Foreword<br />

Systematic research <strong>and</strong> data on <strong>the</strong> health <strong>of</strong> Traveller <strong>and</strong> Gypsy communities,<br />

<strong>and</strong> <strong>the</strong> size <strong>of</strong> <strong>the</strong>se communities, is still lack<strong>in</strong>g. The scarce evidence that does<br />

exist suggests that <strong>the</strong> health <strong>and</strong> wellbe<strong>in</strong>g <strong>of</strong> <strong>the</strong>se communities <strong>and</strong> <strong>the</strong><br />

provision <strong>of</strong> health services to <strong>the</strong>m is extremely poor.<br />

This report presents <strong>the</strong> best evidence available about <strong>the</strong> extent <strong>of</strong> <strong>the</strong>se health<br />

needs. It highlights changes through time, <strong>the</strong> size <strong>and</strong> distribution <strong>of</strong> <strong>the</strong> Traveller<br />

<strong>and</strong> Gypsy communities <strong>in</strong> <strong>the</strong> <strong>South</strong> <strong>West</strong>, <strong>and</strong> <strong>the</strong> services <strong>and</strong> <strong>in</strong>terventions <strong>in</strong><br />

place to meet <strong>the</strong>se needs.<br />

S<strong>in</strong>ce <strong>the</strong> orig<strong>in</strong>al report <strong>in</strong> 2002, <strong>the</strong> situation regard<strong>in</strong>g <strong>the</strong> provision <strong>of</strong> most<br />

services to <strong>the</strong>se communities has improved, although <strong>the</strong>re are still concerns over<br />

<strong>the</strong> provision <strong>of</strong> sexual health, dentistry, <strong>and</strong> drugs <strong>and</strong> alcohol services.<br />

Concern is also expressed regard<strong>in</strong>g <strong>the</strong> susta<strong>in</strong>ability <strong>of</strong> <strong>the</strong> short-term fund<strong>in</strong>g<br />

commitments used to deliver a number <strong>of</strong> <strong>the</strong>se services. To ensure cont<strong>in</strong>ued<br />

improvements long-term commitment must be secured.<br />

Traveller <strong>and</strong> Gypsy communities <strong>in</strong> <strong>the</strong> <strong>South</strong> <strong>West</strong> have cont<strong>in</strong>ued to grow <strong>and</strong><br />

<strong>the</strong>ir distribution has changed. This <strong>in</strong>creases <strong>the</strong> importance <strong>of</strong> tackl<strong>in</strong>g health<br />

issues that affect <strong>the</strong>m <strong>and</strong> address<strong>in</strong>g <strong>the</strong> health <strong>in</strong>equalities between Traveller<br />

<strong>and</strong> Gypsy communities <strong>and</strong> <strong>the</strong> population as a whole.<br />

Statutory, community <strong>and</strong> voluntary organisations <strong>in</strong> <strong>the</strong> <strong>South</strong> <strong>West</strong> need to work<br />

toge<strong>the</strong>r to cont<strong>in</strong>ue to improve services <strong>and</strong> <strong>in</strong>crease <strong>the</strong>ir impact on <strong>the</strong> health<br />

<strong>and</strong> wellbe<strong>in</strong>g <strong>of</strong> <strong>the</strong> Traveller <strong>and</strong> Gypsy communities <strong>in</strong> <strong>the</strong> region.<br />

I urge all those concerned to read this report <strong>and</strong> consider its implications for <strong>the</strong>ir<br />

Traveller <strong>and</strong> Gypsy communities. As a start<strong>in</strong>g po<strong>in</strong>t, it should be used to<br />

establish whe<strong>the</strong>r current service delivery is adequate, given <strong>the</strong> persistent health<br />

problems faced by <strong>the</strong>se communities <strong>and</strong> <strong>the</strong> geographic distribution <strong>and</strong> recent<br />

changes <strong>in</strong> Traveller <strong>and</strong> Gypsy populations.<br />

