The Last Chance Show, Region 18 Championship Show and Eastern Canadian Breeders 

Championship Show are sanctioned as BRONZE competition members of Equine Canada, 2685

offered herein is covered by the rules and specifications of the current rules of EC and will be judged 

in accordance with the EC Rule Book. 

EQUINE CANADA STATEMENT OF PRINCIPLE 

Equine Canada (EC), the national equestrian federation of Canada, supports adherence to humane treatment of horses in all 

competitions under its jurisdiction. 

EC is committed to 

Upholding the welfare of horses, regardless of value, as a primary consideration in all activities; 

Requiring that horses be treated with kindness, respect, and the compassion that they deserve, and that they never be 

subjected to mistreatment; 

Ensuring that owners, trainers and exhibitors or their agents use responsible care in the handling, treatment and 

transportation of their horses as well as horses placed in their care for any purpose; 

Providing for the continuous well-being of horses by encouraging routine inspection and consultation with health care 

professionals and competition officials to achieve the highest possible standards of nutrition, health, comfort, sanitation 

and safety as a matter of standard operating procedure; 

Continuing to support scientific studies on equine health and welfare; 

Increasing education in training and horsemanship practices; 

Requiring owners, trainers and exhibitors to know and follow their sanctioning organization's rules, and to work within 

industry regulations in all equestrian competition; 

Reviewing, revising and developing competition rules and regulations that protect the welfare of horses. 

The standard by which conduct or treatment will be measured is that which a reasonable person, informed and experienced in 

generally accepted equine training and exhibition procedures, would determine to be neither cruel, abusive nor inhumane.

The cost to the exhibitor is $30.00 USD per class added plus the applicable entry fees.

SHOW INFORMATION 

SHOW OFFICE For move-in, the office will be open Sunday, July 

15 4:00 p.m. to 7:00 p.m. Hours for remaining days will be posted 

at the office, but will be at least one half hour prior to the start of 

classes, to the end of the last class each day. 

Exhibitors are 

requested to report to the show office as soon as possible after 

arrival, to pick up show packets, correct any pending items or make 

entries. It is the responsibility of the exhibitor to review and 

confirm their entry paperwork before leaving the show office, 

as any class changes made after that point will be considered 

post entries. 

LOCATION Western Fair District is located north of York Street, 

south of Dundas Street, and West of Highbury Avenue, in London 

Ontario. Highway 401 travelers can exit at Highbury Avenue, go 

North to Florence and turn left, one major block and turn left on 

Egerton Street and turn right into the Western Fair District facility. 

Travel across the back of the facility to the Agriplex. 

ENTRIES are to be received by June 22, 2012 for all horses 

qualified at this time. Entries that qualify for the first time between 

June 22 and July 2 may enter at regular class fees but MUST BE 

RECEIVED BY July 6 th . There will be a DESIGNATED AREA at the 

MSU Summer Showcase for you to deposit your COMPLETED 

entry forms WITH PAYMENT and proper paperwork attached, for 

those which qualify at the MSU Show. PLEASE NOTE that entries 

received after June 22 will be charged post entry fees-see Post 

Entries below. Entries must be submitted on the Official Entry Form 

for the show, which may be duplicated. Entries will not be 

processed without a cheque, and open cheques will be processed 

in the proper amount and deposited when received. CHEQUES 

COVERING MORE THAN ONE ENTRY need to be accompanied 

by a detailed description of how the money is to be applied. 

SINGLE ITEM FEES (SUCH AS STALLS, SPONSORSHIPS, etc.) 

CANNOT BE SPLIT 

All entries must be signed by the (1) owner, (2) rider, driver and/or 

handler, (3) the trainer or by the agent(s) of such person(s). In the 

case of an owner, rider, driver or handler under 18, his/her parent 

or guardian, or if not available, the trainer, must also sign an entry 

blank on the minor’s behalf. If any owner, rider, driver, handler or 

trainer permits his/her agent to sign the entry on his/her behalf, the 

owner, rider, driver, handler or trainer understands and agrees that 

he/she is bound by all of the terms/conditions of the entry form. If 

any of the above fails to sign the entry form, his/her first entry into 

the ring as an owner, rider, driver handler or trainer shall be 

construed as his/her acceptance of the rules of the competition and 

of AHA and shall ipso facto render him/her subject to said rules. 

