Please write clearlyand sign the form ensuring you have clearly writtenyour <strong>OSHC</strong> policy number on the formAttach your receipts to the claim formPost the claim form, original tax invoices andreceipts directly to Allianz Global AssistanceIt is important that you keep a copy of all your invoicesand receipts.For paid accounts onlyCash claimsA Cash claim is a convenient way of processing your claimon‐campus. Cash claims are available at most major institutions.There is a limit of $105 per invoice, and your medical bill musthave already been paid by you.Unpaid accountsIf you have not paid your medical or hospital bill, the benefit willbe paid:• to the nominated health care provider (eg. doctor or hospital).You are responsible for any ‘co-payment’ payable to the provider.In some instances our claims officers will contact you to requestmore information.