1. Introduction<br />

In August 2002 <strong>the</strong> <strong>South</strong> <strong>West</strong> Public <strong>Health</strong> Observatory (SWPHO)<br />

commissioned a report to look at <strong>the</strong> range <strong>and</strong> quality <strong>of</strong> evidence available on <strong>the</strong><br />

topic <strong>of</strong> <strong>the</strong> health <strong>of</strong> <strong>Travellers</strong>. There was a relatively small amount <strong>of</strong> research<br />

available, all <strong>of</strong> which suggested that <strong>Travellers</strong> experienced worse health<br />

outcomes than <strong>the</strong> general population <strong>and</strong> a range <strong>of</strong> problems <strong>in</strong> access<strong>in</strong>g<br />

healthcare services.<br />

The report addressed <strong>the</strong> current state <strong>of</strong> knowledge, <strong>and</strong> suggested strategies<br />

that might lead to a more evidence-based approach to Traveller health <strong>and</strong><br />

healthcare. It concluded that <strong>the</strong> regional picture was one <strong>of</strong> relatively little activity<br />

directed specifically at Traveller communities. With <strong>the</strong> exception <strong>of</strong> those<br />

authorities with specialist liaison workers, <strong>the</strong>re were few examples <strong>of</strong> specialist<br />

services or <strong>of</strong> dedicated systems for data collection or needs assessment.<br />

The Department <strong>of</strong> <strong>Health</strong> <strong>South</strong> <strong>West</strong>, with <strong>the</strong> support <strong>of</strong> <strong>the</strong> SWPHO, agreed to<br />

update <strong>the</strong> orig<strong>in</strong>al report. The aim <strong>of</strong> this is to:<br />

• refresh <strong>the</strong> literature review <strong>of</strong> <strong>the</strong> orig<strong>in</strong>al report <strong>and</strong> analyse any new material<br />

perta<strong>in</strong><strong>in</strong>g to Traveller <strong>and</strong> Gypsy health published s<strong>in</strong>ce <strong>the</strong> orig<strong>in</strong>al study;<br />

• use recent caravan count data to map <strong>Travellers</strong> <strong>and</strong> Gypsy populations <strong>in</strong> <strong>the</strong><br />

<strong>South</strong> <strong>West</strong> <strong>and</strong> to look at trends emerg<strong>in</strong>g from this exercise;<br />

• <strong>in</strong>vestigate, via a questionnaire, how much work, if any, is be<strong>in</strong>g done by Local<br />

Authorities <strong>and</strong> PCTs with regard to Traveller <strong>and</strong> Gypsy health;<br />

• explore if this agenda has moved forward <strong>and</strong> whe<strong>the</strong>r any <strong>of</strong> <strong>the</strong> orig<strong>in</strong>al<br />

recommendations have been addressed.<br />

Importantly, this report should be read <strong>in</strong> conjunction with <strong>the</strong> orig<strong>in</strong>al document<br />

As with all research, issues <strong>of</strong> def<strong>in</strong>ition <strong>and</strong> term<strong>in</strong>ology are at <strong>the</strong> forefront. After<br />

consideration it was decided that <strong>the</strong> research would relate to those who def<strong>in</strong>e<br />

<strong>the</strong>mselves as English/Welsh <strong>and</strong> Roma <strong>Gypsies</strong>, Irish <strong>Travellers</strong> <strong>and</strong> Scottish<br />

<strong>Travellers</strong>. The <strong>in</strong>clusive term '<strong>Travellers</strong> <strong>and</strong> <strong>Gypsies</strong>' is used throughout to cover<br />

all <strong>the</strong>se groups <strong>and</strong>, as a generic term, to refer to people who have a historical<br />

<strong>and</strong> cultural tradition based on a mobile lifestyle.<br />

The aim <strong>of</strong> this report is to provide <strong>in</strong>formation on <strong>the</strong> extent <strong>and</strong> quality <strong>of</strong> <strong>the</strong><br />

evidence base relat<strong>in</strong>g to <strong>Travellers</strong>' health for those whose work relates to<br />

provid<strong>in</strong>g health <strong>and</strong> o<strong>the</strong>r services for this group, as well as for those <strong>in</strong> policymak<strong>in</strong>g<br />

positions regard<strong>in</strong>g <strong>the</strong> distribution <strong>and</strong> utilisation <strong>of</strong> resources. More<br />

generally, it will be <strong>of</strong> <strong>in</strong>terest to anyone concerned with underst<strong>and</strong><strong>in</strong>g issues <strong>of</strong><br />

health <strong>in</strong>equality <strong>and</strong> social exclusion, particularly those with responsibility for<br />

develop<strong>in</strong>g effective strategies for tackl<strong>in</strong>g health <strong>in</strong>equalities <strong>and</strong> improv<strong>in</strong>g <strong>the</strong><br />

health <strong>of</strong> <strong>the</strong> worst <strong>of</strong>f groups <strong>in</strong> society.<br />

