A plan to fit your needs<br />
The UNITEDHEALTH Rx BASIC plan helps you take charge of your prescription drug<br />
costs, with predictability, convenience and ease of use. This plan is a good choice if<br />
you take many prescription drugs or if you think your prescription drug needs will<br />
change in the future. It includes every brand-name prescription drug or its generic<br />
version covered by Medicare Part D. It features:<br />
• No annual deductible.<br />
• Stable <strong>for</strong>mulary – We won’t remove<br />
drugs during the year. 2<br />
• Preferred Mail Service Pharmacy –<br />
Receive a 90-day supply of your<br />
Tier 1 medications <strong>for</strong> $0 copay. 1<br />
No charge <strong>for</strong> standard delivery.<br />
• Convenient payment options – Pay<br />
your premium with electronic funds<br />
transfer (EFT), by check, or by having<br />
your payment deducted from your<br />
Social Security check.<br />

• The Annual Enrollment period runs from November 15 – December 31<br />
each year. This is the time you can enroll in a new plan or switch from<br />
your current plan.<br />
• Once you’re Medicare eligible, you may enroll in a Part D plan. If you don’t,<br />
you may have to pay the Late Enrollment Penalty.<br />
• The UnitedHealth Rx Basic plan helps you manage rising<br />
drug costs.<br />

You are not required to use the plan’s Preferred Mail Service Pharmacy to obtain a supply of your maintenance medications. You have<br />
the option of using the Preferred Mail Service Pharmacy, a retail extended day supply pharmacy or non-preferred mail service pharmacy<br />
in the network to obtain a supply of maintenance medications.<br />
If you choose a retail extended day supply pharmacy or non-preferred mail service pharmacy, you may see out-of-pocket payment<br />
differences when compared to using the Preferred Mail Service Pharmacy. You should experience no out-of-pocket payment differences<br />
if choosing between a retail extended day supply pharmacy and a non-preferred mail service pharmacy.<br />
Please call our Customer Care Associates 24 hours a day, 7 days a week <strong>for</strong> up-to-date in<strong>for</strong>mation on which pharmacies are in<br />
the network.<br />
Your prescriptions should arrive in about seven days from the date the completed order is received by Prescription Solutions. If<br />
Prescription Solutions needs to contact you or your prescribing physician to clarify in<strong>for</strong>mation on your order or to request prescriptions<br />
from your physician, delivery may take longer. If you prefer rush delivery, medications can be shipped overnight <strong>for</strong> an additional charge.<br />
You should fill your prescriptions locally if you are out of your medications and cannot wait <strong>for</strong> your mail order prescriptions to arrive.<br />
Standard delivery is no charge to U.S. addresses, including U.S. territories. For copay amounts, drug coverage in<strong>for</strong>mation or general<br />
plan questions, please call Customer Care at 1-877-286-5420, 24 hours a day, 7 days a week, TTY 1-877-730-4203.<br />
If you qualify <strong>for</strong> extra help, you will not have a late-enrollment penalty (LEP). Also, if you have other prescription coverage at least as<br />
good as Medicare (also known as creditable coverage), you may not be assessed a LEP.<br />
