below and bring <strong>the</strong>m with you <strong>to</strong> your scheduled paperwork appointment.<br />

Note: Each time you write <strong>the</strong> date, please use <strong>the</strong> following format: Month/Day/Year<br />
1. I-9, Employment Eligibility Verification –This form documents your eligibility <strong>to</strong> work in <strong>the</strong> United States. You<br />
will be required <strong>to</strong> provide documents <strong>to</strong> support your eligibility.<br />
Be prepared <strong>to</strong> present ei<strong>the</strong>r one original, unexpired document from List A or one unexpired document<br />
from both List B and List C. (See List of Acceptable Documents.)<br />
Complete only Section 1. Your name must appear exactly as it appears on <strong>the</strong> A or C document from <strong>the</strong><br />

your fingerprints for criminal his<strong>to</strong>ry records from <strong>the</strong> Florida Department of Law Enforcement and <strong>the</strong> FBI.<br />

under <strong>the</strong> National Child Protection Act of 1993, as amended,<br />
and Section 943.0542, Florida Statutes<br />
Pursuant <strong>to</strong> <strong>the</strong> National Child Protection Act of 1993, as amended, and section 943.0542, Florida Statutes, this<br />
form must be completed and signed by every current or prospective employee, volunteer, and contrac<strong>to</strong>r/vendor,<br />
for whom criminal his<strong>to</strong>ry records are requested by a qualified entity under <strong>the</strong>se laws.<br />