All entries received, shall constitute an agreement with and are 

bound by the rules of the show, EC/USEF and AHA, and all fees 

and stabling costs are payable. Exhibitors who wish to reserve 

stalls must include payment in full. Farm/corporation names will 

be used ONLY if the farm/corporation is on the horse's registration 

papers, AND the farm/corporation has been registered with 

EC/USEF AND AHA, and those membership numbers noted on the 

entry form. A photocopy of both sides of the Certificate of 

Registration of each horse entered MUST be submitted with the 

entry forms. If the name shown on the registration paper is 

different from the person entering the horse, an affidavit, a copy of 

the sales contract or authorization form must be filed with the Show 

Office. 

All owners, trainers, riders, drivers and handlers MUST PROVIDE 

COPIES of their current AHA competition membership cards or 

purchase new memberships in order to compete at the Regional 

show. Single Event memberships will be accepted only for the Last 

Chance Show. Exhibitors in the Lead Line classes are exempt from 

membership requirements. 

NOTICE: Each competitor is responsible for supplying 

management with correct association numbers (USEF, EC, AHA) 

for horse, owner, rider and trainer, before the day(s) of the 

competition. The scores of competitors who fail to supply correct 

numbers may not be counted toward National awards. 

BACK NUMBERS WILL NOT BE RELEASED UNTIL ALL 

REQUIRED INFORMATION IS PROVIDED TO THE SHOW 

OFFICE AND PAYMENT FOR CLASSES HAS BEEN MADE. 

INCOMPLETE ENTRY FORMS WILL BE BILLED $15!! 

RETURNED CHEQUES There will be a $100.00 fee for returned 

cheques. 

POST ENTRIES Post entries will be charged additional fees as 

listed in the schedule for each class entered after the appropriate 

closing date/time. 

For Regional Classes, ANY ENTRY MADE AFTER 7:00 

PM Wednesday July 18 may be charged POST 

ENTRY FEES PLUS AN ADDITIONAL $200.

Please note that “changing” a class is in fact the scratch of a 

class followed by a class add and will be subject to appropriate 

fees. 

REGIONAL QUALIFICATION Any eligible horse may show in 

any regional class providing they have accumulated the number 

of points or placings required at AHA Recognized shows, or 

received the required number of qualifying scores at EC open 

dressage shows approved by AHA for Region 18, from January 

2011 to start of 2012 Regional show. For further details regarding 

qualifications, please refer to the new qualification schedule as 

set out in the AHA handbook . Qualifications must be provided 

with entries and will be verified. Also, please note that per AHA 

Article COMP 1351.5 “Top Tens from any show and Regional 

Top Five, Champion or Reserve Champion do not qualify a horse 

for a Regional Championship class. 4. Only points/scores from 

shows other than Regionals or Nationals will count for Regional 

Shows.)” 

AGE OF INDIVIDUALS Juniors age will be determined as of 

December 1 for the purposes of AHA youth classes. All adults 

are determined as of January 1 in accordance with EC rules. 

AHA MEMBERSHIP IS REQUIRED for Regional 

Championships; this includes walk-trot/jog exhibitors. You 

MUST HAVE a copy of your current AHA Membership Card. 

AHA SINGLE EVENT FEE is only an option for the Last Chance 

Arabian Show 

AMATEUR STATUS Every person who has reached his/her 18 th 

birthday and competes in classes for amateurs under EC rules 

must possess current amateur certification issued by Equine 

Canada or equivalent USEF certification if USEF member. 

RECIPROCITY EC/USEF membership status is recognized by 

each of the U.S. and Canada under an agreement between the 

Federations. This agreement pertains to rule infractions. EC 

rules take precedence. EC members MUST be members of their 

Provincial equestrian governing body and show proof of 

insurance coverage (i.e. OEF membership or EC insurance from 

NPP’s). 

REGISTRATION REQUIREMENTS Horses shown at these 

shows must have been issued a certificate of registration from 

the Canadian Horse Registry and/or the Arabian Horse Registry 

of America or if under one year of age be eligible for registration 

and registration applied for, and must be entered in their full 

registered name. Only weanlings that have had registration 

applied for may show without registration papers. A copy of the 

registration papers showing proof of ownership must be 

submitted with the entry form at the time of making entries. In the

case of a sale or lease, it is required that a contract of the sale or 

lease, or statement from the owner be submitted with the copy of 

the registration papers. 

