I am writing in support <strong>of</strong> including more specific and supportive language about Facial<br />
Feminizati<strong>on</strong> Surgery (<strong>FFS</strong>) in future revisi<strong>on</strong>s <strong>of</strong> <str<strong>on</strong>g>HBIGDA</str<strong>on</strong>g> Standards <strong>of</strong> Care. I believe it is<br />
<strong>medical</strong>ly necessary for male-<str<strong>on</strong>g>to</str<strong>on</strong>g>-female transiti<strong>on</strong>ers.<br />
When I transiti<strong>on</strong>ed last century, the c<strong>on</strong>venti<strong>on</strong>al wisdom am<strong>on</strong>g trans health service providers<br />
focused largely <strong>on</strong> the so-called “triadic therapy” <strong>of</strong> horm<strong>on</strong>es, genital surgery and therapy as part<br />
<strong>of</strong> a “real-life test.”<br />
However, it was clear <str<strong>on</strong>g>to</str<strong>on</strong>g> me that the more important procedures for male-<str<strong>on</strong>g>to</str<strong>on</strong>g>-female trans people,<br />
in terms <strong>of</strong> daily social interacti<strong>on</strong> with others, were feminizati<strong>on</strong> <strong>of</strong> the voice and face. I have<br />
been a str<strong>on</strong>g advocate <strong>of</strong> focusing <strong>on</strong> these areas for those who wish <str<strong>on</strong>g>to</str<strong>on</strong>g> assimilate in their target<br />
gender. Vocal and facial cues are far more likely <str<strong>on</strong>g>to</str<strong>on</strong>g> be fac<str<strong>on</strong>g>to</str<strong>on</strong>g>rs in how others resp<strong>on</strong>d <str<strong>on</strong>g>to</str<strong>on</strong>g> a trans<br />
woman and are in my opini<strong>on</strong> the key <str<strong>on</strong>g>to</str<strong>on</strong>g> being accepted more easily in <strong>on</strong>e’s target gender. These<br />
cues affect everything from <strong>on</strong>e’s pers<strong>on</strong>al and pr<strong>of</strong>essi<strong>on</strong>al relati<strong>on</strong>ships <str<strong>on</strong>g>to</str<strong>on</strong>g> <strong>on</strong>e’s ability <str<strong>on</strong>g>to</str<strong>on</strong>g> move<br />
through the world safely. While complete assimilati<strong>on</strong> is not every trans pers<strong>on</strong>’s goal, I would<br />
argue that a transsexual pers<strong>on</strong> is defined as some<strong>on</strong>e who successfully incorporates all available<br />
techniques <str<strong>on</strong>g>to</str<strong>on</strong>g> present in their target gender. My aphorism “passing is from the neck up” holds true<br />
in all but the most intimate situati<strong>on</strong>s. Those who focus <strong>on</strong> the so-called “triadic therapies” while<br />
neglecting these steps typically have a much harder time being accepted as women.<br />
To that end, there are three key elements a trans woman can focus <strong>on</strong> <str<strong>on</strong>g>to</str<strong>on</strong>g> feminize her face:<br />
1. facial hair<br />
2. scalp hair<br />
3. facial feminizati<strong>on</strong> procedures<br />
All three should be c<strong>on</strong>sidered <strong>medical</strong>ly necessary for those who seek feminizati<strong>on</strong>.<br />

Facial hair removal<br />
Halting and eliminating androgen-induced facial hair is critical. In my own case,<br />
permanent facial hair removal was enough <str<strong>on</strong>g>to</str<strong>on</strong>g> allow me <str<strong>on</strong>g>to</str<strong>on</strong>g> “pass” in many situati<strong>on</strong>s. This<br />
costly and painful first step should be emphasized as a bot<str<strong>on</strong>g>to</str<strong>on</strong>g>m-line <strong>necessity</strong>. There’s a<br />
reas<strong>on</strong> many female-<str<strong>on</strong>g>to</str<strong>on</strong>g>-male transiti<strong>on</strong>ers wear facial hair—it is an instant gender cue.<br />
Scalp hair<br />
Halting and correcting androgen-induced hair loss is also critical. In the way that women<br />
undergoing chemotherapy <strong>of</strong>ten find hair loss <str<strong>on</strong>g>to</str<strong>on</strong>g> be emoti<strong>on</strong>ally devastating, trans women<br />
dealing with male-pattern hair loss face a difficult obstacle. They must rectify this<br />
through <strong>medical</strong> procedures like hair transplants or scalp advancement, or through<br />
