Message from the
President and
Executive Director
In our lifetimes, medicine has made tremendous strides in preventing
and treating eye disease. Just 20 years ago, 6 million people in the world
were blind from trachoma. Today, it’s 1.3 million. Just 15 years ago,
there were 350,000 blind from onchocerciasis (river blindness), today
it’s 295,000 thanks to IEF, our sister agencies, the World Health
Organization, and the governments of developing countries themselves.

As these scourges were being reduced, IEF began turning its attention to
new challenges facing developing countries. We began to focus on the
quality of eye care being delivered, and looked for ways to improve the
management, efficiency, productivity, and financial viability of eye clinics
and hospitals. Our experience told us that many patients didn’t seek eye
care because they didn’t want to go to a public hospital and could not
afford the higher prices of a private clinic, or they were afraid of the
medical procedures, or they tired of waiting in long lines and of being
told to return at a later date. Our experience also told us that many of
these patient concerns could be overcome by management improvements
in the eye care delivery system.
IEF developed a prototype for clinic and hospital management called
SightReach® Management. It is an adaptation of the Aravind Eye Care
System in India that combines the best eye care practices with business
planning and management systems. After nine years experience with 12
clinics and hospitals in 8 countries, we see positive results, some of
which are described in this report.

We are passionate about the SightReach® approach and the advances
we can achieve. That is why we will ask for your continued support to
take SightReach Management to 10 more clinics and hospitals by 2012.
We continue to work to eradicate preventable eye diseases such as river
blindness, but cataract is still responsible for half the world’s blindness
and glaucoma, retinal diseases, and refractive error are emerging as the
leading causes of poor vision and loss of sight. Developing countries
require quality eye care to confront these conditions. IEF can help.
We are proud of our 4-star rating from Charity Navigator reflecting
wise use of your gifts to IEF. Thank you for your generous support of
our mission and our programs making it possible for more and more
people in the developing world to save their precious sight.

SightReach® Prevention

Childhood Blindness and Vitamin A Deficiency Control
IEF and Malawi’s Ministry of Health have brought sight and life-saving help to the people in the southern
region where food is scarce, health care is limited, and villagers struggle to feed their families.
Children under age 5 years, the most vulnerable, and their mothers have benefited from IEF’s child
survival program. Funded in part by USAID, vitamin A coverage increased from 49% to over 85% of
eligible children 6-59 months of age. Measles immunization coverage increased from 64% to 91%. Over
150 communities have received thousands of insecticide treated bed nets to prevent malaria.
IEF helps ophthalmologists and medical staff at our partner eye hospitals to provide better care for children with
cataracts, eye infections, injuries, and congenital diseases that lead to blindness. Thanks to your
support, we are expanding specialty eye care services for children and getting the specialized
equipment needed to operate on tiny eyes. Optical services introduced by IEF get glasses for children in
rural areas who would not get them otherwise.
Onchocerciasis—prevention of “river blindness”
IEF has over 16 years experience in onchocerciasis control having established the first “communitybased”
Mectizan Distribution Programs in Africa and Latin America. In 2006, IEF together with the
National Onchocerciasis Programs in Malawi and Cameroon treated over 2 million people with their
annual dose of Mectizan®. Support from the Insel Foundation and the International Foundation are
ensuring that the threat of blindness is eliminated for millions because they will continue to get this
miracle drug each year.

SightReach® Management
Today’s innovative ophthalmologists are tomorrow’s leaders. IEF is
helping them change the way eye care is delivered in developing
countries. Evidence-based strategies lead to increased quality,
efficiency, productivity, and sustainability. IEF invests in training to
improve clinical and surgical outcomes, creating efficient and
patient-centered services, and establishing revenue generating
services that support growth and expansion and subsidize services
for the poor. Since 1999, 12 hospitals in 8 countries have benefited
from IEF’s SightReach Management approach.

one of our original 7, to become a
Regional Demonstration Site.

Workshops developed by IEF and Visualiza enable teams
from other Central American countries to come for
clinical and management training, completely in Spanish and
addressing regional challenges.
153 million people in our world need glasses, 13 million are
children and 45 million are working-age adults. IEF’s model
includes setting up modern optical services where patients
get new, made to prescription spectacles. Optical services
address the global need for glasses, and earned revenue helps
sustain eye care services for all patients including the poor.

Visualiza, IEF’s new Regional Demonstration Site in Latin America, showed a 288%
increase in all eye surgeries and a 326% increase in cataract surgeries from 255 in
2002 to 1,087 in 2005 after IEF introduced efficiencies and management protocols. 3
of Visualiza’s 4 ophthalmologists perform 10% of all cataract surgery in Guatemala.

Expanding the Global Network
Increasing Quality Eye Care Worldwide
IEF is committed to expanding its network of eye clinics and hospitals reaching for and
achieving sustainability. Since 1999, we have been investing grant funds and technical
assistance to build capacities that:
identify innovative medical and management staff and develop leadership,
transform private clinics to serve all patients including the poor,
transform public and government clinics to attract private patients and earn revenue,
institute standards of quality for clinical care and services,
create professional environments that attract and retain qualified eye care professionals,
customize technical assistance to the local, cultural, professional environment of each partner,
establish professional optical services and other revenue generating activities,
establish outreach programs to expand ophthalmic and optometric services,
build regional networks of sustainable eye hospitals and clinics, and
share experience and expertise between partners and stakeholders.

“Promotion of Peace & Vision Awards”
2005
The "Promotion of Peace and Vision Award" honors the memory of founder
Dr. John Harry King, Jr. and his living philosophy “...the promotion of
peace through the prevention of blindness.” Dr. King believed deeply in
the power of international exchange and the sharing of expertise to
achieve the greater goal of sight preservation and blindness prevention.
The event was hosted by Guatemalan Ambassador and Mrs. Guillermo
Castillo at their beautiful home in November 2005. “Ophthalmologist”
recipients Dr. Michael A. Lemp (above left) and Dr. Frank S. Ashburn, Jr.
(above right) were honored for their decades of service both in the field
and on the Board of Directors. Ann Donatelli, a long-time Board member
and Eye Ball gala chair, and her husband Lou (right) were awarded the
2005 “community member” award by President Susan Fritschler. People
such as these make up the rich fabric of IEF and we are very grateful.
Eye Ball® 2006
The 2006 gala was held in April at the historic and magnificent residence
of His Excellency and Mrs. Andres Pastrana, ambassador of Colombia in
Washington, DC. Guests enjoyed elegant dining, dancing to Latin music,
and partied late into the evening. The silent and live auctions raised
critical funds to sustain IEF’s programs.

Society of Eye Surgeons
Friend, colleague and mentor Dr. Ramachandra Pararajasegaram, fondly known as
Para (right), served as Honorary Chair of the Society of Eye Surgeons (SES) meeting
held during the November 2005 American Academy of Ophthalmology (AAO)
conference in Chicago. As the AAO’s 2005 recipient of its “International
Blindness Prevention Award”, he spoke of the importance of IEF’s SightReach®
Management sustainability model having visited three programs in Malawi and
Guatemala. Speakers also included field partners Dr. Joseph Msosa of Lilongwe,
Malawi and Dr. Gerald Msukwa of Blantyre, Malawi pictured below right with
Board member H.E. Bernard Herbert Sande, Malawi’s ambassador to the US during
their visit to IEF headquarters. Also presenting were Drs. Mariano and Nicolas
Yee of Visualiza in Guatemala. The hospitals in Malawi and Guatemala are two of
IEF’s original 7 partners that have transformed to social enterprises as part of IEF’s
growing network of sustainable eye units.