Horses shown in the Half/Anglo Arabian Division must have been 

issued a Registration certificate from CPAR or AHA or be eligible 

for registration if under one year of age. Half-Arabian colts/stallions 

two years and older may not compete in Half-Arabian/Anglo 

Arabian breeding line classes. 

HEALTH PAPERS Current health certificates are required for all 

horses coming from the United States. Please check with your 

Veterinarian regarding border crossing regulations. A copy of the 

Negative Coggins test within 6 months is required for EACH 

horse, and should be sent with entries. 

CROSS ENTRIES Cross-entries are prohibited between the 

Country English Pleasure sections and the English Pleasure 

sections at the same show. 

REFUNDS Refunds on fees will be made upon request with a 

written statement from your Veterinarian on said Veterinarian's 

letterhead, if received by June 22, 2012. NO REFUNDS on stalls 

after June 22, 2012. Office fees are non-refundable. 75% of class 

entry fees will be refunded on receipt of a Veterinarian certificate 

following the close of entries, but prior to your classes. Exhibitors 

must notify the Show Secretary of cancellations prior to the time the 

class is called. 

STABLE AREA 

BARN OFFICE Personnel will be available commencing 12:00 

NOON Sunday, July 15 and throughout the show. Following movein, 

hours for shavings and hay purchase/delivery will be posted. 

ARRIVAL AND DEPARTURE TIMES Stalls are available NOON 

Sunday, July 15, until 10:00PM Saturday, July 21. ACCESS to the 

grounds will NOT be available until NOON Sunday July 15. 

Grounds must be vacated by 10:00 PM Saturday July 21. 

STABLING Stabling is limited and will not be allocated unless stalls 

have been PAID FOR and STABLING FORMS submitted to the 

Show Secretary. Stalls are assigned as stall fees are paid. Early 

entry is an advantage for stabling assignment. Preferred Stabling 

will be given to Gold Sponsors. Exhibitors wishing to stable 

together must send their entries in the same envelope. NO 

REFUNDS on stalls after the closing date, June 22, 2012. Each 

exhibitor is responsible for the cleanliness of his/her aisle. 

Shavings must be used, No straw permitted. HORSES MUST BE 

BATHED IN DESIGNATED WASH STALL AREAS. 

SHAVINGS AND HAY Will be available at current market price. 

MANURE DISPOSAL Manure disposal is provided for in 

designated areas. 

GENERAL 

ADMISSION TO ARENA No person will be permitted to enter the 

arena except class participants, judges and their assistants, show 

photographers and show management personnel. Attendants will 

not be permitted in arena until summoned by order of the judge. 

ASTM/SEI HEADGEAR Properly fitting protective headgear 

meeting ASTM/SEI standards must be worn, with harness secured, 

by the following: 1)All juniors riding in Hunt Seat Equitation 

sections; 2)All juniors riding on competition grounds, unless 

exempted for the class in which they are competing at that time; 

3)All dressage exhibitors warming up for or exhibiting in dressage 

classes. All juniors must also wear footwear with appropriate heel 

at all times when riding. 

CALL FOR CLASSES The two-minute rule will be STRICTLY 

ENFORCED. Only when a horse or rider/diver/handler has back to 

back classes will a 3 minute tack hold be granted if requested 

before the end of the preceding session. All tack changes must be 

made at the entrance gate to the arena. The PA system and 

paddock calls are a courtesy but it is the responsibility of the 

exhibitor to be sure that his/her horse is at the gate at the 

prescribed time. 

CAMPERS There are 50 hook-ups available on the grounds with 

15 amp service and water connection. First paid, first reserved. 

Verification tickets will be provided when entries are picked up, 

and must be visibly displayed. CAMPERS MUST NOT BE 

PLUGGED INTO ANY OF THE BARN HYDRO OUTLETS OR 

TO ANOTHER CAMPER HOOKED UP TO HYDRO. NO 

CAMPING IN TENTS PERMITTED FOR SAFETY REASONS. 

CONDUCT Exhibitors are hereby notified that any act of 

discourtesy or disobedience to Judges, Officials and/or Show 

Staff on the part of the owner, trainer, rider or any other 

attendant, shall be grounds to disqualify the horse, and all entry 

fees and other charges and prize money won shall be forfeited. 

