concerning to my/our Own a Philippine Home Loan Program application.
I/We, agree to secure the validity of the following information from my/our designated employers
and other institutions:
1) Employment background (length of employment, income, benefits and
compensation);
2) Personal banking and deposit accounts;
3) Loan / Credit accommodation rating (opening date, high credit payment
amount, loan balance and payment record);
4) Bank, Trade and Court Checkings (Philippines);
5) Any other information deemed necessary in connection with a credit report.
I/We, also authorize Philippine National Bank, Tokyo Branch to use my/our personal identification,
financial information, and any other information that may be needed for the purpose of audit by
the Philippine Authority and Japanese Authority concerned.
This also allows Philippine National Bank, Tokyo Branch to send marketing or any bank information
for dissemination through the address stated on my/our identification.
The bank shall govern this authorization strictly for loan purpose and other bank related
transactions only.
