New UIIA <strong>Insurance</strong> Submission<br />

Requirements


New <strong>Insurance</strong> Submission Requirements<br />

• Effective August 1, 2012 agents submitting paper certificates to the UIIA via<br />

e-mail, fax or mail will be required to utilize the <strong>ACORD</strong> <strong>22</strong> <strong>Certificate</strong> <strong>of</strong><br />

<strong>Insurance</strong> form.<br />

• No other certificate forms will be accepted after August 1, 2012. Only<br />

exception to this will be state workers compensation insurance policies will<br />

continued to be accepted on the state fund provided certificate forms.<br />

• Use <strong>of</strong> other forms when providing insurance to the UIIA may result in delays<br />

in updating your insured’s account.<br />

• <strong>ACORD</strong> <strong>22</strong> certificate <strong>of</strong> insurance form version 03/2010 will continued to be<br />

accepted until the end <strong>of</strong> 2012.<br />

• IANA has reached out to agency management s<strong>of</strong>tware vendors<br />

encouraging them to include the revised <strong>ACORD</strong> <strong>22</strong> certificate <strong>of</strong> insurance<br />

form in their library <strong>of</strong> forms.<br />

<strong>ACORD</strong> <strong>22</strong> Changes<br />

Effective April 30, 2012, the cancellation language shown on the<br />

<strong>ACORD</strong> <strong>22</strong> <strong>Certificate</strong> <strong>of</strong> <strong>Insurance</strong> was changed to match what is<br />

currently shown on the <strong>ACORD</strong> 25 form.<br />

UIIA Required Cancellation Notice<br />

• Revision to <strong>ACORD</strong> <strong>22</strong> cancellation language does not change the<br />

advanced notice <strong>of</strong> cancellation required under the UIIA.<br />

• Required Notification is – 30 days advanced notice <strong>of</strong> cancellation is<br />

required unless cancellation is a result <strong>of</strong> non-payment <strong>of</strong> premium in which<br />

10 days notice is required.<br />

• The Motor Carrier is contractually obligated to ensure that their agent is<br />

aware <strong>of</strong> these requirements when binding coverage for them to make sure<br />

that should cancellation occur, the UIIA is provided with proper notice.<br />

Completing the <strong>ACORD</strong> <strong>22</strong> <strong>Certificate</strong> <strong>of</strong> <strong>Insurance</strong> Form<br />

• Agent Details - Agents should remember to provide their contact details on<br />

the <strong>ACORD</strong> <strong>22</strong> form: Name, phone, fax & e-mail.<br />

• <strong>Insurance</strong> Companies - The corresponding letter <strong>of</strong> the insurance company<br />

for each type <strong>of</strong> coverage must be provided.<br />

• Policy Details - Policy Limits, Deductibles, Policy Numbers, Effective date<br />

and Expiration dates for each policy listed on the certificate must be included<br />

as well.<br />

• Umbrella/Excess Policies listed on the certificate must identify what<br />

primary coverage these policies are over.<br />

Completing the <strong>ACORD</strong> <strong>22</strong> Continued -<br />

• Description <strong>of</strong> Operations – Remember to check the box to confirm that<br />

the Truckers Uniform Intermodal Interchange Endorsement is part <strong>of</strong> the<br />

auto liability policy(ies) and that the EPs your insured does business with are<br />

additional insured.<br />

• EP Checklist – If providing additional insured information, don’t forget to<br />

include the EP Checklist to identify the companies that have been endorsed<br />

to the appropriate policy as an additional insured.<br />

If an agent is using an alternative list to provide the additional insured<br />

information make sure that the appropriate asterisks (*) have been placed<br />

next to the EP’s company name to indicate what policy they are addl.<br />

insured on.<br />

• Signature - All certificates must be signed. Failure to sign the certificate will<br />

delay your insured getting updated until a signed certificate is provided.<br />

<strong>Insurance</strong> Updates<br />

• Updates to <strong>Insurance</strong> –<br />

Please remember that each time you provide insurance information to the<br />

UIIA <strong>of</strong>fice that all appropriate information must be included with the<br />

<strong>Certificate</strong> <strong>of</strong> <strong>Insurance</strong>. This is true even if the insurance policy<br />

numbers remain the same.<br />

• Additional Insureds – Check the box on the <strong>ACORD</strong> <strong>22</strong> certificate and<br />

remember to include the EP Checklist or alternate listing <strong>of</strong> companies that are<br />

listed as additional insureds on the Motor Carrier’s insurance policies. Make<br />

sure to check all EPs that your insured does business with each time you<br />

submit insurance to our <strong>of</strong>fice.<br />

• UIIE-1, CA23-17, TE23-17B or Equivalent Endorsement – Each time you<br />

provide auto liability on the <strong>Certificate</strong> <strong>of</strong> <strong>Insurance</strong> make sure to check the box<br />

on the <strong>ACORD</strong> <strong>22</strong> certification confirming that the Truckers Uniform Intermodal<br />

Interchange Endorsement (UIIE-1) is endorsed to the auto liability policy.<br />

Agents may also provide the actual endorsement.<br />

Note:<br />

Failure to provide the appropriate information each time insurance updates are<br />

submitted may result in an adverse impact to your insured’s account.<br />

Alternative <strong>Insurance</strong> Submission Method – Register for<br />

Online Submission<br />

• Agents can register to become authorized to submit insurance online to the<br />

UIIA. Registration is available from the <strong>Insurance</strong> Agency login page on the<br />

UIIA website at www.uiia.org.<br />

• Agents complete online registration and fax signed copy <strong>of</strong> registration form<br />

back to the UIIA.<br />

• UIIA confirms that both the agency and the individual agent have active<br />

licenses and activate the account. Agent will receive e-mail with account<br />

number and password to log into the UIIA system.<br />

• Agent will need to obtain their insured’s SCAC Code and <strong>Insurance</strong> Agent<br />

Code the very first time they go to submit insurance online for the company.<br />

After the initial link <strong>of</strong> the company is made to the agent’s account, each time<br />

they log into the system the insured will be shown on their list <strong>of</strong> Motor<br />

Carriers.<br />