prostheses such as wigs and hair replacement systems.<br />
Facial feminizati<strong>on</strong> procedures<br />
Males and females have, <strong>on</strong> average, differentiati<strong>on</strong> <strong>on</strong> some crani<strong>of</strong>acial features.<br />
Reducing or eliminating male-typical facial characteristics can make a major difference<br />
in how <strong>on</strong>e is perceived by others.<br />
This can include b<strong>on</strong>e/cartilage work:<br />
• Reducti<strong>on</strong> <strong>of</strong> brow bossing<br />
• Making the forehead c<strong>on</strong>vex in all planes<br />
• Reducing the mandible and chin<br />
• Feminizing the nose<br />
• Feminizing the throat (trachea reducti<strong>on</strong>)<br />
It can also include s<strong>of</strong>t tissue procedures<br />
• Cheek augmentati<strong>on</strong><br />
• Lip augmentati<strong>on</strong><br />
• Skin resurfacing<br />
• Face lift<br />
While there is a relatively weak argument for s<strong>of</strong>t tissue procedures, it is clear that in<br />
many cases b<strong>on</strong>e and cartilage <strong>FFS</strong> procedures are <strong>medical</strong>ly necessary for those who<br />
wish <str<strong>on</strong>g>to</str<strong>on</strong>g> be accepted as women.<br />
Many women in my community observe the anniversary <strong>of</strong> their vaginoplasty as the apex <strong>of</strong> their<br />
transiti<strong>on</strong>, but I feel my own turning point occurred a decade ago when I had facial feminizati<strong>on</strong><br />
surgery. That was the point I was able <str<strong>on</strong>g>to</str<strong>on</strong>g> move through the world as a woman, <str<strong>on</strong>g>to</str<strong>on</strong>g> transiti<strong>on</strong> <strong>on</strong> the<br />

job in a pr<strong>of</strong>essi<strong>on</strong>al setting, and <str<strong>on</strong>g>to</str<strong>on</strong>g> have pers<strong>on</strong>al relati<strong>on</strong>ships with people who saw me as I saw<br />
myself. I believe the reas<strong>on</strong> my transiti<strong>on</strong> has g<strong>on</strong>e as well as it did is because <strong>of</strong> my focus <strong>on</strong><br />
face and voice, and I c<strong>on</strong>tinue <str<strong>on</strong>g>to</str<strong>on</strong>g> advocate that others make these their <str<strong>on</strong>g>to</str<strong>on</strong>g>p priorities if they wish<br />
<str<strong>on</strong>g>to</str<strong>on</strong>g> assimilate.<br />
Of course, many <strong>of</strong> these issues would be irrelevant if puberty-delaying androgen blockers were<br />
available <str<strong>on</strong>g>to</str<strong>on</strong>g> gender-variant women at the <strong>on</strong>set <strong>of</strong> puberty, but that <str<strong>on</strong>g>to</str<strong>on</strong>g>pic will have <str<strong>on</strong>g>to</str<strong>on</strong>g> wait for<br />
another discussi<strong>on</strong>.<br />
I urge <str<strong>on</strong>g>HBIGDA</str<strong>on</strong>g> and all helping pr<strong>of</strong>essi<strong>on</strong>als <str<strong>on</strong>g>to</str<strong>on</strong>g> place a greater emphasis <strong>on</strong> voice and face in the<br />
Standards <strong>of</strong> Care, and I especially urge you <str<strong>on</strong>g>to</str<strong>on</strong>g> c<strong>on</strong>sider advocating for the <strong>medical</strong> <strong>necessity</strong> <strong>of</strong><br />
facial feminizati<strong>on</strong> procedures for those exposed <str<strong>on</strong>g>to</str<strong>on</strong>g> androgens during puberty. I am happy <str<strong>on</strong>g>to</str<strong>on</strong>g><br />
discuss this further with any<strong>on</strong>e who is interested. I run several websites related <str<strong>on</strong>g>to</str<strong>on</strong>g> trans c<strong>on</strong>sumer<br />
issues which get over 10,000 visi<str<strong>on</strong>g>to</str<strong>on</strong>g>rs each day (about 4 milli<strong>on</strong> annually). I also co-founded a<br />
producti<strong>on</strong> company that focuses <strong>on</strong> trans depicti<strong>on</strong>s in the media. This letter is a distillati<strong>on</strong> <strong>of</strong><br />
what I have learned in ten years <strong>of</strong> corresp<strong>on</strong>dence with women in transiti<strong>on</strong>.<br />