2. Review <strong>of</strong> literature summary<br />

A review <strong>of</strong> <strong>the</strong> literature on <strong>the</strong> health <strong>of</strong> <strong>Travellers</strong> <strong>and</strong> <strong>Gypsies</strong> published s<strong>in</strong>ce<br />

<strong>the</strong> previous report was conducted, <strong>and</strong> sources were assessed to ga<strong>in</strong> an<br />

overview <strong>of</strong> <strong>the</strong>ir health <strong>and</strong> needs.<br />

In order to carry out this review, searches were conducted on five computerised<br />

databases: Medl<strong>in</strong>e, PubMed, Ovid, CINHAL <strong>and</strong> <strong>the</strong> HEA. Search terms <strong>in</strong>cluded<br />

Gyps*, Traveller*, health* <strong>and</strong> Roma* <strong>and</strong> searches were limited to articles<br />

published <strong>in</strong> English s<strong>in</strong>ce January 2001, thus refresh<strong>in</strong>g <strong>the</strong> search conducted for<br />

<strong>the</strong> orig<strong>in</strong>al report. In addition, Internet search eng<strong>in</strong>es were used to f<strong>in</strong>d <strong>the</strong><br />

caravan count details <strong>and</strong> to <strong>in</strong>vestigate if <strong>the</strong>re was any work be<strong>in</strong>g done locally or<br />

nationally regard<strong>in</strong>g Traveller <strong>and</strong> Gypsy health. See Section 4 for l<strong>in</strong>ks to <strong>the</strong><br />

count data. This fresh search identified 16 relevant additions s<strong>in</strong>ce <strong>the</strong> previous<br />

literature review. Details <strong>of</strong> <strong>the</strong>se are shown <strong>in</strong> Appendix 1.<br />

As with <strong>the</strong> previous report, literature <strong>in</strong> this area is very limited <strong>and</strong> <strong>the</strong>re are still<br />

huge gaps <strong>in</strong> research <strong>and</strong> data. The lack <strong>of</strong> research makes it difficult to draw<br />

conclusions but <strong>the</strong> general consensus from <strong>the</strong> sources that are available is that<br />

<strong>the</strong> health <strong>of</strong> <strong>Travellers</strong> <strong>and</strong> <strong>Gypsies</strong> is still worse than <strong>the</strong> national average. This is<br />

echoed by Parry et al. (2007) who are <strong>the</strong> most published scholars on Traveller<br />

<strong>and</strong> Gypsy health <strong>in</strong> Engl<strong>and</strong>:<br />

There is a lack <strong>of</strong> reliable research evidence on <strong>the</strong> health status <strong>of</strong> adult<br />

<strong>Gypsies</strong> <strong>and</strong> <strong>Travellers</strong> <strong>in</strong> Engl<strong>and</strong> <strong>and</strong> no well-designed epidemiological<br />

research, most studies be<strong>in</strong>g small, localised <strong>and</strong> descriptive. These studies<br />

<strong>in</strong>dicate high <strong>in</strong>fant mortality <strong>and</strong> per<strong>in</strong>atal death rates, low birth weight, low<br />

immunisation uptake <strong>and</strong> high accident rate. 1<br />

This view is supported by many <strong>of</strong> <strong>the</strong> o<strong>the</strong>r authors; for example, <strong>the</strong>re is evidence<br />

that:<br />

• <strong>in</strong>fant mortality rates are up to five times higher among this m<strong>in</strong>ority group when<br />

compared to <strong>the</strong> national rate; 2<br />

• <strong>the</strong> immunisation rates among <strong>Travellers</strong>‟ children are low compared with <strong>the</strong><br />

rest <strong>of</strong> <strong>the</strong> population. Some suggest that GPs are reluctant to register<br />