The Show Committee also bears the responsibility to aid the 

judges in avoiding contact with exhibitors or their agents before 

and during the show. 

Horses shall not be punished unnecessarily or severely. No 

excessive whipping will be tolerated. Horses must be treated 

with respect at all times. 

DIVISION/COMBINING OF CLASSES Classes may be 

split/combined at the discretion of show management. 

Management reserves the right to split any class for safety 

purposes. 

DOGS All dogs must be on leashes at all times and not allowed 

in the show arena complex. There will be a $100.00 fine to the 

owner of a dog found loose on the property for the first offense, 

after which removal of the dog from the premises will result. 

DRESSAGE All Regional ride times will be posted at the show 

office, and dressage presentations will be held in the Arena, once 

all dressage rides have been completed. The Champion and 

Reserve Champion horse should be present for the awards; Top 

5’s are encouraged to be there as well.**NEW- EC Headgear 

Policy- see ”ASTM/SEI Headgear” above and EC Rulebook for 

complete regulations. 

DRUG TESTING Any horse entered in any class at a competition 

subject to the requirement to pay drug testing fees may, while on 

the competition grounds and on the recommendation of the chair 

of the provincial Equine Medication Control Committee or the 

chair’s appointee, be selected for equine medication control 

testing. 

FOOTING in the arena is a sand/dirt mix and in the outside 

exercise areas is sand mix. 

GOLF CARTS All operators must be 16 years of age or older 

and all Golf Carts shall be operated in a safe and prudent 

manner at all times. Any violation of this rule, in the sole 

discretion of Show Management, shall result in the forfeiture of 

the Golf Cart to the rental facility without reimbursement and may 

result in further penalties. Operation of any Golf Cart on the show 

grounds constitutes agreement to be bound by these provisions. 

IDENTIFICATION NUMBERS Every horse will be assigned an 

identification number, and this number must be worn by 

exhibitors when the horse is being shown. Any exhibitor using a 

wrong number or having no number may be excused or 

disqualified at the Judge’s discretion. 

JUDGES In the event of illness or failure of a Judge to attend this 

show or perform his duties, the Show Committee reserves the 

right to substitute a judge. No contact shall be made with the 

Judges before or during the period of the Show unless 

accompanied by an EC Steward. Any offensive or abusive 

contact with the Judge by an Exhibitor will be cause for 

immediate removal from the show. Further, no exhibitor shall 

approach a Judge regarding a decision unless he first obtains 

permission from the Show committee or Show Steward, who 

shall arrange an appointment with the Judge if so granted. No 

exhibitor has the right to inspect the Judge's cards without the 

Judge's permission.

LUNGING NO LUNGING IN SHOW RING AT ANY TIME. Lunging 

permitted in outside exercise areas ONLY, and will not be permitted 

in any warm up area where horses are being ridden/driven. 

PADDOCK INSPECTION PROCEDURE In all Regional Show 

Breeding and In-Hand Classes, horses will enter in an assigned 

order and be inspected for signs of abuse immediately prior to 

entrance into the ring for judging. The Order of Go shall be 

determined and posted by the birth date, oldest to youngest. The 

manner of inspection will be at the discretion of the Show 

Management. Any horse that does not enter the ring in his 

assigned order may be eliminated at the discretion of the Show 

Management. Judging will be done in accordance with AHA 

Resolution 11/06 using the “in the ring” procedure”. For more 

information on this system, please visit the AHA website. 

PARKING There are designated parking areas for cars and horse 

transports. Please insure that you do not leave vehicles adjacent to 

barn areas where they will interfere with animals or other exhibitors' 

access. 

P.A. SYSTEM Due to possible unforeseen problems with any 

mechanical device, the P.A. system and class calls over that 

system are considered a courtesy. The exhibitor is responsible for 

having his horse at the gate at the prescribed time. 

PROTESTS Protests may be made to the show committee by an 

exhibitor, owner, agent, trainer or the parent of a junior exhibitor for 

any violation of EC rules. It must be in writing, signed by the 

protester, addressed to the secretary of the competition at which 

time the alleged violation occurred; accompanied by a deposit of 

$100.00. It must be received by the Steward, competition Manager 

or a member of the Show Committee within 48 hours of the alleged 

violation. 

