Teamworks Dog Training, llcRELEASE AND HOLD HARMLESS AGREEMENTI underst<strong>and</strong> that attendance at a dog training class <strong>is</strong> not without r<strong>is</strong>k to myself, members of my family or guests whomay attend, or my dog, because some of the dogs to which I (we) will be exposed may be difficult to control <strong>and</strong> may bethe cause of injury even when h<strong>and</strong>led with the greatest amount of care. I hereby waive <strong>and</strong> release Teamworks DogTraining, its officers, employees, owners, members, <strong>and</strong> agents from any injury or damage resulting from the action ofthe dog, <strong>and</strong> I expressly assume the r<strong>is</strong>k of any such damage or injury while attending any training session or while onthe training grounds or the surrounding area thereto. In consideration of <strong>and</strong> as inducement to the acceptance of myapplication for training I hereby agree to indemnify <strong>and</strong> hold harmless Teamworks Dog Training, its officers,employees, members, <strong>and</strong> agents from any <strong>and</strong> all claims, or claims by any member of my family or any other personac<strong>com</strong>panying me to any training session or while on the grounds or the surrounding area thereto as a result of anyaction of any dog, including my own.I agree to abide by the policies of Teamworks Dog Training l<strong>is</strong>ted below:GENERAL POLICIES: Any canid that <strong>is</strong> part wild canid <strong>and</strong> that <strong>is</strong> not 100% domestic dog will not be allowed in class. Any dog which, in the opinion of the training instructor, <strong>is</strong> uncontrollable, unpredictable, or aggressive willbe removed from a core class <strong>and</strong> referred for private instruction or specialty behavior classes. There should be one primary h<strong>and</strong>ler who attends every class, <strong>and</strong> up to one ass<strong>is</strong>tant. Anyone aged 7 -17 participating in a core class must have a parent or responsible adult as a primary h<strong>and</strong>leror as an ass<strong>is</strong>tant during every class. The exceptions to t<strong>h<strong>is</strong></strong> policy are Kids Agility <strong>and</strong> Teamworks KidsCamp, where kids may be dropped off for the class (due to the greater ratio of instructors to students, <strong>and</strong> thetraining requirements for the participating dogs). Family members other than the primary h<strong>and</strong>ler <strong>and</strong> ass<strong>is</strong>tant are wel<strong>com</strong>e to attend, but for cons<strong>is</strong>tencysake, should watch the class <strong>and</strong> then participate in training at home, under the direction of the primaryh<strong>and</strong>ler. Children under 7 years of age must be ac<strong>com</strong>panied by a non-participating adult at all times while on theprem<strong>is</strong>es. Primary h<strong>and</strong>lers over 18 years of age are required for the Dominant Dogs <strong>and</strong> Fearful Fidos classes.Children <strong>and</strong> v<strong>is</strong>itors are not allowed in the Dominant Dogs <strong>and</strong> Fearful Fidos classes, due to the nature ofthe behavior of the dogs in the class. Children <strong>and</strong> v<strong>is</strong>itors are wel<strong>com</strong>e at all classes except the DominantDogs <strong>and</strong> Fearful Fidos classes, under the conditions stated above. All dogs must remain on leash when on the prem<strong>is</strong>es of the training facilities. All owners must clean up after their dogs when on the prem<strong>is</strong>es of the training facilities. Female dogs in heat are not allowed in the building or on the training grounds. Owners may request atransfer to the next available class at no charge. From our many years of experience, we find that it <strong>is</strong> often counterproductive to have two dogs from thesame household in the same beginner level class. T<strong>h<strong>is</strong></strong> <strong>is</strong> also true for two dogs that know each other verywell. We re<strong>com</strong>mend that you give each dog the benefit of your full attention in class, <strong>and</strong> we find that t<strong>h<strong>is</strong></strong>helps you achieve the same back from your dog. All dogs must remain on leash when on the prem<strong>is</strong>es of the training facilities. All owners must clean up after their dogs when on the prem<strong>is</strong>es of the training facilities. Female dogs in heat should not attend class. Teamworks Dog Training, llcABSENTEE POLICY:Owners should sign up for a class session that they can attend.In fairness to everyone, <strong>and</strong> in an effort to keep class sizes cons<strong>is</strong>tent, MAKE UP CLASSES WILL NOT BEAVAILABLE. If you know you are going to m<strong>is</strong>s a class, <strong>and</strong> would like to schedule a private lesson with yourinstructor to be tutored on the m<strong>is</strong>sed class, private lessons are available at $25 per half hour, scheduled at yourinstructor's convenience. Please contact your instructor directly for private lessons.Attendance by the primary h<strong>and</strong>ler at orientation <strong>and</strong> the first three classes of a CGC-I class <strong>is</strong> absolutely critical forthe dog's success in the class. Absentees during these first four classes may be asked to transfer to a later session at theinstructor's d<strong>is</strong>cretion.Any absence at an agility class will have a significant effect on the dog's progress <strong>and</strong> will put that dog behind the restof the class, thus slowing the progress of the entire class. If you know you are going to m<strong>is</strong>s a class, <strong>and</strong> would like toschedule a private lesson with your instructor to be tutored on the m<strong>is</strong>sed class, private lessons are available at $25 perhalf hour, scheduled at your instructor's convenience. All agility goals for the class must be met before the dog canmove on to the next class level. For t<strong>h<strong>is</strong></strong> reason, if the h<strong>and</strong>ler <strong>is</strong> absent for more than two classes, the h<strong>and</strong>ler will beasked to transfer to the next session to repeat the class.REFUND POLICY:Full credit/transfer/refund if Teamworks Dog Training cancels class.Full credit/transfer or refund minus 10% non-refundable processing fee if client cancels 14 calendar days or more inadvance of class start date.Credit/transfer/refund minus 15% non-refundable processing fee if client cancels less than 14 days in advance of classstart date.Clients who transfer after the start date of a class will be chargeda 20% non-refundable processing fee.If client cancels after starting class, the refund will be prorated <strong>and</strong> 20% non-refundable processing fee will also bededucted.All cancellations <strong>and</strong> transfer requests must be received in writing.All classes are subject to changes in schedule due to inclement weather <strong>and</strong> occasional conflicts in the instructor'sschedule.