<strong>Travellers</strong> as <strong>the</strong>y are <strong>of</strong> no fixed abode, mean<strong>in</strong>g <strong>the</strong>y cannot be counted<br />

towards targets <strong>and</strong> <strong>the</strong>refore remuneration; 3,2,1<br />

• <strong>the</strong>re is a high accident rate among <strong>the</strong> Traveller <strong>and</strong> Gypsy population, which<br />

is directly related to <strong>the</strong> hazardous conditions on many Traveller sites: “<strong>the</strong>y<br />

have been stuck <strong>in</strong> places where no-one else would want to live”. 4 This is<br />

particularly so as sites are <strong>of</strong>ten close to motorways or major roads, refuse tips,<br />

sewage work, railways or <strong>in</strong>dustrialized areas. <strong>Health</strong> <strong>and</strong> safety st<strong>and</strong>ards are<br />

<strong>of</strong>ten poor <strong>and</strong> <strong>the</strong>re is repeated evidence <strong>of</strong> verm<strong>in</strong> <strong>in</strong>festation <strong>and</strong> basic<br />

amenities that are unfit for purpose;<br />

• <strong>Travellers</strong> have lower levels <strong>of</strong> breastfeed<strong>in</strong>g; 5<br />

• <strong>the</strong>re is also a higher prevalence <strong>of</strong> many medical conditions when compared to<br />

<strong>the</strong> general population, <strong>in</strong>clud<strong>in</strong>g miscarriage rate, 6 respiratory problems, 5<br />

arthritis, 6 cardiovascular disease, 1 depression 7 <strong>and</strong> maternal death rates. 8<br />

The literature review also identified a range <strong>of</strong> services that are not readily<br />

available to <strong>the</strong> Traveller community:<br />

• maternity services; 8<br />

• GP services; 3<br />

• sexual health services (<strong>in</strong>clud<strong>in</strong>g family plann<strong>in</strong>g services) 2<br />

• smok<strong>in</strong>g cessation services; 1<br />

• dentistry services; 9<br />

• mental health services; 10<br />

• drug <strong>and</strong> alcohol services. 5<br />

There is also anecdotal evidence concern<strong>in</strong>g <strong>the</strong> risks <strong>of</strong> HIV, <strong>in</strong>ject<strong>in</strong>g drug users<br />

<strong>and</strong> sex workers with<strong>in</strong> <strong>the</strong>se communities, though significant collection <strong>of</strong> data is<br />

absent. 11<br />

Ano<strong>the</strong>r thread runn<strong>in</strong>g throughout <strong>the</strong> literature reviewed was that among<br />

<strong>Travellers</strong> <strong>and</strong> <strong>Gypsies</strong>, coherent cultural beliefs <strong>and</strong> attitudes underp<strong>in</strong> healthrelated<br />

behaviour, <strong>and</strong> health experiences must <strong>the</strong>refore be understood <strong>in</strong> this<br />

context. In this group, “ill health is seen as st<strong>and</strong>ard, an unavoidable consequence<br />

<strong>of</strong> adverse social experiences, <strong>and</strong> is stoically <strong>and</strong> fatalistically accepted”. 6<br />

Generally <strong>the</strong> communities have low expectations <strong>in</strong> regard to <strong>the</strong>ir health <strong>and</strong> life<br />

expectancy. Studies have repeatedly shown that <strong>Travellers</strong> <strong>of</strong>ten live <strong>in</strong> extremely<br />

unhealthy conditions, while at <strong>the</strong> same time us<strong>in</strong>g health services much less <strong>of</strong>ten<br />

than <strong>the</strong> rest <strong>of</strong> <strong>the</strong> population.<br />

The more recent literature published s<strong>in</strong>ce <strong>the</strong> earlier report, cont<strong>in</strong>ues to<br />

document <strong>the</strong> adverse health conditions, adverse lifestyle factors <strong>and</strong> poor access<br />

to services experienced by members <strong>of</strong> <strong>the</strong>se communities. No new issues were<br />

highlighted <strong>in</strong> <strong>the</strong> 16 papers reviewed <strong>and</strong> which are listed below:<br />