CONFIDENTIALITY AGREEMENT<br />
In consideration for my employment and for <strong>the</strong> compensation <strong>to</strong> be paid <strong>to</strong> me by The <strong>Walt</strong> <strong>Disney</strong> Company or a<br />
division, subsidiary, or affiliate <strong>the</strong>reof, or any successor of <strong>the</strong> foregoing (hereinafter termed <strong>the</strong> “Company”), and in<br />
addition <strong>to</strong> any o<strong>the</strong>r obligation, at all times during <strong>the</strong> term of my employment and <strong>the</strong>reafter, I do hereby agree:<br />
1. To hold in strictest confidence, and not disclose <strong>to</strong> any person, firm, or corporation without express authorization<br />
of a corporate officer of <strong>the</strong> Company, any confidential information or trade secret relating <strong>to</strong> <strong>the</strong> products, sales,<br />
or business of <strong>the</strong> Company and not <strong>to</strong> use any such confidential information or trade secret for my own benefit<br />
during <strong>the</strong> term of my employment or <strong>the</strong>reafter.<br />
2. To fully and promptly disclose <strong>to</strong> <strong>the</strong> Company and <strong>to</strong> hold in trust for <strong>the</strong> sole right and benefit of <strong>the</strong> Company,<br />
any and all intellectual property, discoveries, or trade secrets which I may solely or jointly conceive, design,<br />
develop, create or suggest or cause <strong>to</strong> be conceived, designed, developed or created during <strong>the</strong> period of time I<br />
am in <strong>the</strong> employ of <strong>the</strong> Company, which relate <strong>to</strong> or are connected with my employment or <strong>the</strong> business of <strong>the</strong><br />
Company, whe<strong>the</strong>r or not conceived or created during my regular working hours. For purposes of this<br />
agreement, <strong>the</strong> term intellectual property shall include, without limitation, any ideas, concepts, literary material,<br />
designs, drawings, illustrations and pho<strong>to</strong>graphs.<br />
3. That right, title, and interest in and <strong>to</strong> <strong>the</strong> intellectual property, discoveries and trade secrets referred <strong>to</strong> in<br />
Paragraph 2 above, shall be <strong>the</strong> sole and absolute property of <strong>the</strong> Company, subject <strong>to</strong> <strong>the</strong> limitations set forth in<br />
Paragraph 4 below.<br />
4. That I will assign <strong>to</strong> <strong>the</strong> Company all my right, title, and interest in and <strong>to</strong> <strong>the</strong> intellectual property, discoveries<br />
and trade secrets referred <strong>to</strong> in Paragraph 2 above; provided, however, that no provision in this agreement is<br />
intended <strong>to</strong> require assignment of any of my rights in any intellectual property or discovery if (i) no equipment,<br />
supplies, facilities, trade secret or confidential information of <strong>the</strong> Company was used; and 9ii) such discovery or<br />
intellectual property does not related <strong>to</strong> any business of <strong>the</strong> Company or <strong>the</strong> Company’s actual or demonstrably<br />
anticipated research or development or does not result from any work performed by me for <strong>the</strong> Company.<br />
5. I will execute any documents necessary <strong>to</strong> evidence <strong>the</strong> Company’s proprietary interest in any discovery,<br />
intellectual property or trade secret referred <strong>to</strong> in Paragraph 2 above. In <strong>the</strong> event <strong>the</strong> Company is unable, for<br />
any reason whatsoever, <strong>to</strong> secure my signature <strong>to</strong> any lawful and necessary document required <strong>to</strong> apply for<br />
protection of, or enforce any action with respect <strong>to</strong>, copyright, trademark or o<strong>the</strong>r proprietary rights, I hereby<br />
irrevocably designate and appoint <strong>the</strong> Company, and its duly authorized officers and agents, as my agent and<br />
at<strong>to</strong>rney-in-fact whose power is coupled with an interest, <strong>to</strong> act for and in my behalf and stead, <strong>to</strong> execute such<br />
documents and <strong>to</strong> do all o<strong>the</strong>r lawfully permitted acts <strong>to</strong> protect <strong>the</strong> Company’s interest in any copyright,<br />
trademark or o<strong>the</strong>r proprietary right with <strong>the</strong> same legal force and effect as if executed by me.<br />
6. That at <strong>the</strong> time of leaving <strong>the</strong> employ of <strong>the</strong> Company, I will deliver <strong>to</strong> <strong>the</strong> Company, and will not keep in my<br />
possession nor deliver <strong>to</strong> anyone else, any and all drawings, notes, notebooks, memoranda, treatments, scripts,<br />
documents or any o<strong>the</strong>r material connected with my employment by <strong>the</strong> Company or with <strong>the</strong> business of <strong>the</strong><br />
Company.<br />
7. In case of interruption of my employment with <strong>the</strong> Company, by lay-off or o<strong>the</strong>rwise, this agreement, upon reemployment,<br />
will be in full force and effect unless specifically superseded by a new agreement.<br />
8. This agreement shall not embrace or include any copyrights or trademarks or o<strong>the</strong>r proprietary rights owned or<br />
controlled wi<strong>the</strong>r jointly or separately by me prior <strong>to</strong> <strong>the</strong> time of my employment by <strong>the</strong> Company. I am listing<br />
on a separate attached sheet each copyrights, trademark or o<strong>the</strong>r proprietary right which I claim <strong>to</strong> be exempt<br />
from this agreement.<br />
9. This agreement supersedes any prior agreement with <strong>the</strong> Company relating <strong>to</strong> <strong>the</strong> subject matter set forth herein.<br />

