />

your initial enrollment period – your coverage under the<br />

<strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong> will begin on the first day of the month<br />

following your enrollment period.<br />

• Enroll during any subsequent Annual Enrollment period<br />

– your coverage under the <strong>Hyatt</strong> <strong>Legal</strong> <strong>Plan</strong> will begin on<br />

the subsequent January 1.<br />

Once enrolled, your coverage under the <strong>Hyatt</strong> <strong>Legal</strong><br />

<strong>Plan</strong> will remain in effect until you elect to drop this<br />

coverage during a subsequent Annual Enrollment or, if<br />

sooner, your <strong>Advocate</strong> employment and/or your benefits<br />

eligibility ends.<br />

Do I have to use a plan attorney?<br />

reimbursement of your attorney’s fees and the benefits<br />
