Page 2 ADCI Certification Protocol The following Protocols are provided for each circumstance and type of application. By following these protocols, you are facilitating a quicker processing time for you applications. For all applications, allow 3-4 weeks processing time. Diver Certification—ADCI Member Company and Member School 1. Please use the ADCI Member Company Application or Member School Application. 2. Include a passport style photo for the diver. If the diver holds an ADCI card, please include the cert number or indicate that he has a previous card. If in doubt, include a passport style photo for that diver—clearly labeled. 3. If the diver has never had an ADCI card, please include a copy of his dive school diploma. 4. Include an ID number (unique numerical identifier) for each diver. All of our forms can be downloaded at www.adc-int.org. Formal Training is now a requirement for certification. If you have never had an ADCI card before, you must present a copy of your dive school diploma with your application. 5. We are happy to invoice Member Companies and Member Schools. If your company requires a PO Number, include the number on the application or a copy of the PO with the application. If you do not want to be invoiced, be sure to include payment with the application. Diver Certification—Individual 1. If you are not applying through a member company or member school, you must use the application for individuals. 2. Refer to the Chart of Requirements and submit all required documentation for the classification of card on your application. If you have never had an ADCI card before you must provide a copy of your dive school diploma and a passport style photo. 3. If you have an ADCI card, please attach a copy, or provide the cert number on your application, or otherwise indicate that you have an ADCI card. 4. Include payment by check, money order, American Express, MasterCard, or VISA. Replacing a Lost or Stolen Card If your certification card has been lost, stolen, or otherwise misplaced—and it is NOT expired: 1. Use the application for Lost or Stolen Card. 2. If at all possible, provide your certification card number on the application. Lost your Certification Card? 3. Include payment with application—Check, Money Order, VISA MasterCard, American Express. If the Certification Card has expired, you must renew the card through a member company or using an individual application.
ADCI Certification Protocol Page 3 ADCI Supervisor or LST Certification 1. Please use the dedicated application for Supervisor or LST. 2. Non-Member Companies and Individuals must provide documentation to support your qualifications for Supervisor. If you have never had an ADCI card before, a copy of your dive school diploma must be included in your application. 3. The Proctor for your test must be your supervisor at work, a designated person at your workplace who has been authorized to Proctor, or you may arrange for a proctor with the Public Library or a local college. 4. Include payment with your application. (Member Companies can be invoiced). Check, money order, VISA, MasterCard, American Express. 5. If your application is approved, your login and password for the test will be sent to your Proctor. At this point all test information must go through your proctor. 6. When you pass your test, the Proctor will notify our office and your card will be made. How ADCI Processes Your Application When the ADCI office receives your application, we check for all components: correct application used, all information provided, photo, payment, documentation. We verify whether you have had an ADCI card, the category and expiration. Next, the application goes to the Executive Director for review and approval. If there are questions or deficiencies, the application is held for more information or it is rejected and notice is sent. If approved, the application is sent to Accounting. Accounting will then create an invoice or register payment according to the type of application. If paying by credit card, the charge will be made. Personal checks must clear the bank before the application is released for processing. If the credit card is declined, the application will be held until payment can be made. Please allow 3 to 4 weeks for processing your application. The ADCI Consensus Standards provides authoritative guidelines for a very high standard of safety within the diving industry. Once Payment or invoicing is complete, the application goes to processing for the card to be made and mailed; or in the case of a Supervisor or LST, processing will send login and password to the Proctor. Supervisor and LST cards are made after the test has been passed. Responsibilities of ADCI The Certification Card Program was developed to assist our Member Companies and their clients. Certification gives them the added assurance that dive operations will be conducted in the safest possible manner by well qualified personnel; thereby lowering the risk of injury to the diver and the client’s risk of liability. Safe Practices and Proper Training saves lives, keeps dive contractors in business, and keeps divers employed. ADCI takes the responsibility of vetting certification applications very seriously, and endeavors to be thorough, consistent, and fair. Proper Certification of your divers provides added confidence for your Client.
