homelessservices<strong>People</strong> <strong>Incorporated</strong>’s <strong>Homeless</strong> <strong>Services</strong>programs assist chronically homeless adultswith mental health and chemical dependencyissues in achieving their goals related to safety,stability, and recovery. The transition from livingon the street to living in permanent housingis made in stages, and staff works to create anenvironment where individuals feel at ease andout of danger. <strong>Services</strong> are delivered in a flexiblemanner to meet each individual’s unique needs.Our servicesOutreachStaff goes into homeless camps to provide survival gear,assistance with basic needs, and referrals for educational,vocational, and health-related services.Housing Support / Case ManagementWe assist individuals in applying for housing assistance,and provide ongoing housing support to assist individualsin maintaining housing and achieving their goals related toeducation, vocation, and health.AssessmentsStaff conducts physical and mental health assessments asneeded. These assist both staff and clients in determininggoals and planning treatment.GroupsWe offer a variety of group therapy services, includingtopical, process, and treatment; psycho-education aboutmental illness; resources for emotional support; curriculumbasedteaching and practice for independent living skills;and social skills enhancement.Who we serveStaff serves single adults who are experiencing mentalillness and who have been living unsheltered for a yearor more.ReferralsIndividuals are referred to <strong>Homeless</strong> <strong>Services</strong> programsthrough the outreach referral and information line at651.225.8963, Ext. 2.Payment and reimbursement<strong>Services</strong> are paid for through Group Residential Housinggrants, client rental payments, and Medical Assistance.Our StaffStaff includes a Registered Nurse, LICSW, bachelor-levelcase managers, and mental health practitioners.About our facilitiesAll programs and facilities are conveniently located close topublic transportation or major freeways.ourgoalsWe provide the highest-quality mental health care in order to help individuals optimize their mental healthand wellness, and develop strategies to help them live as successfully as possible within the community.