RESPONSIBILITY Region 18 Last Chance Arabian Horse Show, 

AHA Region 18 and/or the owners of the show facility will not be 

responsible for any article of any kind or nature that may be lost or 

destroyed or in any way damaged. Each exhibitor will be 

responsible for any injury that may be occasioned to any person or 

any animal, or damage to any property while on the show grounds 

by any horse owned or exhibited by him/her, and shall indemnify 

the Management against all legally established claims or 

damages of any kind or nature that may grow out of any injury 

occasioned by any horse owned or exhibited by him/her. 

PRESENTATION OF THE SIGNED ENTRY FORM SHALL BE 

DEEMED AN ACCEPTANCE OF THESE RULES, AND ALL 

RULES OF EC AND AHA. 

SCHEDULE Show Management reserves the right to change 

and modify the schedule of events as necessary to 

accommodate the entries received. Any class where numbers of 

entries will cause overcrowding in the arena will be split upon the 

decision of Show Management; such splits to be announced. 

SHOEING REGULATIONS See current EC rule book for 

shoeing requirements – Arabians 4 1/2" toe; H/A Arabians 5" toe. 

Only clear material may be used on Breeding/In-Hand horses 

being exhibited. We reserve the right to check any horse for 

compliance with shoeing rules.. 

SMOKING RESTRICTIONS/FIRE REGULATIONS Smoking is 

prohibited in the barns, in the show arena, in the paddock areas 

or where otherwise posted. No open flames in stabling areas. 

No pressurized propane tanks allowed in stabling areas. No 

excessive accumulation of litter or rubbish. All entrances, aisles, 

stairways, etc. must be free and clear of obstructions. All stall 

curtains to be of flame-retardant material. 

TACK HOLDS Only when a horse or rider/driver/handler has 

back to back classes will a 3 minute tack hold be granted if 

requested before the end of the preceding session. 

TOP 5 AWARDS Top 5 Awards will be made regardless of the 

number of entries in a class. Only awards made to the top 50% 

rounded up, of the class will receive IA Breeders Sweepstakes 

payout, AHA Horse Achievement Award, High Point Horse 

Achievement Award, High Point Amateur Achievement Award 

and Distance Award Points, and ARRAB programme payout.

The ARRAB program was designed as an incentive for Canadian Amateur Riders and Owners. The 

nomination fees are pooled with sponsorships and fundraising proceeds, and this creates funds for 

payback to the Top 5 winners. Horses do not need to be entered in the ARRAB program to show in the 

classes, but do need to be nominated PRIOR to JULY 1 st 2012. Nomination fee per horse is $50, 

non-refundable and in addition to the class entry fees. Exhibitors are responsible for entering their 

class(es) at the Regional Championship Show. For a list of the Royal Red Classes offered this year, 

please refer to the end of “Classes Listed by Section”

DIRECTIONS TO WESTERN FAIR DISTRICT 

From Hwy 401 (Westbound from Toronto/Kitchener) Exit at Highbury Avenue. Travel North approximately 12 

minutes to Florence Street, turn left. Stay on Florence for about 10 minutes; at Egerton, turn left. Turn right into 

the fairgrounds and proceed straight ahead to the back of the Agriplex. 

From Hwy 401 (Eastbound from Sarnia (Hwy 402), Windsor) Exit at Wellington Street. Travel North 

approximately 15 minutes to the downtown core. Go through the overpass, turn right at the lights onto York 

Street. Continue on York Street for about 8 minutes, past the Agriplex & the Sportsplex and just before the next 

lights, veer right onto Egerton Street. Turn right into the fairgrounds and proceed straight ahead to the back of 

the Agriplex where the Barn Manager can assist you further. 

The Show Office is located at the front of the Agriplex.

Equine Canada NOTICE: Every entry at a recognized Show shall constitute an agreement that the person making it, owner, lessee, minor, manager, agent, coach,

I AGREE that I choose to participate voluntarily in this Competition, as a rider, driver, handler, lessee, owner, agent, coach, trainer, junior exhibitor, or as a 

parent or guardian of a junior exhibitor. I AM FULLY AWARE AND ACKNOWLEDGE THAT HORSE SPORTS AND PARTICIPATION IN THIS COMPETITION 

INVOLVE SERIOUS RISK OF HARM INCLUDING, BUT NOT LIMITED TO, RISKS OF ACCIDENT, SERIOUS BODILY INJURY, INCLUDING DEATH,

in the premises. 

