John Dempsey HospitalDepartment of NursingPage 1 of 3Herrick Drake Scholarship FundHistory:The Herrick Drake Scholarship Fund was established in 1984 from donations in memory ofHerrick Drake, who experienced an unexpected cardiac arrest. Mr. Drake's wife, Judith, andother family members wished to acknowledge the efforts and contributions of nurses in the careof patients with cardiac disease. The Fund has now matured and is ready to be utilized forscholarship awards.Purpose:The Herrick Drake Fund Scholarship will be used to support individual nurses' continuingeducation activities, with preference given to nurses studying cardiac-related topics. Attendanceat Regional/National Conferences will be encouraged.Eligibility:1. Clinical Nurse 2, Clinical Nurse 3 or Clinical Nurse 4 in Department of Nursing.2. Letter of support from Nursing Manager of unit.3. Demonstrated contribution to quality patient care through clinical activities.4. Completed application (including description of educational program to be attended andanticipated expenses). Preference will be given to cardiac-related topics.Scholarship Funding:1. Approximately $840.00 in scholarship monies will be available in 2012.2. Applications with expenses ranging from $250 - $840 will be considered. More than onescholarship may be awarded annually.

Upon completion of educational program, documentation of attendance must be submitted.3.