3. Questionnaire<br />

3.1 Questionnaire methodology<br />

The questionnaire design was based on <strong>the</strong> template used <strong>in</strong> <strong>the</strong> orig<strong>in</strong>al report. It<br />

was modernised somewhat to extract more detailed <strong>in</strong>formation from <strong>the</strong> Primary<br />

Care Trusts <strong>and</strong> Local Authorities <strong>in</strong> <strong>the</strong> <strong>South</strong> <strong>West</strong>. For example, Question 7 was<br />

added to acquire more <strong>in</strong>formation on <strong>the</strong> availability <strong>of</strong> those specific services<br />

identified by <strong>the</strong> literature review as “not readily available” to <strong>the</strong> <strong>Travellers</strong> <strong>and</strong><br />

Gypsy communities <strong>in</strong> <strong>the</strong> region.<br />

The revised template was circulated to colleagues <strong>in</strong> <strong>the</strong> Regional Public <strong>Health</strong><br />

Group for comment <strong>and</strong> <strong>the</strong>se comments were <strong>in</strong>corporated <strong>in</strong>to <strong>the</strong> f<strong>in</strong>al design.<br />

See Appendix 2 for a copy <strong>of</strong> <strong>the</strong> f<strong>in</strong>al template.<br />

The questionnaire was circulated to all <strong>the</strong> Directors <strong>of</strong> Public <strong>Health</strong> at <strong>the</strong><br />

fourteen Primary Care Trusts <strong>in</strong> <strong>the</strong> region. It was accompanied by a letter<br />

expla<strong>in</strong><strong>in</strong>g <strong>the</strong> nature <strong>and</strong> aim <strong>of</strong> <strong>the</strong> questionnaire for dissem<strong>in</strong>ation to <strong>the</strong><br />

Traveller <strong>and</strong> Gypsy lead ei<strong>the</strong>r with<strong>in</strong> <strong>the</strong> PCT or Local Authority, or <strong>in</strong> some cases<br />

both.<br />

A summary <strong>of</strong> <strong>the</strong> responses can be found <strong>in</strong> Appendix 3.<br />

100% <strong>of</strong> <strong>the</strong> questionnaires were returned <strong>and</strong> <strong>in</strong> some cases a response was<br />

received from both <strong>the</strong> Primary Care Trust <strong>and</strong> Local Authority. The follow<strong>in</strong>g<br />

The authors acknowledge that <strong>the</strong>re have been changes to <strong>the</strong> NHS <strong>and</strong> Local<br />

Authority boundaries s<strong>in</strong>ce <strong>the</strong> last report was published; never<strong>the</strong>less, we have<br />

attempted to compare responses <strong>in</strong> a way that reflects <strong>the</strong>se adjustments.<br />

Aga<strong>in</strong>, <strong>the</strong>re has been a marked improvement s<strong>in</strong>ce 2002.<br />

4. Caravan count<br />

4.1 Caravan count summary<br />

The system <strong>of</strong> twice-yearly counts <strong>of</strong> Traveller <strong>and</strong> Gypsy caravans <strong>and</strong> families<br />

was <strong>in</strong>troduced <strong>in</strong> Engl<strong>and</strong> <strong>in</strong> 1979. Local Authorities carry out <strong>the</strong> count <strong>and</strong> return<br />

figures to <strong>the</strong> Department for Communities <strong>and</strong> Local Government. The count is<br />

carried out on a specified date <strong>in</strong> January <strong>and</strong> July each year, giv<strong>in</strong>g a snapshot <strong>of</strong><br />

<strong>the</strong> number <strong>and</strong> location <strong>of</strong> caravans/families on those days. January <strong>and</strong> July<br />

were selected to give an idea <strong>of</strong> w<strong>in</strong>ter <strong>and</strong> summer patterns, given <strong>the</strong> known<br />

seasonality <strong>of</strong> travell<strong>in</strong>g.<br />

The count system was reviewed <strong>in</strong> 2004 but none <strong>of</strong> <strong>the</strong> recommendations have<br />

been implemented (N<strong>in</strong>er, P. Count<strong>in</strong>g <strong>Gypsies</strong> & <strong>Travellers</strong>: A Review <strong>of</strong> <strong>the</strong><br />

Gypsy Caravan Count System, February 2004, Office <strong>of</strong> <strong>the</strong> Deputy Prime M<strong>in</strong>ister:<br />