efficiencies or o<strong>the</strong>rwise enhance <strong>the</strong> WALT DISNEY WORLD <strong>Resort</strong>. I fur<strong>the</strong>r understand that<br />
this is a limited exception <strong>to</strong> The <strong>Walt</strong> <strong>Disney</strong> Company’s policy regarding <strong>the</strong> submission of<br />
unsolicited ideas and in consideration of this I agree that <strong>the</strong> following terms and conditions shall<br />
govern with respect <strong>to</strong> any ideas or suggestions I submit or o<strong>the</strong>rwise communicate <strong>to</strong> The <strong>Walt</strong><br />
<strong>Disney</strong> Company or any of its related, subsidiary or affiliated companies (each, a “<strong>Disney</strong><br />
Company”) at any time during my employment by a <strong>Disney</strong> Company, whe<strong>the</strong>r or not such ideas<br />
or suggestions are related <strong>to</strong> matters that are included within my regular job responsibilities:<br />
I agree that all ideas or suggestions I submit or o<strong>the</strong>rwise communicate shall immediately become<br />
<strong>the</strong> sole property of The <strong>Walt</strong> <strong>Disney</strong> Company, and I hereby assign all rights (including, without<br />
limitation, copyright and patent) that I might have or acquire <strong>the</strong>rein <strong>to</strong> The <strong>Walt</strong> <strong>Disney</strong><br />
Company. I understand and agree that no payment or o<strong>the</strong>r compensation shall be payable by any<br />
<strong>Disney</strong> Company in connection with any ideas or suggestions I submit or o<strong>the</strong>rwise<br />
communicate, whe<strong>the</strong>r or not <strong>the</strong> ideas or suggestions are implemented, and that any idea or<br />
suggestion I submit or o<strong>the</strong>rwise communicate may be used in any manner (or not used at all) and<br />
for any purpose by any <strong>Disney</strong> Company. I shall only submit or o<strong>the</strong>rwise communicate ideas or<br />
suggestions that have been created and developed solely by me and I agree that no o<strong>the</strong>r party<br />
holds any rights with respect <strong>to</strong> <strong>the</strong>se ideas and suggestions. I fur<strong>the</strong>r acknowledge and agree that<br />
I shall continue <strong>to</strong> be bound by <strong>the</strong> terms of my confidentiality agreement with The <strong>Walt</strong> <strong>Disney</strong><br />
Company and that, for purposes of that confidentiality agreement, <strong>the</strong> ideas or suggestions I<br />
submit or o<strong>the</strong>rwise communicate relate <strong>to</strong> <strong>the</strong> business of a <strong>Disney</strong> Company and/or result from<br />
work performed by me for a <strong>Disney</strong> Company. I agree that no variation, modification, waiver,<br />
change or amendment of this Agreement shall be binding upon any <strong>Disney</strong> Company unless such<br />
variation, modification, change or amendment is in writing and duly authorized and executed by a<br />
corporate officer of The <strong>Walt</strong> <strong>Disney</strong> Company.<br />

CAST MEMBER POLICIES, PROCEDURES AND TERMS<br />
The following are examples of expressly forbidden conduct that you cannot do, even one time, and which may<br />
lead <strong>to</strong> disciplinary action, not excluding termination:<br />
• Our Guests may not always be right, but <strong>the</strong>y will always be our Guests. You may never insult, argue<br />
with, be discourteous <strong>to</strong>, or use profane language in <strong>the</strong> presence of our Guests.<br />
• Fighting at <strong>the</strong> WALT DISNEY WORLD® <strong>Resort</strong>, regardless of who started it, may result in termination<br />
of <strong>the</strong> parties involved.<br />
• Falsification of any documents or records, such as medical forms, payroll records, or employment applications,<br />
may lead <strong>to</strong> dismissal.<br />
• Personally using, being in possession of, or being under <strong>the</strong> influence of any controlled substance,<br />
in<strong>to</strong>xicants, drugs, or hallucina<strong>to</strong>ry agents on Company property, or reporting <strong>to</strong> work under such<br />
conditions, may result in dismissal.<br />
• <strong>Walt</strong> <strong>Disney</strong> <strong>World</strong> Co. and affiliated companies, as responsible vendors of alcoholic beverages, wish<br />
<strong>to</strong> advise all Cast Members that Florida law prohibits <strong>the</strong> purchase, possession, and consumption of<br />
alcoholic beverages by persons under <strong>the</strong> age of 21 years, as outlined in <strong>the</strong> form titled “Manda<strong>to</strong>ry<br />
Requirements for Sale and Service of Alcoholic Beverages by Employees.” Fur<strong>the</strong>r, it is unlawful for<br />
a person of legal drinking age <strong>to</strong> purchase or provide alcoholic beverages <strong>to</strong> a person under <strong>the</strong> age of<br />
21 years. Therefore, any Cast Member who violates this law on our property at any time commits a<br />
serious breach of Company policy and will be subject <strong>to</strong> termination.<br />
• Conviction of, plea of no contest, plea of guilty, or acceptance of any pre-trial diversion in lieu of charge<br />

