Application in plain paper as per prescribed form annexed herewith alongwith attested photocopies oftestimonials and relevant certificates etc. with self attested photo pasted in the specified space in the formto be submitted by the eligible candidates by 19 th August’2011 (4-00 p.m.) to the State TB Officer,Programme Officer of RNTCP, Swasthya Bhawan, A-Wing, 2 nd Floor, GN-29, Sector-V, Salt Lake City,Kolkata-700091 by ordinary post or in person. The List of Short-listed candidates for interview will bedisplayed in the website www.wbhealth.gov.in on 25/08/2011.N.B. :1. Application form not properly filled in or incomplete in any respect are liable to be cancelled.2. Applicant must not submit more than one application for a particular post.3. If any stage even after issue of a letter of recommendation for appointment a candidate is found ineligiblein terms of notice, his /her candidature will be cancelled without further reference to him/her.4.