London). The primary concern with <strong>the</strong> count is <strong>the</strong> accuracy <strong>of</strong> numbers. Often,<br />

when <strong>in</strong>formation is not available by <strong>the</strong> return date, a zero is entered. This is<br />

exacerbated by <strong>the</strong> fact that <strong>the</strong> counts are voluntary. There are no rewards for<br />

Local Authorities complet<strong>in</strong>g <strong>the</strong> returns or penalties for those who fail to do so.<br />

The count is also based on <strong>the</strong> number <strong>of</strong> caravans <strong>and</strong> <strong>the</strong>refore does not give an<br />

accurate number <strong>of</strong> <strong>Travellers</strong> but ra<strong>the</strong>r a record <strong>of</strong> Traveller units or families.<br />

Data on numbers <strong>of</strong> adults <strong>and</strong> children <strong>in</strong> each caravan are not analysed, <strong>and</strong><br />

<strong>the</strong>refore are never published. The count also significantly does not count<br />

<strong>Travellers</strong> <strong>and</strong> <strong>Gypsies</strong> liv<strong>in</strong>g <strong>in</strong> houses.<br />

Figure 1 shows <strong>the</strong> total number <strong>of</strong> caravans <strong>in</strong> January 2009. The ranges are grouped<br />

<strong>in</strong> approximate qu<strong>in</strong>tiles, or fifths, accord<strong>in</strong>g to <strong>the</strong> number <strong>of</strong> caravans counted.<br />

Figure 2 shows <strong>the</strong> difference between <strong>the</strong> January 2007 <strong>and</strong> January 2009 total,<br />

expressed as a percentage. The ranges for display have been custom selected to<br />

give a reasonable group<strong>in</strong>g <strong>of</strong> areas. Around half <strong>of</strong> Local Authorities <strong>in</strong> <strong>the</strong> <strong>South</strong><br />

<strong>West</strong> have seen a decrease <strong>in</strong> <strong>the</strong> number <strong>of</strong> caravans over this period. In<br />

contrast, some areas have seen considerable <strong>in</strong>creases <strong>in</strong> numbers with North<br />

Dorset (265%), Mid Devon (330%) <strong>and</strong>, <strong>in</strong> particular, <strong>West</strong> Wiltshire (393%)<br />

show<strong>in</strong>g <strong>the</strong> biggest <strong>in</strong>creases.<br />

The cumulative rank<strong>in</strong>g for each area (Figure 3) is formed by rank<strong>in</strong>g areas on <strong>the</strong><br />

total number <strong>of</strong> caravans at each measurement date. These rank<strong>in</strong>gs are <strong>the</strong>n<br />

summed to give a total overall rank<strong>in</strong>g. This rank<strong>in</strong>g should give an idea <strong>of</strong> which<br />

areas have consistently had high/low numbers <strong>of</strong> caravans. A higher rank<strong>in</strong>g<br />

equates to a higher number. The ranges are grouped <strong>in</strong> approximate qu<strong>in</strong>tiles, so<br />

that roughly <strong>the</strong> fifth <strong>of</strong> areas with <strong>the</strong> highest rank are shaded <strong>in</strong> <strong>the</strong> darkest<br />

colour. Overall, <strong>the</strong>re is no significant pattern; however, it is noted that <strong>the</strong> areas<br />

with consistently lower numbers are scattered across <strong>the</strong> region <strong>and</strong> <strong>in</strong>clude rural<br />

<strong>and</strong> urban areas.<br />

Figure 4 shows <strong>the</strong> number <strong>of</strong> caravans per square-mile, based on <strong>the</strong> January<br />

2009 totals. The ranges are grouped <strong>in</strong> approximate qu<strong>in</strong>tiles, so that roughly <strong>the</strong><br />

fifth <strong>of</strong> areas with <strong>the</strong> highest density <strong>of</strong> caravans are shaded <strong>in</strong> <strong>the</strong> darkest colour.<br />

