The <strong>Arkansas</strong> Pharmac<strong>is</strong>ts Associati<strong>on</strong> <strong>is</strong> saddened by the recent deaths from men<strong>in</strong>git<strong>is</strong> traced<br />

back to steroid <strong>in</strong>jecti<strong>on</strong>s from New England <strong>Compound<strong>in</strong>g</strong> Center of Fram<strong>in</strong>gham, MA.<br />

The <strong>Arkansas</strong> State Board of Pharmacy actively regulates compound<strong>in</strong>g pharmacies <strong>in</strong> <strong>Arkansas</strong>,<br />

sets high standards and actively <strong>in</strong>spects pharmacies <strong>on</strong> an annual bas<strong>is</strong>. There are very few states<br />

that have the same degree of oversight. There are many pharmac<strong>is</strong>ts <strong>in</strong> <strong>Arkansas</strong> who safely<br />

compound life-sav<strong>in</strong>g medicati<strong>on</strong>s for their patients every day.<br />

The <strong>Arkansas</strong> Pharmac<strong>is</strong>ts Associati<strong>on</strong> has 94 pharmac<strong>is</strong>ts <strong>in</strong> its <strong>Compound<strong>in</strong>g</strong> Academy, a network<br />

for pharmac<strong>is</strong>ts who work <strong>in</strong> compound<strong>in</strong>g. Dr. Kr<strong>is</strong>ten Riddle of U.S. <strong>Compound<strong>in</strong>g</strong> <strong>in</strong> C<strong>on</strong>way <strong>is</strong><br />

the current president of the <strong>Compound<strong>in</strong>g</strong> Academy.<br />

<strong>What</strong> <strong>is</strong> Pharmaceutical <strong>Compound<strong>in</strong>g</strong><br />

<strong>Compound<strong>in</strong>g</strong> can be as simple as rec<strong>on</strong>stitut<strong>in</strong>g a dry powder or mix<strong>in</strong>g two creams together, all<br />

the way to creat<strong>in</strong>g highly soph<strong>is</strong>ticated sterile products.<br />

<strong>Compound<strong>in</strong>g</strong> <strong>is</strong> an important and l<strong>on</strong>g-stand<strong>in</strong>g practice that meets important patient needs <strong>in</strong><br />

hospitals, nurs<strong>in</strong>g homes and many community sett<strong>in</strong>gs. Many patients have unique medical needs<br />

that pharmac<strong>is</strong>ts and physicians cannot meet us<strong>in</strong>g commercially available medicati<strong>on</strong> dosage<br />

forms.<br />

Some patients need strengths, liquid dosage forms or comb<strong>in</strong>ati<strong>on</strong>s of <strong>in</strong>gredients that are not<br />

available from manufacturers. In additi<strong>on</strong>, compounders have compounded many life-sav<strong>in</strong>g drugs<br />

<strong>in</strong> the wake of the drug shortages that have impacted hospitals and other health care facilities. The<br />

American Society of Health- System Pharmac<strong>is</strong>ts ma<strong>in</strong>ta<strong>in</strong>s a web page that l<strong>is</strong>ts new drug<br />

shortages regularly.<br />

Compounders take f<strong>in</strong><strong>is</strong>hed drugs or bulk powders and scientifically manipulate them <strong>in</strong>to unique,<br />

unavailable dosage forms. Compounds are made by pharmac<strong>is</strong>ts <strong>in</strong> local pharmacies, not drug<br />

manufacturers.<br />

Compounders depend <strong>on</strong> U.S- based chemical companies like Professi<strong>on</strong>al <strong>Compound<strong>in</strong>g</strong> Centers of<br />

America (PCCA) to source top quality chemicals.<br />

Who Regulates <strong>Compound<strong>in</strong>g</strong><br />

Compounders are regulated by State Boards of Pharmacy like the <strong>Arkansas</strong> State Board of<br />

Pharmacy.<br />

The Food and Drug Adm<strong>in</strong><strong>is</strong>trati<strong>on</strong> (FDA) provides oversight through regulati<strong>on</strong> of the <strong>in</strong>tegrity of<br />

the drugs and of the Active Pharmaceutical Ingredients (APIs) ordered, stored, and used <strong>in</strong><br />

compounds. Pharmac<strong>is</strong>ts who compound are expected to follow appropriate procedures for the<br />

types of products compounded.<br />

The Drug Enforcement Agency (DEA) regulates handl<strong>in</strong>g of c<strong>on</strong>trolled substances used <strong>in</strong> the<br />

preparati<strong>on</strong> of compounded medicati<strong>on</strong>s.<br />

An <strong>in</strong>dependent organizati<strong>on</strong> called United States Pharmacopeia (USP) sets standards for sterile<br />

compound<strong>in</strong>g. USP Chapter 797 <strong>is</strong> <strong>is</strong>sued by U.S. Pharmacopoeia and applies to any pharmacy that<br />

prepares “compounded sterile preparati<strong>on</strong>s” (CSPs). These standards have been <strong>in</strong>corporated <strong>in</strong>to<br />

<strong>Arkansas</strong> law and are required practice <strong>in</strong> <strong>Arkansas</strong>.<br />

Pharmacies can voluntarily become accredited by the Pharmacy <strong>Compound<strong>in</strong>g</strong> Accreditati<strong>on</strong> Board<br />

(PCAB). PCAB Accreditati<strong>on</strong> denotes a more str<strong>in</strong>gent, comprehensive standard of quality <strong>in</strong><br />

compound<strong>in</strong>g. PCAB provides compound<strong>in</strong>g practices an opportunity to compare their practices<br />

aga<strong>in</strong>st nati<strong>on</strong>al standards and achieve accreditati<strong>on</strong> that provides c<strong>on</strong>sumers an <strong>in</strong>dicator of<br />

quality practices.<br />

How Does <strong>Arkansas</strong> Regulate <strong>Compound<strong>in</strong>g</strong> Pharmacies<br />

<strong>Arkansas</strong> State Board of Pharmacy adopted USP Chapter 797 <strong>in</strong> 2003 and requires both sterility and<br />

potency test<strong>in</strong>g. USP Chapter 797 <strong>is</strong> a far-reach<strong>in</strong>g standard that governs a wide range of pharmacy<br />

policies and procedures. It <strong>is</strong> designed both to <strong>in</strong>crease quality to patients through pharmaceutical<br />

products and to better protect staff work<strong>in</strong>g <strong>in</strong> pharmacies <strong>in</strong> the course of their exposure to<br />

pharmaceuticals.<br />

The <strong>Arkansas</strong> State Board of Pharmacy <strong>in</strong>spects every pharmacy facility <strong>in</strong> the state annually. There<br />

are <strong>on</strong>ly a handful of other states that do that. For example, some state Boards may <strong>on</strong>ly employ a<br />

s<strong>in</strong>gle pharmac<strong>is</strong>t; the <strong>Arkansas</strong> State Board has three pharmac<strong>is</strong>ts do<strong>in</strong>g rout<strong>in</strong>e <strong>in</strong>specti<strong>on</strong>s.<br />

The <strong>Arkansas</strong> State Board of Pharmacy has shared its guidel<strong>in</strong>es for <strong>in</strong>spect<strong>in</strong>g sterile compound<strong>in</strong>g<br />

pharmacies with other states.<br />

At the University of <strong>Arkansas</strong> for Medical Sciences College of Pharmacy, first-year pharmacy<br />

students get a more thorough educati<strong>on</strong> <strong>on</strong> compound<strong>in</strong>g than at most other state colleges of<br />

pharmacy.<br />
