activation, <strong>and</strong> adhesion to the endothelium, followed by<br />

extravasation <strong>of</strong> <strong>in</strong>flammatory cells. Once the <strong>in</strong>flammatory<br />

cells have <strong>in</strong>filtrated <strong>in</strong>to the tissue, they respond to<br />

chemotactic gradients established by chemoattractant<br />

cytok<strong>in</strong>es <strong>and</strong> chemok<strong>in</strong>es, which emanate from sites <strong>of</strong><br />

<strong>in</strong>jury or <strong>in</strong>fection. These molecules play a central role <strong>in</strong><br />

def<strong>in</strong><strong>in</strong>g the nature <strong>of</strong> the <strong>in</strong>flammatory <strong>in</strong>filtrate <strong>in</strong> AD.<br />

The onset <strong>of</strong> acute AD is strongly associated with the<br />

production <strong>of</strong> TH2-produced cytok<strong>in</strong>es, notably IL-4 <strong>and</strong><br />

IL-13, levels <strong>of</strong> which are significantly higher <strong>in</strong> AD<br />

<strong>in</strong>dividuals compared with control subjects (28). Mediat<strong>in</strong>g<br />

isotype switch<strong>in</strong>g to IgE synthesis <strong>and</strong> upregulat<strong>in</strong>g<br />

expression <strong>of</strong> adhesion molecules on endothelial cells, IL-<br />

4 <strong>and</strong> IL-13 are implicated <strong>in</strong> the <strong>in</strong>itial phase <strong>of</strong> tissue<br />

<strong>in</strong>flammation, whereas the T H2 cytok<strong>in</strong>e IL-5, which is<br />

<strong>in</strong>volved <strong>in</strong> eos<strong>in</strong>ophil development <strong>and</strong> survival, predom<strong>in</strong>ates<br />

<strong>in</strong> the chronic form <strong>of</strong> the disease (28).<br />

Increased production <strong>of</strong> GM-CSF <strong>in</strong> patients with AD<br />

is reported to <strong>in</strong>hibit apoptosis <strong>of</strong> monocytes, thereby<br />

contribut<strong>in</strong>g to the chronicity <strong>of</strong> this condition (29). The<br />

ma<strong>in</strong>tenance <strong>of</strong> chronic AD also <strong>in</strong>volves production <strong>of</strong><br />

the TH1-like cytok<strong>in</strong>es IL-12 <strong>and</strong> IL-18, as well as several<br />

remodel<strong>in</strong>g-associated cytok<strong>in</strong>es, such as IL-11 <strong>and</strong> TGFb1,<br />

which are expressed preferentially <strong>in</strong> chronic forms <strong>of</strong><br />

the disease (30).<br />

Increased expression <strong>of</strong> C-C chemok<strong>in</strong>es (monocyte<br />

chemoattractant prote<strong>in</strong> 4, eotax<strong>in</strong>, <strong>and</strong> RANTES) contributes<br />

to <strong>in</strong>filtration <strong>of</strong> macrophages, eos<strong>in</strong>ophils, <strong>and</strong> T<br />

cells <strong>in</strong>to acute <strong>and</strong> chronic AD sk<strong>in</strong> lesions (31). Cutaneous<br />

T cell–attract<strong>in</strong>g chemok<strong>in</strong>e (CCL27) is highly upregulated<br />

<strong>in</strong> AD <strong>and</strong> preferentially attracts cutaneous<br />

lymphocyte antigen–positive T cells <strong>in</strong>to the sk<strong>in</strong>. F<strong>in</strong>ally,<br />

selective recruitment <strong>of</strong> CCR4-express<strong>in</strong>g T H2 cells is<br />

mediated by macrophage-derived chemok<strong>in</strong>e <strong>and</strong> thymus<br />

<strong>and</strong> activation-regulated cytok<strong>in</strong>e, levels <strong>of</strong> which are<br />

<strong>in</strong>creased <strong>in</strong> patients with AD (27). Severity <strong>of</strong> AD has been<br />

l<strong>in</strong>ked to magnitude <strong>of</strong> thymus <strong>and</strong> activation-regulated<br />

cytok<strong>in</strong>e levels (32). In addition, chemok<strong>in</strong>es, such as<br />

fractalk<strong>in</strong>e (33), IFN-c–<strong>in</strong>ducible prote<strong>in</strong> 10, monok<strong>in</strong>e<br />

<strong>in</strong>duced by IFN-c, <strong>and</strong> IFN-c–<strong>in</strong>ducible a chemoattractant,<br />

are strongly upregulated <strong>in</strong> kerat<strong>in</strong>ocytes (34) <strong>and</strong><br />

contribute to TH1 cell migration toward the epidermis.<br />

IgE <strong>and</strong> IgE receptors<br />

In about 80% <strong>of</strong> adult patients with AD, the disease is<br />

associated with <strong>in</strong>creased serum IgE levels (>150 kU/L),<br />

sensitization aga<strong>in</strong>st aeroallergens <strong>and</strong> food allergens,<br />

<strong>and</strong>/or concomitant allergic rh<strong>in</strong>itis <strong>and</strong> asthma (35, 36).<br />

In contrast, 20% <strong>of</strong> adult patients with AD have normal<br />

serum IgE levels. This subtype <strong>of</strong> AD <strong>of</strong>ten has a late<br />

onset (>20 years <strong>of</strong> life) <strong>and</strong> a lack <strong>of</strong> IgE sensitization<br />

aga<strong>in</strong>st <strong>in</strong>halant or food allergens (35, 36). However,<br />

some <strong>of</strong> these patients might have IgE sensitization<br />

aga<strong>in</strong>st microbial antigens, such as Staphylococcus aureus<br />

enterotox<strong>in</strong>s <strong>and</strong> C<strong>and</strong>ida albicans or Malassezia sympo-<br />