When look<strong>in</strong>g at <strong>the</strong> density <strong>of</strong> caravans per square mile, a clearer pattern<br />

emerges, with higher rates <strong>in</strong> Somerset, <strong>West</strong> Gloucestershire <strong>and</strong> <strong>the</strong> old Avon<br />

area.<br />

4.3 Summary<br />

When consider<strong>in</strong>g all four maps toge<strong>the</strong>r, <strong>the</strong>re are two ma<strong>in</strong> conclusions which<br />

can be drawn. The first is that over half <strong>of</strong> Local Authorities <strong>in</strong> <strong>the</strong> <strong>South</strong> <strong>West</strong><br />

exhibited a decl<strong>in</strong>e <strong>in</strong> <strong>the</strong> numbers <strong>of</strong> caravans. However, North Dorset, Mid<br />

Devon, <strong>and</strong> <strong>West</strong> Wiltshire are notable for show<strong>in</strong>g <strong>in</strong>creases <strong>of</strong> 265%, 330% <strong>and</strong><br />

393%. Secondly, when <strong>the</strong> density <strong>of</strong> caravans <strong>in</strong> any Local Authority is taken <strong>in</strong>to<br />

account, a pattern <strong>of</strong> higher density <strong>in</strong> <strong>the</strong> North East <strong>of</strong> <strong>the</strong> region is apparent –<br />

although <strong>the</strong>re are exceptions <strong>in</strong> East Wiltshire <strong>and</strong> parts <strong>of</strong> Gloucestershire.<br />

Generally speak<strong>in</strong>g, densities are lower <strong>in</strong> <strong>the</strong> south-western parts <strong>of</strong> <strong>the</strong> region. It<br />

is not clear why this may be but vary<strong>in</strong>g levels <strong>of</strong> access by road are likely to be a<br />

factor.<br />

It is important to note that <strong>in</strong> most Local Authorities <strong>the</strong> total number <strong>of</strong> caravans is<br />

small <strong>and</strong> hence <strong>the</strong>re is an <strong>in</strong>herent uncerta<strong>in</strong>ty <strong>in</strong> any conclusions drawn from <strong>the</strong><br />

data.<br />

5. Summary <strong>and</strong> recommendations<br />

As with <strong>the</strong> orig<strong>in</strong>al report, <strong>the</strong> review <strong>of</strong> literature <strong>in</strong>dicates that work <strong>in</strong> this area is<br />

very limited <strong>and</strong> <strong>the</strong>re are still vast gaps <strong>in</strong> research <strong>and</strong> data perta<strong>in</strong><strong>in</strong>g to <strong>the</strong><br />

health <strong>of</strong> Traveller <strong>and</strong> Gypsy communities. The lack <strong>of</strong> research on <strong>Travellers</strong> <strong>and</strong><br />

<strong>Gypsies</strong> makes it challeng<strong>in</strong>g to draw conclusions but <strong>the</strong> general consensus from<br />

<strong>the</strong> sources that are available is that <strong>the</strong> healthcare <strong>of</strong> <strong>Travellers</strong> <strong>and</strong> <strong>Gypsies</strong> is<br />

still worse than <strong>the</strong> national average.<br />

The questionnaire results pa<strong>in</strong>t a far more positive picture regard<strong>in</strong>g work be<strong>in</strong>g<br />

done <strong>in</strong> <strong>the</strong> <strong>South</strong> <strong>West</strong> region to address <strong>the</strong> health <strong>in</strong>equalities experienced by<br />

this m<strong>in</strong>ority group. For example, 13 <strong>of</strong> <strong>the</strong> 14 local partnerships have a specific<br />

person designated to deal with <strong>the</strong> health <strong>and</strong> wellbe<strong>in</strong>g <strong>of</strong> <strong>Travellers</strong> <strong>and</strong> <strong>Gypsies</strong>,<br />

a significant change from 2002.<br />

In addition 9 <strong>of</strong> <strong>the</strong> 14 local partnerships have a specific Traveller <strong>and</strong> Gypsy forum<br />

or network <strong>in</strong> which <strong>the</strong>se communities can discuss health <strong>and</strong> wellbe<strong>in</strong>g issues<br />

