Cigna Care Network
Your plan may include the Cigna Care Network, composed of specialists selected from Cigna’s broad network of
participating health care professionals. If Cigna Care Network is part of your plan, and you need certain types of specialty
care, you will receive a higher level of in-network benefits when you receive covered services from Cigna Care designated
doctors.
A Cigna Care designated doctor is a Cigna participating doctor who practices in one of the specialties below and meets
certain criteria related to quality and cost-efficiency. Another objective of the Cigna Care Network is to help ensure our
customers have adequate access to specialty care.

Ear/Nose/Throat (Otolaryngology)

If ‘‘Cigna Care Network’’is printed on your ID card and you need care in one of the specialties listed above, the following
apply:

When you receive covered services from specialists who are Cigna Care designated doctors, you receive the
in-network benefit level and your copayment or level of coinsurance will be lower than if a non-designated
specialist were chosen.

If you receive covered services from a Cigna participating doctor who is not a Cigna Care designated specialist,
you are covered at the in-network benefit level—but your copayment or level of coinsurance will be higher
than if a designated specialist were chosen.
Cigna Care designations may change. Before visiting your doctor, always check your directory to find out if the doctor
is a Cigna Care designated specialist. You will find the latest information on www.cigna.com.

Participation by health care professionals or physicians listed in this directory is not guaranteed and may be subject