I AGREE to indemnify and hold harmless (that is pay all losses, damages, attorneys fees and costs of) the Released Parties from and against any and all 

claims, demands, penalties, actions, losses, costs, damages, injuries, liabilities and obligations (including attorneys fees) of whatsoever kind and nature, which 

may be asserted against or incurred by any of them as a result of (1) my participation in the Competition or (2) any act, failure to act, or neglect (a) by me, my 

agents, employees, riders, handlers, trainers, coaches, drivers, contractors or invitees, or (b) by any animal owned or exhibited by me or in my custody or control. 

I AGREE and represent that I am qualified and eligible to enter and/or participate in the Competition, and every horse I am entering is qualified and eligible as 

entered. 

I AGREE to accept AS FINAL any decision of AHA, the Show Commission or Show Officials concerning my qualification or the qualification of my horse to enter 

the Competition or any results of the Competition, except to the extent that the Rules of AHA, the Competition, EC or USA Equestrian permit a protest or hearing 

of such decisions. Should a hearing be requested, I agree to accept AS FINAL the decision of the particular hearing body. I agree to release, hold harmless and 

not to sue AHA, the Competition Sponsor, their officers, directors, employees, volunteers or members concerning any decision of AHA, the Competition, its Show 

Commission, Show Officials or any hearing body that relates to my qualifications or my horse(s)’ qualifications to enter the Competition or any results of the 

Competition. 

I AGREE that AHA has the sole right to control, sell, supervise or give away (or assign to others the right to do so) the exclusive rights to broadcast, televise, 

reproduce, transmit and disseminate all or part of this event, and I agree that AHA may use or assign, in any way AHA sees fit, photographs, films, videos, audios, 

cablecasts, or other likenesses of me and my horse taken during the course of the Competition for the promotion, coverage or benefit of the Competition or AHA. 

Those likenesses shall not be used to advertise a product and they may not be used in such a way which implies endorsement of any company, product, product 

category or service. I hereby expressly and irrevocably waive and release any rights in connection with such use, including any claim to compensation, invasion of 

privacy, right of publicity, or to misappropriation. 

By signing below as a parent or guardian of a junior exhibitor, I consent to the child’s participation and agree to all of the above provisions, and further agree to 

assume all of the obligations of this AHA Assumption of Risk, Release and Indemnification personally and on behalf of the child. In the event that my child 

participates in an Equine Canada sanctioned competition where approved headgear is required for juniors, he/she will wear a properly fitted ASTM/SEI or BSI 

approved helmet. It is understood that juniors not meeting this requirement will not be allowed to compete at these competitions. 

This Assumption of Risk, Release and Indemnification is governed by the Laws of the Province of Ontario and is intended to be interpreted as broadly as 

possible in favour of the Released Parties. I agree that exclusive jurisdiction and venue (place) for any legal action against any of the Released Parties shall be in 

the Courts of the Province of Ontario. If any part of this agreement is determined to be unenforceable, all other parts shall remain effect.

PART I: ENTRY REQUIREMENTS 

A. To be eligible to compete in the Canadian Arabian Breeders Championship classes a horse must: 

1. BE REGISTERED with the Canadian Arabian Horse Registry or the Canadian Partbred Arabian Registry or the appropriate 

WAHO approved registry.. 

2. BE CANADIAN BRED, as recorded by the certificate of registration. 

3. All Canadian owned horses must be registered with Canadian Arabian Horse Registry or Canadian Partbred Arabian 

Registry. unless the horse is currently standing outside of Canada for breeding purposes or the horse permanently* resides 

outside of Canada. Horses that do not reside in Canada must have a valid registration certificate from the WAHO 

approved registry in the country in which they reside. *WAHO rules require any horse that resides in a country for longer 

than 6 months at a time must be registered with that country’s registry. 

4. Be properly entered in the Canadian Arabian Breeders Championship class prior to the dates for closing of entries and post 

entries and have attached a photocopy of the Canadian Arabian Horse Registry or the Canadian Partbred Arabian Registry 

or active Arabian Horse Association (Canadian Bred/US owned) registration certificate (FRONT AND BACK). 

PLEASE NOTE ENTRIES CANNOT BE ACCEPTED WITHOUT AN UP-TO-DATE COPY OF THE 

REGISTRATION CERTIFICATE. A COPY OF EACH REGISTRATION CERTIFICATE MUST BE FORWARDED
