Dear Customer,
Thank you for choosing Canton Chair Rental to provide a canopy for your upcoming event.
Canton Chair Rental is committed to the safety of our customers, your property and your guests.
With the increase in utilities being buried underground, the risk for personal injury and property
damage becomes a concern with canopy installations. Ohio Law (3718.25-32 of the Ohio
Revised Code) requires that anyone penetrating the surface of the earth by more than 12 inches
must have the utilities marked on their property before they “dig.” Canton Chair Rental is
providing you with the information you need to abide by this law.
You must call the Ohio Utilities Protection Service (OUPS) and the Oil and Gas Producers
Underground Protection Service (OGPUPS) to make arrangements for the marking of all
underground utility lines. You will find the contact information for both organizations on the
attached waiver. In order to properly mark utilities, paint or flags may be placed on your
property.
Please read the enclosed waiver carefully and return the signed waiver to our office as soon
as possible. We will be happy to answer any questions you may have.

CANOPY INSTALLATION WAIVER- CUSTOMER INSTALL
MARKING PUBLIC UTILITIES:
For your safety and for the safety of your property and your guests, please have all underground
utilities marked.
The Ohio Utilities Protection Service (OUPS) will arrange to have any public underground utilities
marked on your property. Please call them at 1-800-362-2764 up to 4 days prior to the installation date,
but no later than 48 hours prior. Similarly, the Ohio Oil & Gas Producers Utilities Protection Service
(OGPUPS) should be called. They can be reached at 1-800-925-0988. You will need to know your
county, township, and township section number for your event location. Both services are free.
Please be aware that someone will be on the property to mark the utilities and that any pets should be
restrained and fenced areas will need to be made available.
It is also very important to look above the space you intend to install the canopy for any above ground
utility lines, tree limbs, or other obstructions!
MARKING PRIVATE UTILITIES:
Remember that there may be private utilities buried on your property also. OUPS and OGPUPS do NOT
mark private utilities. You, the customer, are responsible for making sure any privately installed utilities
are not struck by stakes. These include, but not limited to, sprinkler systems, electrical lighting wires, gas
lines (i.e. propane), artesian wells, underground structures, septic systems, down spouts, leech beds,
phone lines, cable lines, pool equipment, invisible fencing, etc.
Your signature accepting your responsibilities outlined above is required below before we can rent the
canopy to you. We will be happy to answer any questions you may have.

I have been informed of my responsibility of contacting the Ohio Utilities Protection Service and the Ohio
Oil & Gas Producers Utilities Protection Service and I understand and accept the responsibility for any
above or below ground damage that may occur including, but not limited to, underground utilities,
sprinkler systems, electrical lighting wires, artesian wells, propane lines, septic systems, down spouts,
leech beds, phone lines, cable, pool equipment, invisible fencing, overhead wires, trees/vegetation, etc.
