methods <strong>of</strong> vag<strong>in</strong>al substitution <strong>in</strong>clude passive dilation, <strong>in</strong>laysk<strong>in</strong> grafts, rotational myocutaneous flaps, <strong>and</strong> bowel substitute<strong>vag<strong>in</strong>oplasty</strong>. The American College <strong>of</strong> Obstetricians<strong>and</strong> Gynecologists committee op<strong>in</strong>ion on adolescent healthcare (34) published that the condition can usually be successfullymanaged <strong>in</strong> a conservative way by the use <strong>of</strong> vag<strong>in</strong>al dilatorsas long as the patient is sufficiently motivated. Ifa surgical option is considered, a number <strong>of</strong> approaches areavailable; with the Abbe-McIndoe operation be<strong>in</strong>g the mostcommon.The Abbe-McIndoe technique (29) is <strong>in</strong>deed considered tobe a valid treatment option for <strong>vag<strong>in</strong>oplasty</strong>, but no consensushas been reached on what material should be used for the neovag<strong>in</strong>acanal wall l<strong>in</strong><strong>in</strong>g (27). Various authors have reportedon the use <strong>of</strong> several artificial or biologic materials to coverthe neovag<strong>in</strong>al cavity <strong>and</strong> <strong>in</strong>duce epithelialization. The most relevant seriesare presented <strong>in</strong> Table 3. One <strong>of</strong> the first attempts <strong>of</strong> vag<strong>in</strong>alreconstruction from amniotic membranes was <strong>in</strong> 1934 <strong>in</strong> theFrench-language literature, where Br<strong>in</strong>deau used <strong>amnion</strong> toconstruct a neovag<strong>in</strong>a for a patient with mullerian agenesis(39).In the present analysis, we report very good <strong>anatomic</strong> <strong>and</strong>functional <strong>results</strong> <strong>of</strong> <strong>amnion</strong> <strong>vag<strong>in</strong>oplasty</strong> similar to <strong>results</strong>from previous reports. These f<strong>in</strong>d<strong>in</strong>gs confirm those obta<strong>in</strong>edby many previous authors apply<strong>in</strong>g the same method (Table3). In a total <strong>of</strong> 68 reported patients (14, 16, 20, 30, 36,38), all major complications were related to rectal <strong>in</strong>jury (3cases, 4.4%) with otherwise very satisfy<strong>in</strong>g <strong>anatomic</strong> <strong>results</strong>.Of great importance is the fact that follow<strong>in</strong>g diagnos<strong>in</strong>g <strong>of</strong>vag<strong>in</strong>al agenesis <strong>in</strong> adolescent girls, it is important to wait untilthe patient is ready to engage <strong>in</strong> sexual activity before <strong>in</strong>itiat<strong>in</strong>gany k<strong>in</strong>d <strong>of</strong> treatment. For those patients who havea 2–3-cm hymenal fossa, Frank’s method <strong>of</strong> progressive vag<strong>in</strong>aldilation should be always <strong>of</strong>fered (8), but the patientshould be <strong>in</strong>formed that success has proven to be variable<strong>and</strong> unpredictable <strong>and</strong> that the patient needs to be highly motivated<strong>and</strong> will<strong>in</strong>g to cont<strong>in</strong>ue long-term dilations (21, 41).Effective management should, moreover, <strong>in</strong>clude a carefulcomprehensive psychologic preparation <strong>and</strong> support <strong>of</strong> thepatient.We believe that vag<strong>in</strong>al construction from amniotic membranescarries the follow<strong>in</strong>g advantages over other operativereconstructive methods.