<strong>and</strong> concerns.<br />

The questionnaire highlighted that specific services for <strong>Travellers</strong> have improved<br />

significantly s<strong>in</strong>ce <strong>the</strong> orig<strong>in</strong>al study. For example, access to GPs, breastfeed<strong>in</strong>g,<br />

immunisations <strong>and</strong> <strong>in</strong>jury prevention services are available to <strong>Travellers</strong> <strong>in</strong> almost<br />

half <strong>of</strong> local areas.<br />

Areas that need to be improved are sexual health, dentistry, <strong>and</strong> drugs <strong>and</strong> alcohol<br />

services, all <strong>of</strong> which scored low on <strong>the</strong> questionnaire.<br />

Despite not ask<strong>in</strong>g for specific <strong>in</strong>formation regard<strong>in</strong>g <strong>the</strong> security <strong>of</strong> fund<strong>in</strong>g<br />

available for <strong>the</strong> provision <strong>of</strong> Traveller-specific health <strong>in</strong>terventions, a number <strong>of</strong><br />

respondents expressed concern that much <strong>of</strong> <strong>the</strong> current activity is based on shortterm<br />

fund<strong>in</strong>g commitments <strong>and</strong> that to ensure cont<strong>in</strong>ued improvements long-term<br />

commitment must be secured.<br />

There is considerable scope for improvement <strong>in</strong> terms <strong>of</strong> our ability to estimate <strong>the</strong><br />

size <strong>of</strong> <strong>the</strong> Traveller <strong>and</strong> Gypsy population with<strong>in</strong> <strong>the</strong> region. The available data,<br />

The Department <strong>of</strong> <strong>Health</strong> has recently launched a new <strong>in</strong>itiative, <strong>the</strong> Pacesetters<br />

Programme, to deal with health <strong>in</strong>equalities among m<strong>in</strong>ority groups.<br />

One <strong>of</strong> <strong>the</strong> facets <strong>of</strong> this programme is engagement with <strong>the</strong> Traveller <strong>and</strong> Gypsy<br />

community. Contact has been made with <strong>the</strong> programme leads to source more<br />

<strong>in</strong>formation on how <strong>the</strong> <strong>in</strong>itiative will target <strong>Travellers</strong> <strong>in</strong> <strong>the</strong> <strong>South</strong> <strong>West</strong> region but,<br />

Observatory<br />

The <strong>South</strong> <strong>West</strong> Public <strong>Health</strong> Observatory (SWPHO) is<br />

part <strong>of</strong> a network <strong>of</strong> 12 public health observatories work<strong>in</strong>g<br />

across <strong>the</strong> five nations <strong>of</strong> Engl<strong>and</strong>, Scotl<strong>and</strong>, Wales,<br />

Nor<strong>the</strong>rn Irel<strong>and</strong> <strong>and</strong> <strong>the</strong> Republic <strong>of</strong> Irel<strong>and</strong>. The n<strong>in</strong>e<br />

Public <strong>Health</strong> Observatories <strong>in</strong> Engl<strong>and</strong> work toge<strong>the</strong>r<br />

through a s<strong>in</strong>gle work programme which conta<strong>in</strong>s both<br />

national <strong>and</strong> local elements. We produce <strong>in</strong>formation,<br />

data <strong>and</strong> <strong>in</strong>telligence on people’s health <strong>and</strong> health care<br />

for practitioners, policy makers <strong>and</strong> <strong>the</strong> wider community.<br />

Our expertise lies <strong>in</strong> turn<strong>in</strong>g <strong>in</strong>formation <strong>and</strong> data <strong>in</strong>to<br />

mean<strong>in</strong>gful health <strong>in</strong>telligence to support decision makers.<br />

On behalf <strong>of</strong> <strong>the</strong> Department <strong>of</strong> <strong>Health</strong>, <strong>the</strong> SWPHO works<br />

<strong>in</strong> partnership with <strong>the</strong> NHS, local authorities, researchers,<br />

national agencies as well as agencies <strong>in</strong> <strong>the</strong> <strong>South</strong> <strong>West</strong>.<br />

The SWPHO <strong>in</strong>corporates <strong>the</strong> National Drug Treatment<br />

Monitor<strong>in</strong>g System <strong>South</strong> <strong>West</strong> (NDTMS-SW), <strong>and</strong> <strong>in</strong> April<br />

2005 merged with <strong>the</strong> <strong>South</strong> <strong>West</strong> Cancer Intelligence<br />

Service (SWCIS).<br />