• Violation of <strong>the</strong> Company’s Substance Abuse Policy may result in dismissal.<br />
• Theft of any food, beverage, merchandise (including items earmarked for disposal), services, or<br />
property belonging <strong>to</strong> <strong>Walt</strong> <strong>Disney</strong> <strong>World</strong> Co., its affiliated and related companies, a guest, or fellow employee,<br />
may result in termination.<br />

It is <strong>the</strong> policy of <strong>the</strong> Company that its computer systems be used appropriately and in a secure environment.<br />

Costuming Issue and Return Policy<br />
Cast Member Understanding and Authorization for Payroll Deduction<br />
The costumes issued <strong>to</strong> you by <strong>the</strong> Company will remain <strong>the</strong> property of <strong>Walt</strong> <strong>Disney</strong> <strong>World</strong> Co. It is your<br />
obligation <strong>to</strong> return <strong>the</strong> costumes issued <strong>to</strong> you by <strong>the</strong> specified due date if it is on a short term basis (special<br />
issue), or upon termination of your employment. You will be financially responsible through payroll<br />
deduction for any costumes that are not returned <strong>to</strong> <strong>Walt</strong> <strong>Disney</strong> <strong>World</strong> Co. as requested or are damaged in<br />
excess of normal wear and tear. Costumes may be returned <strong>to</strong> any WDW Costuming location.<br />
Specifically, if at any time you exceed <strong>the</strong> number of costumes you are allotted or you are issued a costume<br />
on a short term basis (i.e., a special issue costume and/or accessory causing you <strong>to</strong> exceed your authorized<br />
costume allotment,) it is your responsibility <strong>to</strong> return it by <strong>the</strong> specified due date, which is one (1) week from<br />
<strong>the</strong> issue date. IMPORTANT: If a costume or accessory is not returned by <strong>the</strong> required due date, you<br />
will be subject <strong>to</strong> payroll deduction(s) for <strong>the</strong> amount of <strong>the</strong> costume and/or accessory. Your pay will<br />
be au<strong>to</strong>matically deducted up <strong>to</strong> twenty-five dollars ($25.00) per week until <strong>the</strong> cost of <strong>the</strong> item is<br />
recovered.<br />
Cast Members replacing lost name tags will also be subject <strong>to</strong> an au<strong>to</strong>matic payroll deduction for <strong>the</strong><br />
replacement cost of <strong>the</strong> name tag.<br />
Upon termination from <strong>the</strong> Company, it is your responsibility <strong>to</strong> return all costume items issued <strong>to</strong> you. If you<br />
fail <strong>to</strong> do so you will be subject <strong>to</strong> payroll deduction from your final paycheck. Your pay will be deducted for<br />
<strong>the</strong> <strong>to</strong>tal dollar value of all costume items issued <strong>to</strong> you.<br />
In addition, <strong>the</strong> costumes issued <strong>to</strong> you are not <strong>to</strong> be given, sold, loaned, leased, rented or transferred <strong>to</strong> any<br />
o<strong>the</strong>r person. Fur<strong>the</strong>rmore, failure <strong>to</strong> return <strong>the</strong> costumes issued <strong>to</strong> you may be considered <strong>the</strong>ft of Company<br />
property and may result in disciplinary action, up <strong>to</strong> and including termination. Additionally, your use of <strong>the</strong><br />
costumes issued <strong>to</strong> you is governed by <strong>the</strong> policies set forth by <strong>the</strong> <strong>Walt</strong> <strong>Disney</strong> <strong>World</strong> Co. relating <strong>to</strong> <strong>the</strong> use<br />
