It will include the session, course title,and room assignment.I hereby give my permission for the named childto parti cipate in programs offered by the Altoona Parks& Recreation Department and the School District ofAltoona. I also certify that the named child is in normalhealth and capable of participating safely in the programshe/she is registered in. Furthermore, I recognize andacknowledge that there is the possibility of physical injuryin these programs and do hereby agree to save harmlessthe City of Altoona and its employees, the School Districtof Altoona and its employees and sponsors of programsoffered by the Altoona Recreation Department from andagainst all liability, damage or claims of whatever naturearising for injury to the named child as are result of his/herparticipation in these programs.Classes may fill up or cancelled and registration isdeter mined on a first come first serve basis, therefore Igive permission for the Altoona Parks and Recreation tochange my child’s schedule at any time.
