Implement and audit area-safety-concern log book.N. Accept responsibility to address issues, pursue unsafe practices, and conditions tocorrection.O. Handling safety concerns.1. Encourage all employees to relay safety concerns/complaints to their immediate supervisorfor correction.2. If a concern is brought to your attention, inquire to find out if the employee has discussedit with his/her supervisor. If the supervisor has not been contacted, encouragethe employee to accompany you to discuss the safety concern with the supervisor andhelp develop a solution.3. Review safety concern with the supervisor in charge to develop a satisfactory solutionor response. Time allowed to implement the solution and/or develop a response shouldbe related to the magnitude/complexity of the problem and should be reasonable dependingon the situation. A sincere effort must be made to resolve the safety issue atthe supervisor level before proceeding any further.4. The employee initiating the safety concern should be contacted, if not involved, andinformed of the outcome of the review with the supervisor.5. If the response or corrective measures resulting from the review with the supervisor arenot satisfactory and cannot be resolved, the department superintendent should becontacted.Continued on next screen

The intent to proceed to the next higher level and generatea “greenie” should be communicated.6. The “greenie” should be completed and copies distributed per instructions on theform. The “greenie” should be reviewed with the department superintendent for his/her response/input. Copies of the green slip should be sent to the safety supervisorand union head safety committee representative.7. If the safety issue is not resolved at the superintendent level, the issue is to be investigatedand reviewed by the department Joint Safety and Health Committee.8. If the safety issue is not resolved at the department Joint Safety and Health Committeelevel, the safety superintendent and head union safety committee representativewill investigate with involved parties. The issue will be reviewed with the Joint Safetyand Health Committee or pursued by the head union safety committee representativeand safety superintendent.P. Handling Imminent Danger Situations:1. When an unsafe condition exists, or practice involving an imminent danger situationwhere there is a significant likelihood of serious injury or equipment damage occurring,the supervisor in charge and head shift safety committee representative shouldbe contacted immediately. Individual workers also have the ability/responsibility tostop work in imminent danger situations; especially where there is a potential forserious injury!2. The department superintendent, union head safety committee representative and safetysuperintendent should be contacted if the issue is not resolved.
