You must complete and return this <strong>form</strong> with your registration agreement. Your answers to these<br />

questions will be used for reporting purposes by The Open University and the Higher Education<br />

Statistics Agency (<strong>HESA</strong>). The in<strong>form</strong>ation you provide will not affect your access to OU courses<br />

and services, but you do need to complete this <strong>form</strong> in order to register.<br />

The Open University, like all UK universities, is required to collect in<strong>form</strong>ation about its students for<br />

reporting to the Higher Education Statistics Agency (<strong>HESA</strong>). The University also uses in<strong>form</strong>ation<br />

about its students to ensure that all our services are accessible and that we achieve fair outcomes<br />
