For an Indoor Facility Rental Form or Permit for Use <strong>of</strong><br />
Outdoor Facilities and a copy <strong>of</strong> regulations, visit the<br />
Recreation Department link on the <strong>Village</strong> website at<br />
www.palospark.org or the Recreation Center. Rentals<br />
are accepted upon availability <strong>of</strong> the date and<br />
approval <strong>of</strong> the Recreation Director. The facility will<br />
not be available until time stated on the rental<br />
agreement. If time is needed for decorating, it must<br />
be included in the requested time.<br />
Cancellation Policy:<br />
The Recreation Department must be contacted within<br />
48 hours <strong>of</strong> the scheduled event for cancellation <strong>of</strong> the<br />
rental request. The <strong>Village</strong> <strong>of</strong> <strong>Palos</strong> <strong>Park</strong> reserves the<br />
right to cancel the scheduled event in cases <strong>of</strong><br />
emergency or hazardous situations.<br />
Security Deposit:<br />
A $150 refundable security deposit in cash or check<br />
(no credit card) is required with the rental request to<br />
reserve the date.<br />

Registration:<br />
Registration may be made in-person, by mail, fax, or<br />
online. No registrations are taken by phone.<br />
Payments may be made by cash, check, credit card,<br />
or from account. There will be a $35 processing fee<br />
assessed for checks returned for insufficient funds.<br />
Registration Policies:<br />
Anyone residing outside the boundaries <strong>of</strong> the <strong>Village</strong><br />
<strong>of</strong> <strong>Palos</strong> <strong>Park</strong> (in unincorporated <strong>Palos</strong> <strong>Park</strong> or in<br />
neighboring communities) must pay the non-resident<br />
fee for activities. If you do not pay taxes to the<br />
<strong>Village</strong> <strong>of</strong> <strong>Palos</strong> <strong>Park</strong>, you are a non-resident.<br />
The Recreation Department <strong>of</strong> the <strong>Village</strong> <strong>of</strong> <strong>Palos</strong><br />
<strong>Park</strong> assumes no responsibility for personal injuries or<br />
loss <strong>of</strong> personal property while attending a Recreation<br />
Department sponsored activity.<br />
Minimum and Maximum:<br />
All classes have a minimum and maximum number <strong>of</strong><br />
participants. Programs may be cancelled due to low<br />
enrollment. A waitlist is formed once the registration<br />
has reached the maximum. Participants will be<br />
removed from the waitlist and enrolled if space<br />
becomes available.<br />
The Recreation Department reserves the right to<br />
change instructors listed for the programs to benefit<br />
the best interest <strong>of</strong> the participants.<br />
Refunds:<br />
There will be a processing fee <strong>of</strong> $5.00 on all refunds<br />
except when the Recreation Department cancels the<br />
program, and then a full refund will be given. Any<br />
other refund given is less class time elapsed. There<br />
will be NO REFUNDS issued after the first class, no<br />
exceptions. Refunds are issued by check within 3-4<br />
weeks or may be held on an account with the<br />
Recreation Department.<br />
Special Events: Participants who withdraw from a<br />
special event at least 10 working days prior to the<br />
event will receive a refund.<br />
Senior Citizen Discount:<br />
Senior Citizens age 65 and older may receive a 10%<br />
discount on any adult activity excluding trips, special<br />
events, and Senior Wellness class.<br />
Photographs:<br />
Participating in our various programs, you may be<br />
photographed which may be used in a variety <strong>of</strong><br />
advertising materials. By registering for our<br />
programs, participants consent to the use <strong>of</strong> these<br />
pictures.<br />
Registration Information<br />
The <strong>Palos</strong> <strong>Park</strong> Recreation Department accepts online<br />
registration. The first step to registering online is to<br />
create a family account.<br />
Requesting an Online Account<br />
Go to www.activenet.active.com/palospark. Click<br />
“Request Account” or “My Account”.<br />
Fill out all required information, including your<br />
birthdate, then click “Submit”. Since this form will be<br />
used to set-up your account, it should be filled out<br />
with information for a head <strong>of</strong> household, not for a<br />
child you wish to register for a program. You will be<br />
able to add information for other family members<br />
once your account has been approved. Please create<br />
only one account per family.<br />
Be sure to remember your login and password!<br />
Click “Submit”.<br />
Your request will then be submitted to the Recreation<br />
Department. An email will be sent notifying you that<br />
the account request has been approved within two<br />
business days. Although you can always browse<br />
program information online, you will not be able to<br />
register until your account has been approved.<br />
To log in, once you have been approved, go to<br />
www.activenet.active.com/palospark and click the<br />
“My Account” button in the top right-hand corner <strong>of</strong><br />
your screen. From there, you will be able to change<br />
your personal information, add family members to<br />
your account, register for activities, view a list <strong>of</strong> your<br />
transactions or a daily schedule for your family, and<br />
more.<br />
Adding Family Members<br />
To add other family members to your account, select<br />
“Change Information about Family Members”. Fill out<br />
the form provided for each member <strong>of</strong> your family,<br />
including birthdates, if the members are children or<br />
adults, and any medical alerts <strong>of</strong> which our staff<br />
should be made aware.<br />
(An accurate birthdate is important to determine<br />
registration eligibility<br />
for age-based<br />

STAGEPLAY: MUSICAL THEATER FOR KIDS CAMP<br />
Does your child love to sing and dance and perform for others? Enjoy this<br />
intense yet entertaining program in musical theater. This camp emphasizes<br />
the journey toward producing a show from start to finish, creating a fun<br />
and non-competitive environment for kids to stretch their dramatic wings.<br />
Celebrate the "Broadway Beat" with hits from Hairspray, The Music Man,<br />
The King And I, Wicked, Rent, and Grease, and a medley <strong>of</strong> favorites from<br />
George M. Cohan. All participants will learn songs and choreography at an<br />
age-appropriate level <strong>of</strong> difficulty while learning about the history <strong>of</strong><br />
Broadway. The StagePlay program also incorporates valuable life lessons<br />
such as teamwork, commitment, conquering fear, confidence, and theater<br />
etiquette. On the last day <strong>of</strong> camp, our talented stars perform for friends<br />
